This is a short interview of the author on a recent retrospective review that revealed no increase in cardiovascular events in men.  The author rightly points out his data, which is quite different than the previous JAMA and PLOSone publications which showed an increase in cardiovascular events.   However, the lead author points out the main problem with comparisons:  his study group was younger and healthier.  So, the comparison is apples to oranges.  Studies still point to increased risk of Testosterone therapy in men with pre-existing CVD.  This new study does not refute this point at all.   Another serious flaw here, is that only Testosterone was followed.  This logic is seriously flawed as I have previously documented.  The author points out the flaw in the levels in the JAMA study post treatment.  But he fails to account for the the lack of adequate pathway assessment i.e.aromatazation.  Also, no inflammatory cytokine evaluation was performed in that study.  Both of these should have been highlighted.    In contrast, a positive was the length of follow.

Declining Testosterone, calculated free Testosterone, Estradiol, and Estrone levels appear to be associated with muscle mass/strength decline in elderly men.  This points to a global decrease in HPA function rather than an individual, specific hormone deficiency.

Abstract published ahead of print finds that DHT and calculated free DHT was associated with CVD and all-cause mortality.  This cohort found that Total Testosterone and calculated free Testosterone were not.  

Retrospective study finds no increase in new MI and/or stroke in men with Testosterone therapy.  There are some men that Testosterone therapy is cardioprotective and some that are not.

In men <45, a declining Testosterone level is associated with an increasing incidence of Metabolic syndrome.  This association was independent of age.  Testosterone was positively associated with HDL and inversely associated with BMI, waist circumference, Triglycerides, Trigylceride:HDL cholesterols, and glucose.

Total Testosterone and SHBG inversely associated with metabolic syndrome in wide age group of Korean men.  This was found to be independent of age and BMI. 

Small study found that 2 months of clomid therapy increased LH, FSH, total Testosterone, and free Testosterone over placebo.  Improvement in ED was minimal and most in young men.  This is consistent with other studies that have found that less effects are seen in those with multiple comorbidities.

Hypogonadism appears to be associated with Celiac disease in men.  Infertility appears to be an association with women.

Only abstract available here.  I can't find the full print.  For some, there is inherent thrombosis risk with Testosterone. HOWEVER, it is due to metabolism of Testosterone i.e. aromatase activity, and to supra physiologic dosing that is prevalent through the medical community.  For those with pre-existing cardiovascular disease, a full biochemical evaluation of cardiovascular risk must be undertaken prior to the initiation of Testosterone therapy.
