This article, abstract only, looks at the cause of low Testosterone.  It is well known that low T increases CAD in men.  But what causes the low T?  Is the "normal" decline in testosterone actually normal or a product of accumulative dysfunction.

In obese men, low T is the result of increased estradiol formation from aromatase activity.  Low SHBG is no enough to offset the decrease in free T levels.

As I have been saying for awhile now, low Testosterone is the effect not the cause: new study finds that low T is a biomarker of diabetes and not a cause.  

Interesting study. In this study group, 36% of type II diabetes with ED had low Testosterone.  This fits with the estimated 40% of those with Diabetes have low T. What is really interesting about this study is that the degree of improvement with ED by Sildenfafil was dependent on the Testosterone level in the low T group.  Meanining that Testosterone therapy in these men would probably be more of a therapy directed at the cause and not Sildenafil.  Of course, the Testosterone therapy would benefit glucose regulation as well documented in the literature.

Startling study on the knowledge of Testosterone and cardiovascular disease in general practitioners and cardiologists in Canada.  Eight-eight percent did not screen patients with cardiovascular disease for low Testosterone.  A whopping 67% of physicians did not know that low T was a risk factor for cardiovascular disease, yet 62% believed Testosterone would increase exercise tolerance. The lack of knowledge displayed by physicians today is staggering and is an indictment of the governing bodies.  This was a survey conducted in Canada so there are obvious limitations to the strength/conclusion of this study.

Interesting study finds that men with low serum Total Testosterone, elevated RBP4 and low SHBG at baseline predict weight regain.  Thus Testosterone should be used as a biomarker of failure in weight loss and if low, Testosterone therapy should be employed to improved metabolic function.  Other parameters, such as leptin, adiponectin, prolactin, progesterone...were not predictive.

pubmed review of 33 years finds mix bag on low T. Low T clearly associated with increased hypertension, dyslipidemia, atherosclerosis, thrombosis, cardiac dysfunction, and CV events.  Studies on Testosterone therapy in resolution of these dysfunctions are low.  This article claims there is a lack of evidence.  That is not true.  

Increasing glucose levels associated with declining LH pulses in men with type II diabetes.  This is one proposed mechanism for low T in men.  There has been great debate about if Low T was in part the cause of Diabetes or an effect of diabetes.  This proposes that low T is due to a decreased LH pulse as a result of rising glucose.

low Progesterone and progesterone "disturbances" found to be common in juvenile Lupus.  The authors in this study proposed it was due to low follicular reserves.  The point is that low progesterone increases inflammatory cytokines.

Study of Korean men finds late onset of hypogonadism to be 16.8% in their study of 1,235 men older than 40.  This is a higher rate than other studies, but lower than others (38.7% in American men).  The test definition of low T was set at 3.2 ng/ml.  Abdominal obesity, metabolic syndrome, diabetes, and increasing age were associated with low T. Low T is clearly not a problem unique to America.  It definitely appears that a decreasing Testosterone level is a marker of poor health in men.
