MiamiOH OARS on 24 Apr 15This funding opportunity is designed to bring to scale and sustain evidence-based self-management education programs that empower older adults and adults with disabilities from underserved areas and populations to better manage their chronic conditions. The grants are intended to increase the number of chronic disease self-management education program participants, while concurrently increasing the sustainability of these proven programs through innovative funding arrangements and by embedding the programs into the nation’s health and long-term services and supports systems. The awards will be made in the form of cooperative agreements because AoA will be substantially involved.
