This announcement solicits applications for the Rural Health Care Coordination Network Partnership Program (Care Coordination Program). The purpose of the Rural Health Care Coordination Network Partnership Program is to support the development of formal, mature rural health networks that focus on care coordination activities for the following chronic conditions: diabetes, congestive heart failure (CHF) and chronic obstructive pulmonary disease (COPD). Care coordination in the primary care practice involves deliberately organizing patient care activities and sharing information among all of the participants concerned with a patient¿s care to achieve safer and more effective care. Rural Americans are unhealthier, with higher rates of chronic illnesses, such as diabetes, CHF, and COPD and have higher rates of high-risk behaviors such as smoking, physical inactivity, and poor nutrition.[1],[2],[3],[4] These high-risk behaviors cause many of the illnesses, suffering and deaths due to chronic diseases and conditions.[5] The increasing prevalence of chronic diseases and the high cost of health care in the U.S. bring treatment of the ¿whole¿ person to the forefront, especially as there are often psychosocial (psychological and social) issues related to chronic diseases; for example, there is a link between diabetes and depression. In addition, more mental health problems are seen in the primary care setting than other health care settings; thus, integrating behavioral health care into primary care helps address both the physical and psychosocial aspects of health and wellness. Pediatric Pharmacy Advocacy Group is an international, nonprofit, professional association representing the interests of pediatric pharmacists and their patients. We are dedicated to improving medication therapy in children. Our sole purpose is to promote safe and effective medication use in children through communication, education, and research.

The National Kidney Foundation recognizes that future improvements in the treatment and prevention of kidney disease will rest largely on the accomplishments of individuals now commencing their investigative work. To that end, the foundation has established a Young Investigator Grant Program. Through the program, NKF will award grants of up to $35,000 to individuals who have completed their fellowship training and who hold junior faculty positions at a university-affiliated medical center in the United States for research in the field of nephrology or a related discipline. Projects must be patient-oriented and may include but are not limited to the development of new technologies, mechanisms of human disease, educational or therapeutic interventions, epidemiological studies, health policy studies, and clinical trials. Systems for Action (S4A) is a national program of the Robert Wood Johnson Foundation that aims to discover and apply new evidence about ways of aligning the delivery and financing systems for medical, social, and public health services that support a Culture of Health. S4A uses a wide research lens that includes and extends beyond medical care and public health systems to include other sectors that affect health and well-being, such as housing, transportation, social services, education, criminal and juvenile justice, and economic and community development.

The purpose of this research project is to use medical and pharmacy claims data in real-time to: 1) identify HIV-infected patients who have stopped filling anti-retroviral (ARV) prescriptions and to target these individuals for adherence and retention intervention(s) (Category A); and 2) identify persons living with HIV (including pregnant women) and to ensure these individuals are receiving ARV therapy - Medical and Pharmacy Claims (Category B).

This notice announces the opportunity to apply for funding under the Innovations in Access to Care for Children and Youth with Epilepsy Program. The purpose of this program is to support those working to improve access to coordinated, comprehensive, quality care for children and youth with epilepsy (CYE) in medically underserved and/or rural areas. The project will support recipients of funding under the Transforming Health Care for CYE program (HRSA-19-060) and provide national leadership and resources on practices, policies, and tools to improve the care of CYE. The recipient will coordinate HRSA-19-060 grant recipient activities and provide HRSA-19-060 grant recipients with technical assistance, training, and education to implement telehealth and telemedicine strategies and improve youth health care transition processes, communication and collaboration between primary and specialty care, data collection, quality improvement, and evaluation. Additionally, the recipient will analyze, compile and disseminate evidence-based and innovative practices, policies, tools, and resources related to health care management of CYE and develop partnerships with national organizations and key stakeholders to improve health care of CYE, especially those residing in rural and/or medically underserved areas.

The purpose of this NOFO is to develop effective, sustainable, and replicable program models that support continuity of HIV medical care for people with HIV (PWH) released from state prisons into the community, with emphasis on linkage to and retention in community HIV medical care, adherence to HIV treatment, and suppression of HIV viral load.

This notice announces the opportunity to apply for funding under the Innovations in Care Coordination for Children and Youth with Autism Spectrum Disorders (ASD) and Other Developmental Disabilities (DD) Program. The purpose of this program is to build on past HRSA programs (HRSA-16-048 and HRSA-13-207) to improve access to coordinated and integrated care for children with, or at risk for, ASD/DD and their families in medically underserved areas and populations.[1],[2] The purpose and objectives of this program will be accomplished by implementing the following two strategies: Family Navigation - Increasing family navigation services to improve communication between families and primary and specialty providers; link children with/at risk for ASD/DD to diagnostic evaluations, ASD/DD services, and community resources; and provide education to families of children with/at risk for ASD/DD to improve self-efficacy in navigating the system of care for children with/at risk for ASD/DD; and Provider Education - Providing education, training, and technical assistance to providers, and community based-organizations providing services to ASD/DD on improving care for children with/at risk for ASD/DD through a learning community. The goal of the interagency Smartand Connected Health (SCH): Connecting Data, People and Systems program is to accelerate the development and integration of innovative computer and information scienceand engineering approaches to support the transformation of health and medicine. Approaches that partner technology-based solutions with biomedical and biobehavioral research are supported by multiple agencies of the federal government including the National Science Foundation (NSF) and the National Institutes of Health (NIH). The purpose of this program is to develop next-generation multidisciplinary science that encourages existing and new research communities to focus on breakthrough ideas in a variety of areas of value to health, such as networking, pervasive computing, advanced analytics, sensor integration, privacy and security, modeling of socio-behavioral and cognitive processes and system and process modeling. Effective solutions must satisfy a multitude of constraints arising from clinical/medical needs, barriers to change, heterogeneity of data, semantic mismatch and limitations of current cyberphysical systems and an aging population.Such solutions demand multidisciplinary teams ready to address issues ranging from fundamental science and engineering to medical and public health practice.

Since its founding in 1979, the Skin Cancer Foundation has set the standard for educating the public and the medical community about skin cancer. The foundation is committed to reducing the incidence and mortality of the world's most common cancer, which is now occurring at epidemic levels. As part of this mission, the foundation is accepting proposals for its 2019 Research Grants Awards program. Two grants of $50,000 and one grant of $25,000 will be awarded to support one-year pilot research projects related to the disease. Researchers are invited to submit applications for projects to be conducted within the dermatology departments of medical institutions in the United States and Canada. The program is open to dermatology residents, fellows, and junior investigators.

DLT grants are designed to provide access to education, training, and health care resources for rural Americans. The DLT Program is authorized by 7 U.S.C. 950aaa and provides financial assistance to encourage and improve telemedicine and distance learning services in rural areas through the use of telecommunications, computer networks, and related advanced technologies that students, teachers, medical professionals, and rural residents can use. The regulation for the DLT Program can be found at 7 CFR part 1734 Subpart B. The grants, which are awarded through a competitive process, may be used to fund telecommunications-enabled information, audio and video equipment, transmission facilities, and related advanced technologies which extend educational and medical applications into rural areas. Grants are intended to benefit end users in rural areas, who are often not in the same location as the source of the educational or health care service.

The Foundation for Women's Wellness, a nonprofit charity dedicated to improving women's health by raising support for innovative early stage research and education, is accepting Letters of Inquiry for small short-term studies with potential for improving medical knowledge in the area of women's health. Through the FWW Research Awards program, the foundation will award grants of up to $25,000 for research projects, with an emphasis on cardiovascular disease, female cancers, the role of hormones in disease, and/or stage-of-life health concerns such as pregnancy and menopause and diseases disproportionately affecting women. This may include disease prevention or treatment; clarification of gender differences; and/or quality-of-life concerns. Priority will be given to projects or studies that have direct clinical application in preventing and/or treating disease, and have the potential to yield results that will attract larger sources of funding for further study and/or influence long-term research and clinical care directions. To be eligible, lead investigators must be an MD and/or PhD with a faculty appointment at an accredited medical institution in the United States. In addition, the affiliated institution must accept the award on the awardee's behalf (with no administrative costs deducted). LOIs must be received no later than June 20. The Tell Every Amazing Lady About Ovarian Cancer Louisa M. McGregor Ovarian Cancer Foundation, also known as T.E.A.L.®, is accepting applications for its 2018 Medical Research Program. The program, which aids in the advancement of ovarian cancer treatment research and the need for a screening test, offers funding in support of ovarian cancer research studies that are deemed to be most aligned with T.E.A.L.®'s mission by the organization's board of directors and a scientific advisory board. This year, the program has special interest in research related to the p53 gene, although it will accept proposals for other research related to ovarian cancer. In 2018, T.E.A.L.® will award one-year grants of up to $50,000. Project costs must be thoroughly outlined in the budget portion of the application, and grants cannot be applied to indirect costs or PI salary. In 1994 the Centers for Disease Control and Prevention (CDC) collaborated with a broad range of partners in clinical medicine and public health to develop an emerging infections strategy in response to the recommendations issued by the Institute of Medicine (IOM) in its 1993 report, “Emerging Infections: Microbial Threats to Health in the United States.” Leading this strategy was the CDC Office of Infectious Diseases (OID)/National Center for Emerging and Zoonotic Infectious Diseases (NCEZID). One goal was to strengthen local, state, and federal public health infrastructures to support surveillance and implement prevention and control programs. The implementation strategy for this goal was to: 1) provide state-of-the-art training in diagnostic evaluation and testing for medical laboratory personnel to ensure the diagnosis and surveillance of emerging infections; and 2) establish a public health laboratory fellowship in infectious diseases that will train medical microbiologists in public health approaches to diagnosis and molecular epidemiology.

The research will fall into one or more categories corresponding to HRP's five Elements: Space Radiation, Human Health Countermeasures, Exploration Medical Capability, Human Factors and Behavioral Performance, and International Space Station Medical Projects. This NRA covers all aspects of research to provide human health and performance countermeasures, knowledge, technologies, and tools to enable safe, reliable, and productive human space exploration.
