    Great review of the current understanding of thyroid hormone metabolism in cardiac tissue.  Low T3 and increased rT3 (via increased D3 activity) is CLEARLY associated with poor cardiac performance and post MI and CHF is associated with poor outcomes.  T3 is critical in cardiac remodeling and recovery post MI.  T3 is actually a vasodilatory in the coronary arteries.   Why a endocrinologist would call rT3 useless only points to their ignorance of the literature.

    Another study regarding reverse T3 that many Endocrinologists need to read.  In this study, the authors find that significantly higher reverse T3 levels were associated with myocardial function impairment (MFI) class IV.  This MFI class IV was also found associated with a decreased fT3/rT3.

    Some startling statistics in this 2013 review on Testosterone in men.  Studies reflect an inverse relationship between Testosterone and CAD severity.  That is, the lower the Testosterone levels, the increase in severity of CAD. This same association was also found with CHF.  Low Testosterone is common in those with CAD, CHF, type II diabetes, increased IMT in carotids and aorta, and obesity when compared to "healthy" individuals.  Testosterone therapy in those with CAD found benefits: prolongation of ST segment depression, coronary vasodilation, improved exercise capacity in those with CHF, shift to type I muscle fibers, shorten the QTc interval.  Testosterone therapy has been shown to improve insulin resistance, improve HgbA1c and decrease waist circumference and fat loss in obese individuals.  Otherwise, a good review of the association between a declining Testosterone and cardiovascular disease.

    Good article.  Heart failure in men associated with lower Total Testosterone and estimated Free Testosterone. The severity of disease was associated with lower Testosterone levels.   I wish they would have measured free levels not estimated them.  

    Increased SHBG concentrations was associated increased CHF as well.

    This study found no association with mortality.

    Study finds that Testosterone therapy improves insulin sensitivity, muscle strength and function in women with advanced CHF.  No levels were noted.  If this is similar to men, I would expect a low Testosterone, which Testosterone replacement would then increase physiologic performance.
