    LC-MS/MS shown to be "accurate and highly sensitive" when compared to GC-MS for saliva testing of hormones.  Up to this point, GC-MS has been considered the "gold standard".

    Great review on the literature of LC-MS/MS and now the new ESI-MS/MS method in saliva hormone testing. This technique is highly sensitive and specific. This article goes through the literature support for each hormone. It also discusses how/why saliva testing is valid. Conclusion: "the author believes that the saliva-based hormone test will be integrated into clinical practice in the near future".

    LC-MS/MS shown to be useful in evaluation in people with suspected elevated cortisol levels.  In this study, they found the dexamethasone suppression test was required.  The point however is that cortisol is a valid medium and this was with LC-MS/MS instead of the more commonly used ELIZA technique.

  • It is now well recognized that the disease manifestation is reduced in pregnant women with relapsing-remitting MS

    good read on an alternate hypothesis of the pathophysiology of MS. THe author proposes that MS is an inflammatory induced dysfunction of lipid metabolism. This is in contrast to the current held dogma that MS is an autoimmune disease.

    Pregnancy has a positive effect on MS patients, especially during the 3rd trimester.  Rebound of MS increases in the postpartum period.  Estriol is the dominant pregnancy estrogen.  Progesterone is also produced at high levels.  The withdrawal of these hormones in the postpartum period are likely to lead to this rebound in MS.

    salivary testosterone and DHEA using LC-MS/MS accuracy exceeds 98.5%.  Compare that with historical 50% resolution of pain with a hysterectomy.  There is no perfect test, but if accuracy exceeds 98%, you have  reliable test.

    Granted, this study was in an animal model of MS, but signaling through ER-beta was shown to decrease inflammation from astrocytes and microglial cells in the brain.  These immune cells are known to play prominent roles in excitotoxic diseases such as MS.  So, again, it is not just about the message, but also about how the message is interpreted.

    MS-LS show to be highly sensitive in the dual identification of testosterone and cortisol in saliva. These hormones were found to be stable up to 28 days at room temperature. This disproves the argument that saliva, as a test medium, has a very short shelf life.

    LC-MS for salivary testosterone and DHEA validated.  Also, T and DHEA were shown to have specific diurnal rhythms.  Changes in levels were also followed after DHEA therapy.

    Relapsing remitting MS patients found to have in increased 8OHdG, reduced glutathione, reduced glutathione to oxidized glutathione ratio, super oxidized disumutase, glutathione reductase, and global oxidative stress.  In contrast, decreased total antioxidant capacity, oxidized glutathione, glutathione perioxidase, and glutathione transferase were found in relapsing remitting MS patients.
