Drug Wars: An Epic Tale of Asthma and Bacterial Pneumonia - National Center for Case St... - 0 views

    This integrative case study is based on real events that the author experienced with her ten-year-old daughter. Although the names have been changed, all of the events (symptoms, diagnoses and treatments, types of healthcare professionals) are recorded exactly as they occurred. Both asthma and pneumonia are common in the United States (and globally). Many of the drugs described in the case study are frequently prescribed for a wide variety of ailments. The four "episodes" that constitute the case cover 1) a study of asthma triggers, incidence and treatment; 2) side effects and possible medication errors associated with steroids; 3) causes, diagnosis, treatment and prevention of community-acquired bacterial pneumonia; and 4) the mechanism of action of two different antibiotics. The "epilogue" explores a few larger themes related to healthcare and public health (e.g., continuity of care, drug resistance, asthma prevention programs). The case study could be used in an undergraduate, upper-level, infectious disease, microbiology, public health or physiology course or in a graduate-level health professions program.

    When a person dies from cancer, the culprit is usually not the original tumor but rather the cancerous cells that spread throughout the body and replicate in distant organs, a process called metastasis. Researchers have long known that metastasizing cancer cells slip their bonds and avoid immune detection by altering the sugars on their surfaces. They've even come up with a would-be drug to prevent such sugar alterations. But that compound interferes with needed sugars on normal cells, too, with lethal results in animals. Now, Dutch researchers report that they've packaged the drug in nanoparticles targeted exclusively to cancer cells, and they've shown that this combination prevents cancer cells from metastasizing in mice.

    This case study is centered on a real scenario from 2013 in which the Kickstarter fundraising platform included a fundraising proposal from a group wanting to use synthetic biology to make glowing plants. The proposal raised controversy because the Glowing Plant group offered to give away their genetically-modified glowing plant product to investors.  Kickstarter subsequently changed its policy to prevent use of genetically modified organisms as rewards or incentives to investors. This case study was developed for a college first-year seminar focused on the ethics of synthetic biology, and was introduced during the first week of the semester. The case uses the jigsaw teaching method to introduce students both to the mechanics of synthetic biology and to questions about how we should distribute the benefits and burdens associated with new or emerging technologies. It could be adapted for courses in introductory college biology, genetics or bioethics.

His parents' clashing views on Eastern and Western medicine prevent them from agreeing on a course of treatment. As the case unfolds, students follow the progression of their son's illness. After a physician is finally seen and the results of tests are evident, students should be able to narrow the list of possible etiological agents and suggest a potential treatment. In addition, the students should integrate the different health beliefs into the final treatment as well as the aspects of patient counseling. The case works well as an interrupted case that can be assigned to individual students or student teams. This case is appropriate for graduate courses with a component in health care, therapeutics, medicinal chemistry, medicinal plants, microbiology, epidemiology, or cultural competency. Instructors can choose to focus only on the medical components of the case or incorporate the cultural and ethical aspects, depending on course goals and subject area.

Students (and everyone else) may wonder where new infectious diseases come from, how scientists assess the risk of a pandemic, and how we might go about preventing one. This case study uses a PowerPoint presentation to explore these questions by focusing on HIV, a pandemic that began as an emerging disease. The storyline progresses backwards through time as scientists attempt to unravel the origins of a new, mysterious plague. Much of the case relies on audio excerpts from an episode produced by Radiolab, an acclaimed radio show that explores a variety of topics in science and culture (www. radiolab.org). Students use graphics, animations, and sound clips presented in the PowerPoint slides to discuss several sets of questions. The case is suitable for a wide range of high school and college introductory biology courses, as well as undergraduate microbiology, ethics, and public health courses.
