Input Needed from Family Members in Montana: Money Follows the Person Survey - 0 views

    What:
    DPHHS is applying for a Money Follows the Person Grant. The "Money Follows the Person" Rebalancing Demonstration Program (MFP) helps States rebalance their long-term care systems to transition people with Medicaid from institutions to the community. Forty-three States and the District of Columbia have implemented MFP Programs.  From spring 2008 through December 2010, nearly 12,000 people have transitioned back into the community through MFP Programs. The Affordable Care Act of 2010 strengthens and expands the "Money Follows the Person" Program to more States. Click here to fill out the survey. MFP Program Goals Increase the use of home and community-based services (HCBS) and reduce the use of institutionally-based services. Eliminate barriers in State law, State Medicaid plans, and State budgets that restrict the use of Medicaid funds to let people get long-term care in the settings of their choice. Strengthen the ability of Medicaid programs to provide HCBS to people who choose to transition out of institutions. Put procedures in place to provide quality assurance and improvement of HCBS.     WebEase (Web Epilepsy, Awareness, Support and Education) is the first interactive, on-line self-management program for people with epilepsy shown to be effective in improving self-management skills. WebEase was developed by a team of researchers from Emory University with funding from the Centers for Disease Control and Prevention, and is now available to the general public through the Epilepsy Foundation at www.webease.org. The purpose of this presentation is to provide interested epilepsy health care and social services providers with a brief overview of WebEase. In addition to exploring the benefits of WebEase for people living with epilepsy, the Webinar will review the structure and components of the program, its theoretical principles and its evidence base.   • The Patient Protection and Affordable Care Act (the "Affordable Care Act," or "ACA") (P.L. 111-148) is expected to expand health care coverage to an additional 32 million individuals through a combination of new health insurance marketplaces (formerly referred to as exchanges) and a state's option to expand Medicaid. In addition, the ACA includes a number of critical protections for consumers or beneficiaries, as well as provisions to improve the quality of care while bending the curve of health care costs.

Child Care and the ADA Resources - 0 views

  • Child care providers encounter unique challenges when implementing the ADA at their centers.   • But a growing concept in health care reform called the "medical home" offers parents a way to simplify, organize and coordinate the complexities of their medically fragile child's health care needs. The medical home is not a location but an approach to care coordination designed to provide a constant trusted source of care, typically by a general pediatrician.

Portfolios are also used by youth to introduce themselves at their IEP meetings and to develop self-advocacy skills. During this webinar, you will learn how Portfolios can be an important piece of the health care transition for young adults. Through a Portfolio's concise text and photos, individuals with disabilities can describe the impact of their disability; list current medications and side effects they are experiencing; discuss seizures (what type, how often, etc.); introduce and explain medical equipment; share information about helpful accommodations and supports; and articulate what they want and need from their medical provider.   • Today, as part of the Obama administration’s work to make our health care system more transparent, affordable, and accountable, Health and Human Services (HHS) Secretary Kathleen Sebelius announced the release of new, privacy-protected data on services and procedures provided to Medicare beneficiaries by physicians and other health care professionals. The new data also show payment and submitted charges, or bills, for those services and procedures by provider.“Currently, consumers have limited information about how physicians and other health care professionals practice medicine,” said Secretary Sebelius “This data will help fill that gap by offering insight into the Medicare portion of a physician’s practice. The data released today afford researchers, policymakers and the public a new window into health care spending and physician practice patterns.”

Emphasis is placed on Medicaid and the Children's Health Insurance Program (CHIP). Separate sections present resources for professionals (health professionals, program administrators, policymakers, and researchers) and for families.
