    In what may feel like a flashback to the porn wars of the '60s, a new study investigates the link between a country's relative gender equality and the degree of female "empowerment" in the X-rated entertainment it consumes. Researchers at the University of Hawaii focused on three countries in particular: Norway, the United States and Japan, which are respectively ranked 1st, 15th and (yikes) 54th on the United Nations' Gender Empowerment Measure (GEM). To simplify their analysis, their library of smut was limited to explicit photographs of women "from mainstream pornographic magazines and Internet websites, as well as from the portfolios of the most popular porn stars from each nation." Then they set out to evaluate each image on both a disempowerment and an empowerment scale, using respective measures like whether the woman is "bound and dominated" by "leashes, collars, gags, or handcuffs" or "whether she has a natural looking body." Their hypothesis was that societies with greater gender equity will consume pornography that has more representations of "empowered women" and less of "disempowered women." It turned out the former was true, but, contradictory as it may sound, the latter was not. "While Norwegian pornography offers a wider variety of body types -- conforming less to a societal ideal that is disempowering to the average woman -- there are still many images that do not promote a healthy respect for women," the researchers explain. In other words, Norwegian porn showed more signs of female empowerment, but X-rated images in all three countries equally depicted women in demeaning positions and scenarios. This, the researchers surmise, "suggests that empowerment and disempowerment within pornography are potentially different constructs." The latest research from the University of Missouri shows that BPA causes male deer mice to become demasculinized and behave more like females in their spatial navigational abilities, leading scientists to conclude that exposure to BPA during human development could be damaging to behavioral and cognitive traits that are unique to each sex and important in reproduction.

    David Baum, University of Wisconsin, says: "All agree that eukaryotes arose from a symbiotic relationship between two cell types: bacteria that became mitochondria and a host cell, archaea, or a close relative of archaea, that became the cytoplasm and nucleus. This symbiosis explains the origin of mitochondria, but what about other eukaryotic structures, most notably the nucleus?" The Baums' inside-out theory provides a gradual path by which eukaryotic cells could have evolved. The first stage began with a bacterial cell whose outer membrane forms protrusions, which the Baums call 'blebs', that reached out from the cell. These protrusions trapped free-living mitochondria-like bacteria between them. Using the energy gained from being in close contact with bacteria (and using bacterial-derived lipids), cells were able to get bigger and expand the size of their blebs. The sides of the blebs formed the endoplasmic reticulum and their inner surfaces formed the outer membrane of the nucleus, with the original outer membrane of the archaeon becoming what we now call the inner nuclear membrane. Finally, the fusion of blebs with one another led to the formation of the plasma membrane. The result was the eukaryotic cell as we now know it. This inside-out theory is explained in more detail using a diagram in the research article (see notes to editors).

  • The North American Spine Society and the Society of Interventional Radiology have pointed to flaws in both studies. And earlier studies, published over 15 years, found major benefits to kyphoplasty and a related procedure called vertebroplasty. "We're missing opportunities for patients to receive a safe and effective treatment that can significantly reduce their pain and disability," said Malamis, an interventional radiologist. The procedures are used to treat vertebral compression fractures in patients with osteoporosis and other conditions that result in brittle bones. In a vertebroplasty, an acrylic cement is injected into a fractured vertebra. In a kyphoplasty, a balloon-tipped catheter first is inserted into the fracture. The balloon is inflated to restore the height and shape of the vertebra before the cement is injected. Neva Nelson, 74, of Naperville, Ill., said a kyphoplasty that Malamis performed in October, 2009, has greatly reduced her pain in a vertebra in her lower back that she fractured after falling on ice. Before her kyphoplasty, Nelson had to sit on cushions. Walking, and especially standing, were painful. "I had to do something," she said. "I could not go on like that." Nelson said that since undergoing her kyphoplasty, "I don't have to worry about my back any more." In the controversial studies, patients were randomly assigned to receive a vertebroplasty or a placebo-like "sham" procedure. In the sham procedure, patients received an injection of anesthetic, but no cement. However, patients in severe pain are reluctant to enroll in a trial where there's a 50 percent chance of receiving a sham treatment. In one of the studies, researchers had to screen 1,813 patients to enroll just 131 subjects. In the other study, only 78 of 219 eligible patients were enrolled. This low enrollment rate raises the possibility that the patients who did enroll were not representative. Patients experience the greatest pain during the first three months after a compression fracture. Thereafter, pain gradually subsides. Thus, a vertebroplasty or kyphoplasty provides the greatest benefit when performed within a week or two of the fracture. But the studies enrolled patients up to 12 months after fractures. In addition to reducing pain and disability, a kyphoplasty can reduce the risk of subsequent fractures by improving the angle and height of the spine. The studies evaluated vertebroplasty alone, and did not include the more innovative and very different kyphoplasty procedure. Malamis suggests the medical community wait for the results of additional studies now underway before passing final judgment on vertebroplasty or kyphoplasty. In the mean time, he notes that Medicare still covers the procedures.

But the number of kyphoplasty referrals Malamis has received from primary care doctors has dropped sharply since two controversial studies were published last year in the New England Journal of Medicine. In findings that have been disputed by two medical societies, researchers reported that a procedure related to kyphoplasty was not significantly better than a placebo-like procedure in reducing pain and disability.

"There's a popular perception that with DNA, you get results," Kaye said. "You're either guilty or innocent, and the DNA speaks the truth. That goes too far. DNA is a tool. Perhaps in many cases it's open and shut, in other cases it's not. There's ambiguity."

"They don't have to be scientists or technicians, but they do have to know enough to understand what's going on and whether the statements that experts are making are well-founded.   • Statistical validation of results, as Shaffer described it, simply involves testing the null hypothesis: that the pattern you detect in your data occurs at random. If you can reject the null hypothesis—and science and medicine have settled on rejecting it when there's only a five percent or less chance that it occurred at random—then you accept that your actual finding is significant. The problem now is that we're rapidly expanding our ability to do tests. Various speakers pointed to data sources as diverse as gene expression chips and the Sloan Digital Sky Survey, which provide tens of thousands of individual data points to analyze. At the same time, the growth of computing power has meant that we can ask many questions of these large data sets at once, and each one of these tests increases the prospects than an error will occur in a study; as Shaffer put it, "every decision increases your error prospects." She pointed out that dividing data into subgroups, which can often identify susceptible subpopulations, is also a decision, and increases the chances of a spurious error. Smaller populations are also more prone to random associations. In the end, Young noted, by the time you reach 61 tests, there's a 95 percent chance that you'll get a significant result at random. And, let's face it—researchers want to see a significant result, so there's a strong, unintentional bias towards trying different tests until something pops out. Young went on to describe a study, published in JAMA, that was a multiple testing train wreck: exposures to 275 chemicals were considered, 32 health outcomes were tracked, and 10 demographic variables were used as controls. That was about 8,800 different tests, and as many as 9 million ways of looking at the data once the demographics were considered.

    It's possible to get the mental equivalent of whiplash from the latest medical findings, as risk factors are identified one year and exonerated the next. According to a panel at the American Association for the Advancement of Science, this isn't a failure of medical research; it's a failure of statistics, and one that is becoming more common in fields ranging from genomics to astronomy. The problem is that our statistical tools for evaluating the probability of error haven't kept pace with our own successes, in the form of our ability to obtain massive data sets and perform multiple tests on them. Even given a low tolerance for error, the sheer number of tests performed ensures that some of them will produce erroneous results at random.
