Unless they contain BOMs, being in UTF-8 often doesn't mean much for texts containing only English.

The extension recognised by default is ".m*d*".
You can add the extensions you need yourself.

No idea why you would have a different behaviour.
I tested 4.1 with "ANSI" (i.e., ASCII) source documents, and, translating into French, I got UTF-8 target files, with the encoding set to "auto" both for source and target.

I was able to force the output to Windows 1252 (as appropriate for French) and UTF-16 without any issue.
