Insulin is a hormone produced by the pancreas to control blood sugar. Diabetes can be caused by too little insulin, resistance to insulin, or both. To understand diabetes, it is important to first understand the normal process by which food is broken down and used by the body for energy. Several things happen when food is digested: A sugar called glucose enters the bloodstream. Glucose is a source of fuel for the body. An organ called the pancreas makes insulin. The role of insulin is to move glucose from the bloodstream into muscle, fat, and liver cells, where it can be used as fuel. People with diabetes have high blood sugar because their body cannot move sugar into fat, liver, and muscle cells to be stored for energy. This study, a review of 16 articles, found little scientific support for the "blood type" diet.  This is why I have not made this recommendation to our clients--no real data to support this diet.   One question, is what was the eligibility criteria for inclusion of the studies.  Only one out of the 16 was selected.  Inclusion/exclusion criteria in these studies can brian bias.

Systematic review finds very little clinical benefit from Testosterone therapy.  The problem here is one typically made by allopathic medicine.  They look for a benefit in a box.  They look for one therapy to cure all.  Testosterone therapy should be the last thing you prescribe as usually low Testosterone is the effect and early is not the cause.  Clearly, in the right individual, Testosterone therapy can have tremendous positive effects; however, it must accompany nutrition, exercise, inflammation control...  Testosterone is just one piece of a very complex male physiologic puzzle.

Not surprising, but nice to see in study.  Dr. McTiernan showed that weight loss as little as 5% reduces breast cancer risk.  The reason?  Decreased Testosterone production in women due to insulin resistance and decreased fat for aromatase conversion of Testosterone to Estradiol and decreased androstenedione to estrone.  This study confirms the importance of the obesity epidemic to health.

LC-MS/MS shown to be reliable in salivary cortisol evaluation.  Obese individuals create special challenges with regards to coritsol levels, due to peripheral cortisol to cortisone conversion.   This study points out that saliva should not be used solely in the diagnosis of Cushing's syndrome due to unreferenced norms.   That however, doesn't nullify salivary cortisol.  It requires a look at the references.  for those that are obese and being evaluated for cushing's.  There is no perfect test.  T This study is a little misleading in its conclusion.  Salivary testing is valid, but the reference ranges in the obese with suspected cushing's syndrome needs further evaluation: according to these authors.

this study revealed increased breast cancer risk in women on "estrogen" and "testosterone" therapy.  Now, several problems here: first, are these synthetic hormone or bioidentical.  Second, the dosages appear, in what is written, to be supra physiologic.  Third, giving supra physiologic estradiol and testosterone will obviously create imbalances and growth potential.  Fourth, how were the women evaluated prior to starting hormone therapy and then were they remonitered (unlikely), fifth, were hormone metabolites evaluated (too, also unlikely).  This study has serious flaws and very little can be extrapolated other than: don't take supra physiologic hormone levels without appropriate evaluation.  Enough said

Nice review of the mixed data on Testosterone and Prostate disease. It is clear that Testosterone does not precipitate prostate cancer.  The intraprostatic hormone milieu likely is different than that present in the serum.  No surprise there.  5alpha reductase decreases prostate volume, PSA, and low-grade prostate cancer, but actually increases aggressive prostate cancer. Supraphysiologic doping in young men associated with no increase in prostate disease. PSA no longer to be followed in men < 55.  Mortality rate not changed.  PSA change of 1.4 ng/ml is appropriate for additional prostate evaluation.  Testosterone therapy on average increased 0.5 ng/ml. Still, no mention of aromatase activity in this article.  Why is it that hormone sensitive disease in men is only with regards to androgens and women estrogen.

No correlation seen between serum hormones and prostate cancer risk.  Serum evaluation of hormones provides little, if any, clinically, relevant information.  The only association was found with a DHT metabolite--androstanediol.
