    The purpose of this Funding Opportunity Announcement (FOA) is to stimulate developmental research evaluating the effect of care planning on self-management of late effects of cancer therapy; adherence to medications, cancer screening, and health behavior guidelines; utilization of follow-up care; survivors' health and psychosocial outcomes.     The Agency for Healthcare Research and Quality's (AHRQ) mission is to produce evidence to make health care safer, higher quality, more accessible, equitable, and affordable, and to work within the U.S. Department of Health and Human Services and with other partners to make sure that the evidence is understood and used. AHRQ's Center for Quality Improvement and Patient Safety (CQuIPS) is addressing patient safety and medication research by focusing on the safe usage of medications.  This perspective centers on how medications move through the health care system and how this systemic process can be improved so that patients are not harmed, while health care delivery is improved.  CQuIPS encourages the involvement of all members of the health care team, especially patients and families, nurses, pharmacists, technicians (pharmacy and medication administration technicians), health care administrators, risk managers, and physicians across all settings of care, including in the home. This FOA will fund investigative research demonstration projects that examine the effective implementation of processes, policies, and behaviors that support safe use of medication as well as its sustainment and dissemination.

    The American College of Cardiology's Accreditation Services help hospitals improve care for patients with acute coronary syndromes, heart failure, and atrial fibrillation. In 2016, ACC merged with the Society of Cardiovascular Patient Care (SCPC), a nonprofit organization dedicated to improving the care and outcomes of patients worldwide facing the same complications. To preserve the legacy of SCPC, an endowment was created to support future cardiovascular quality or performance improvement initiatives, particularly those that underscore the importance of accreditation. To advance that mission, ACCF is accepting proposals that support cardiovascular quality improvement of cardiovascular care and patient outcomes.     This notice solicits applications for a new three-year Special Projects of National Significance (SPNS) initiative entitled Capacity Building in the Ryan White HIV/AIDS Program to Support Innovative Program Model Replication. This Notice of Funding Opportunity (NOFO) responds to recent changes in the HIV epidemic and the need to find effective solutions that improve health outcomes along the HIV care continuum for people living with HIV (PLWH). During recent years, a number of innovative interventions have demonstrated effective responses that contribute to ending the HIV epidemic. Ryan White providers who have developed these evidence-informed interventions have produced replication manuals and other tools to assist organizations and jurisdictions interested in adapting and/or replicating such interventions. This NOFO seeks to build the capacity of Ryan White HIV/AIDS Program (RWHAP) recipients and subrecipients to replicate effective models of care., with a particular focus on RWHAP jurisdictions identified as disproportionately affected by HIV By building capacity and encouraging the implementation or replication of effective models of care, the RWHAP intends that these jurisdictions will bring recent innovations to scale, thus improving systems of care, reducing health disparities, improving outcomes along the HIV care continuum, and ultimately contributing towards meeting the goals and objectives of the National HIV/AIDS Strategy.

    The FY17 JPC-1/MSIS TRANSfeR Award Program Announcement/Funding Opportunity is seeking research to determine whether the medical skill learned on a simulation system has a downstream beneficial effect to patients and/or the MHS in the real clinical world. The Program Announcement/Funding Opportunity seeks applications for research to demonstrate that simulation-based medical training has a measurable outcome on patient care. Previous T1 studies have shown improvement in skills in the simulated environment when deliberate practice and mastery learning (a set of group-based, individualized, learning strategies based on the belief that students will achieve a high level of understanding in a given area when given enough time) occur as part of training. The next set of studies should measure whether these same techniques translate to patient care and affect systems of care such as return-to-duty rates and morbidity and mortality statistics. Such research will involve taking the lessons learned in the laboratory and measuring outcomes in the patients who are cared for either in an operational environment or medical treatment facility. Historical patient outcome data does exist for the way medical professionals are trained now, so the variable being introduced in new studies would be simulation-based training.

    This FOA aims to increase testing of populations most affected by chronic hepatitis B and/or hepatitis C; and training of health care providers. Part A is Networking & Capacity Building and Part B is Professional Education & Training. Part A strategies & activities should focus on leading and growing an existing national coalition that is geographically diverse, is in at least different 10 states; and whose members are actively involved in viral hepatitis. The aim is to share strategies, materials, and lessons learned among coalition members; provide technical assistance to conduct culturally competent educational outreach; build the capacity to test the target populations for chronic viral hepatitis; and engage members in implementing CDC’s national viral hepatitis educational campaign(s).Outcomes for Part A are: increased number and capacity of coalition members to deliver culturally competent educational outreach to target population; increased numbers of individuals tested for chronic viral hepatitis; and increased exposure to CDC’s national education campaign. Part B activities are: developing free, accurate, up-to-date, CE-accredited comprehensive web-based, on-demand hepatitis materials, resources and trainings for health care professionals. The outcomes for part B are increasing the knowledge and skills of health care professionals, including the ability to screen, diagnose, medically manage, and provide care to people with chronic viral hepatitis.

Characterizing the Complications Associated with Therapeutic Blood Transfusions for Hem... - 0 views

    This NOFO aims to improve access to, coordination of, and continuity of health care for individuals with thalassemia or sickle cell disease which will lead to a decreased occurrence of transfusion-related complications, improved quality and increased length of life. These health care improvements will result from a multi-faceted approach that includes (1) identification of patients, families, communities, and providers who will benefit from increased knowledge about therapeutic transfusions and their potential complications, (2) development and dissemination of materials that will increase understanding of the diagnosis and management of these blood disorders, and (3) collection of biological specimens from individuals with thalassemia or sickle cell disease. This NOFO builds upon findings from the work completed as part of CDC-RFA-DD14-1406. In particular, (1) the difficulty in identifying any thalassemia patient in the U.S. and their health care provider prior to medical complications manifesting, other than those born in states where the condition is a part of the mandatory newborn screening panel, (2) the lack of standardized practices across, and sometimes within, clinical care settings and blood banks for transfusions of patients with thalassemia or sickle cell disease, (3) the need for dissemination of evidence- or consensus-based guidelines about best practices for transfusions, and (4) the room for improvement in increasing blood donation from communities most affected by thalassemia or sickle cell disease.

Funding to support these projects is made possible by the Society of Cardiovascular Patient Care (SCPC), which was a nonprofit organization dedicated to improving the care and outcomes of patients worldwide with suspected acute coronary syndrome, heart failure, and atrial fibrillation through innovative cross-disciplinary processes. Its merger with the American College of Cardiology Foundation (ACCF) in 2016 provided an opportunity to leverage the accreditation strength of the SCPC into the large portfolio of services the ACCF offers.     The purpose of this U24 Funding Opportunity Announcement (FOA) is to support a bioinformatics group (BIG) that will analyze a set of asthma-specific common data elements (CDEs) retrospectively collected from the electronic health record data (EHR) of patients in the US whose asthma is managed by a primary care provider (PCP). Based on a large, nationally representative data set, the BIG will identify the current management practices (CMPs) of PCPs that result in the best asthma outcomes for specific groups of patients in different clinical settings or situations. The CMPs will be established by the BIG in collaboration with a clinical consultant team (CCT) with expertise in asthma and a primary care network (PCN), experienced in managing the primary care of patients. Following the BIG's analysis of retrospective data, the CCT and PCN will design and then conduct prospective clinical trials to compare the retrospectively identified CMPs in appropriate patients with asthma. At the conclusion of the clinical trials, the BIG will perform the data analysis for the assessment of the CMPs, refining any of the processes needed to analyze the asthma CDE data.

    This NOFO will build on previous PEPFAR support under the HHS/CDC HIV treatment program in Côte d'Ivoire (CI) to ensure continuity of comprehensive HIV/AIDS services to an existing pool of clients receiving HIV/AIDS care, support, and/or treatment. The program will also continue expanding access to HIV/AIDS services while building the capacity of national structures and contributing to sustainable service delivery within the health sector in CI. Specifically, it serves to increase capacity and sustainability of the response toward controlling the HIV/AIDS epidemic by initially providing support for HIV service delivery aligning with PEPFAR geographic and programmatic pivots by local indigenous organizations and ultimately providing technical assistance to the national Ministry of Health and Public Hygiene (MSHP) to sustain and expand comprehensive HIV prevention, care, and antiretroviral therapy (ART) programs. The recipient(s) will combine a facility and community-based strategy to support HIV/AIDS services. At the end of the 5-year project period, the recipient(s) should be able to collect and evaluate program data that demonstrates improved quality of HIV prevention, care, and treatment services in CI and to transition activities to MSHP and/or local organizations to sustain a basic HIV service package.

    The Agency for Healthcare Research and Quality (AHRQ) is interested in funding a diverse set of projects that develop, test and evaluate various simulation approaches for the purpose of improving the safe delivery of health care. Simulation in health care serves multiple purposes. As a training technique, it exposes individuals and teams to realistic clinical challenges through the use of mannequins, task trainers, virtual reality, standardized patients or other forms, and allows participants to experience in real-time the consequences of their decisions and actions. The principal advantage of simulation is that it provides a safe environment for health care practitioners to acquire valuable experience without putting patients at risk. Simulation also can be used as a test-bed to improve clinical processes and to identify failure modes or other areas of concern in new procedures and technologies that might otherwise be unanticipated and serve as threats to patient safety. Yet another application of simulation focuses on the establishment of valid and reliable measures of clinical performance competency and their potential use for credentialing and certification purposes. The foremost aim of the announcement is to advance patient safety. Keeping this aim in mind, applications that address a variety of simulation techniques, clinical settings, provider groups, priority populations, and patient conditions are welcomed.
