Hand Function, Experienced Pain, and Disability After Distal Radius Fracture - 0 views

    During the past 12 weeks at my site, I had the opportunity to work with many patients who had experienced distal radius fractures and were coming to therapy post surgery. This article discusses patients pain level, grip strength, pinch strength, and ROM following therapy 12 months out. Patients almost reached the same ROM in the treated hand that they did in the uninjured hand. Also, patients had increased grip and pinch strength. The interesting finding was that at 12 months, patients reported decreased pain, however between the 12-24 months there was increased pain at rest and during activity. The article discusses multiple patients having pain due to a volar locking plate as fixation. However, patients who did have the plate removed had significant pain and were not as satisfied with the result either at 12 to 24 months following surgery. I found this interesting because most of my patient's who would come to me for therapy would continue to complain about pain and decreased ROM throughout our sessions. Educating patients on their limitations due to the plate and learning more about how to help individuals with a plate in the volar aspect of the arm is key to helping them

    This article discusses commonly used modalities for managing pain symptoms in the rehabilitation process. I am currently in a SNF and many of our patients have pain that limits their functional performance. We often utilize many of these modalities at the beginning of our treatment to reduce pain levels and allow for increased participation in the therapy session.

Effects of a soft prefabricated thumb orthosis in carpometacarpal - 1 views

    This article examines the effect of a prefabricated soft thumb-base orthosis on pain, hand strength, and activity performance in persons with osteoarthritis of the CMC joint. At my facility, I see quite a bit of CMC arthritis and wanted to explore the use of an orthotic since most of my pt.'s say it helps with pain and protection of their thumb. Much to my surprise, this study found that the soft prefabricated orthosis seems to have an immediate pain-relieving effect during use, but no effects in terms of less hand pain, or improved strength or activity performance when not worn. There were no significant difference found between the experimental (orthosis+exercises) and control (exercises only) groups.

    This article is one of the articles I used in a K tape in service for this rotation. I used this article as an example of evidenced based practice for my project. This systematic review focused on the efficacy of taping for pain reduction associated with rotator cuff tendinopathy. The review found 10 articles that met inclusion, exclusion, and quality standards. The review found that k-taping significantly increased pain-free range of motion in shoulder flexion and abduction, however, there is still insufficient evidence to conclusively state that k tape alone or in conjunction with other methods is effective for pain reduction in rotator cuff tendinopathy.

Both positive and negative themes emerged including: losing valued roles, having depression and receiving compensation and to knowing: being fearful, looking for a cure or not knowing what to expect, being unable to set goals and being unable to use strategies.     This article is an example of what one would use if searching for evidenced based practice methods. My current fieldwork experience is in a skilled nursing facility and we frequently use the e-stim for strengthening and pain. Therefore for an article on evidence based practice i decided to search for an evidence based article that would prove what we are doing with the e-stim machine is beneficial. The article is a literature review and seeks to gather the evidence for e-stim and muscle strengthening. The results say that theoretically the e-stim is a complimentary tool for muscle strengthening. However, the e-stim still has little evidence for muscle strengthening and does not show that it is any better than traditional strengthening methods.

    I was very excited to talk talk about this article. During my rotation, I have found that my FE uses kinesiotape fairly often with patients, depending on condition and circumstance, obviously. I know in our MOT class/faculty, we definitely had some "doubters" in the effects kinesiotape can have. One of my patients was a lady in her 70's who had a CVA more than 10 years ago, and has since suffered from significant shoulder subluxation. She came in to therapy after trialing botox. When she came to us, she was very discouraged that botox had not helped, and also discouraged because past OT had not helped her at all, and was now dealing with significant pain from subluxation. After research and discovering this article, I discussed with my FE and we agreed that this California Tri-Pull taping method would be appropriate to try with our patient. In short- the patient found that the tape really helped her, and felt like it was the first thing that had provided her a little bit of relief in a long time. -The taping method itself actually is not too complicated, and did not take long to practice and learn before completing on a patient. In short- this article discusses the methodology for taping, as well as research that backs the effectiveness of the taping method. Overall, the study finds some mixed results in effectiveness in terms of reducing subluxation and reducing pain, but overall can be a promising adjunct to the management of the hemiplegic subluxed shoulder

Treatment of reflex sympathetic dystrophy using the stress-loading program - 0 views

    This article talks about a stress-loading, also known as the "Scrub & Carry Program," beneficial for patients with Complex Regional Pain Syndrome/Reflex Sympathetic Dystrophy. This program entails stressing the body part, typically a hand in hopes of the nervous system regulating the pain signals it is sending to the brain. Patients using this program will "scrub" using a brush or the Dystrophile and scrubbing foreward/backward & side-to-side, initially starting for 3 minutes at a time and gradually increasing. Then patients are to carry a weighted object, to their tolerance, in the affected hand for as much of the day as possible. We used this with a patient who had a traumatic work place amputation to his ring finger, and deep laceration (down to the bone) to his middle finger and was starting to show signs of RSD. Though he has poor compliance with this program at home, we did see some benefit from it in the clinic.
