LC-MS/MS overcomes the limitations of immunoassays in hormone testing.  LC-MS/MS is the new gold standard with sensitivity, specificity, and ease second to none.  LC-MS/MS is the bench science now coming to the practice of medicine.

The long term effect of lowering cholesterol MAY stabilize plaques (not reduce), but at the expense of the brain. So, we may all have stable plaques, but we just won't know it because our brains will be fried.

knowledge of progesterone and its metabolites are important in the use of hormones.  As the authors concluded here: "natural steroids should not be disparaged...an update on endocrinological knowledge and experience is rather mandatory for gynecologists".  There in lies the problem.  The IOM has shown that the average physician practices at a level of 17 years behind the current scientific knowledge and the environment created by medical governing bodies discourages questions, so as to protect the system.

This study from 2004 reveals the marketing-based medicine era that we live in.  This meta-analysis of studies conducted for 5 year survival benefit found the contribution of chemotherapy was 2.3% in Australia and 2.1% in America.  The authors conclusion: "it is clear that cytotoxic chemotherapy only makes a minor contribution to cancer survival".  Compare this to the chemotherapy harm and the balance of benefit versus harm reveals a complete lack of evidence with regards to how therapy is practiced in Cancer

Study finds poor effectiveness in Hepatitis B vaccine  in prevention of HVB in children born to infected mothers.  The author's conclusion says it all: "the current practice of administration of vaccine with HBIG at birth to babies born of HBsAg-positive mothers is not effective in preventing occult HBV infection in babies.

This study confirms what we know about Testosterone, but this study finds that Estradiol aids libido and fat loss.  The conclusion on Estradiol I believe to be extremely premature.  First, it flies in the face of all the accumulative data on estradiol, second, what normal physiology is being replicated with goserelin???  Goserelin has been shown to decrease Prolactin which can effect libido also.  What about the potential there?  The men included in the study were described as "healthy".  So, you are taking "healthy" normal funcitoning men, throwing in a monkey wrench and looking at the effects of your monkey wrench.  Sorry, not physiologic.  In all my practice, I have seen one man with low Estradiol levels.  There is no reference to the hormone levels in the men preceding the suppression with goserelin.  This is a study that lacks application.

This study reveals what is wrong with a lot of the "standard practice" and literature in medicine today.  At 5 years, almost 50% of weight loss with gastric sleeve is regained--defined as excess weight loss (only 50% weight loss of excess weight).  That is a terrible 5 year outcome; not to mention the definition of success.  Even at 3 years, 31% had regained their "excess weight".  Add to that, the numbers of patients that followed up was terrible.  This poor follow up points to worse data outcomes than this study claims.

What is "evidence-based medicine"?   Most recommendations and clinical practices are based on the lowest level of evidence: essentially opinion-based therapy.

Physical activity in colon cancer patients improves survival compared to increased mortality with increase TV viewing.  We physicians must increase the practice of exercise prescriptions.

I am not a fan of guideline medicine, but this review of the literature on low T is worth while.

we need to get away from expert opinion statements like this. Not that their conclusions are incorrect; but it promotes laziness among physicians.  We physicians need to take control over the data by which we practice medicine and the only way we can do that is to read it.
