testosterone therapy has tremendous cardiovascular benefit in men with low T.  The key here is physiologic replacement of Testosterone.  Testosterone is a vasodilator and anti-inflammatory agent in men with low T.  Testosterone therapy improves cardiac function in those with DHF and angina.  Testosterone is found to be a Ca++ channel blocker--anyone say hypertension treatment?

Great confusion exists in the medical profession about Testosterone and PSA and the health of the prostate. The conversion of Estrogen, whether E2 or E1, and other variables are responsible for increases in PSA while on Testosterone therapy. This study points out that Estradiol in men stimulates cell line growth of prostate cancer. In contrast, Epitestosterone, an androgen metabolite, has antiandrogen, inhibits this estrogen activity. Epitestosterone exists in an inverse relationship to Estradiol and IGF-1.

Study of Korean men finds late onset of hypogonadism to be 16.8% in their study of 1,235 men older than 40.  This is a higher rate than other studies, but lower than others (38.7% in American men).  The test definition of low T was set at 3.2 ng/ml.  Abdominal obesity, metabolic syndrome, diabetes, and increasing age were associated with low T. Low T is clearly not a problem unique to America.  It definitely appears that a decreasing Testosterone level is a marker of poor health in men.  

Again, Testosterone and here Estradiol are merely there for libido and sex.  What tunnel vision?!  What about hsCRP?  What about fibrinogen?  What about IL-1beta?  What about TNF-alpha?  These inflammatory cytokines have all been reported to elevate as a result of estrogen production in men.   And PSA?  No mention of it here.   This linear, tunnel vision thinking on hormones has got to stop! The study points out that all clients were using AIs and SERMs irregardless of whether they had elevated estrogens or not.  That is not a well designed study.  One group should have had AI's if elevated estrogens were present and another group should not--this would compare the effects of aromatase activity.  Second, this was simply a retrospective chart review.  Third, a 50% conversion of 34,000 + men is very high when you look at the literature.  Fourth, they point to gynecomastia as a means of negative?  The cardiovascular implications are more significant.  These studies just seem to focus on superficial things.  Fifth, did libido problems exist before?  What were the free levels?   This falls in the paucity of data (2 studies) that point to excessive lowering of estradiol effecting libido and sexual performance.

Review of low T, evaluation, and health benefits of physiologic Testosterone replacement.

So this is what passes as science these days.  The study looked at whether pre-therapy Testosterone evaluation was beneficial in men prescribed Testosterone therapy.  What?!

Weight loss increases Testosterone levels.  This author points to low Testosterone as merely a biomarker of poor health.  In some this is correct, but Testosterone can have significant positive effects.

Australian study of older men, 70-89, found that normal Testosterone and DHT levels were associated with lower death rates.  This is important as this is the first study to find the positive health benefit of DHT as well as Testosterone.  

Men with infertility associated with 2.3 fold higher risk of death. The factors evaluated were semen volume, sperm concentration, sperm motility, total sperm count, total motile sperm count.  Those that have 2 or more positive parameters were associated with increased death compared to those with normal semen analysis.

Systemic illness i.e. inflammation is associated with impaired HPA function and gonadal function.  This can be translated as low T.  Low Testosterone is a biomarker of poor health in men.

Testosterone levels appear to be more of a biomarker of poor health in older men that are obese with diabetes.  More of these men had low free Testosterone (calc) versus Total Testosterone.  Using QOL questionnaires, weak correlation (not statistically significant) with androgen deficiency symptoms and ED were found.

low SHBG found to be more associated with metabolic syndrome in older men than low Testosterone.  The study consisted of 2502 men older than 70.  The authors proposed this occurs through the relationship between SHBG and insulin sensitivity.  I think that is a little too simplistic.

Opioid use, despite type (including methadone), is associated with lower Testosterone in men.  This is not new information, but this meta-analysis reconfirms this relationship, particularly that methadone has the same effect.  This relationship was not found in women.  These physicians that regularly prescribe opiates  to men are doing them an incredible disservice.  A lower T in men is a significant negative health biomarker.
