This notice solicits applications for the Health Center Program's Service Area Competition (SAC). The Health Center Program supports public and private nonprofit community-based and patient-directed organizations that provide primary health care services to the Nation's medically underserved. The purpose of the SAC NOFO is to ensure continued access to affordable, quality primary health care services for communities and vulnerable populations currently served by the Health Center Program. This NOFO details the SAC eligibility requirements, review criteria, and awarding factors for organizations seeking funding for operational support to provide primary health care services to an announced service area under the Health Center Program. For the purposes of this document, the term "health center" encompasses Health Center Program award recipients funded under the following subsections: Community Health Center (CHC - section 330(e)), Migrant Health Center (MHC - section 330(g)), Health Care for the Homeless (HCH - section 330(h)), and/or Public Housing Primary Care (PHPC - section 330(i)).

This Funding Opportunity Announcement (FOA) solicits resource grant applications for projects that will bring useful, usable health information to health disparity populations and their health care providers. Access to useful, usable, understandable health information is an important factor when making health decisions. Proposed projects should exploit the capabilities of computer and information technology and health sciences libraries to bring health-related information to consumers and their health care providers. Because this FOA focuses on providing health information to health disparity populations, institutions with demonstrated commitment to the needs of health disparity communities (including Tribal Colleges and Universities (TCU), Historically Black Colleges and Universities (HBCU), Hispanic-Serving Institutions (HSI) and other institutions in rural and socially disadvantaged areas) are encouraged to apply.

The purpose of the Health Workforce Improvement Program (HWIP) is to address critical human resources challenges in Ethiopia's health sector. The program will build on the work carried out under the Strengthening Human Resources for Health (HRH) Cooperative Agreement (AID-663-A-12-00008) that was awarded to Jhpiego in 2012. The Health Workforce Improvement Program will work with the Ministry of Health (MOH) and Ministry of Science and Higher Education (MOSHE) at the national level and their respective structures at the sub-national levels (regional, zonal and woreda levels), Higher Education Institutes (HEIs) levels that provide healthcare education at national and sub-national, and targeted health worker professional associations to improve the quality of Ethiopia's health workforce. The focus areas of this Activity are: 1. Institutional and individual capacity building to improve the quality of pre-service education for priority clinical cadres to improve the competency of clinical health workers, 2. Improve the human resource for health management and regulation capacity of national and sub-national entities. The program will target program managers at MOH, Regional Health Bureau (RHB) and District Health Office; and faculty at HEIs, building their leadership, management and governance capacity for human resource development and management. 3. Purpose NIMH seeks applications for innovative research projects to test strategies to increase the reach, efficiency, effectiveness, and quality of digital mental health interventions which may impact mental health outcomes, including suicide behaviors and serious mental illness. This FOA is intended to support the development of digital health test beds that leverage well-established digital mental health platforms/infrastructure to rapidly refine and optimize existing evidence based digital health interventions and conduct clinical trials testing digital mental health interventions that are statistically powered to provide a definitive answer regarding the intervention's effectiveness. Background The healthcare landscape in the United States is constantly changing, creating new challenges to the delivery of high-quality treatments and services to children, youth, adults, and older adults with unmet or under-met mental health needs. Epidemiological findings suggest that approximately one half of the United States population meets lifetime criteria for a mental disorder, and approximately one quarter of the population meets criteria in any given year. However, only one half of people with any mental health disorder and only two thirds of people with a serious mental health disorder received mental health services in the previous year. Of those that find their way into mental health care, many fall out of care and/or do not receive guideline concordant treatment, including suicide risk assessment and evidence-based preventive interventions. Disparities in population status (e.g., members of racial, ethnic, sexual, and gender minority communities), a fragmented healthcare system, provider shortages, healthcare affordability, and other factors moderate these findings.

ACHF is offering a $2,500 award in conjunction with Student Health 101 to support new efforts to improve and promote health education programs on college campuses. These initiatives could include health promotion communications, research, advocacy, and other activities related to health prevention. The American College Health Foundation's Student Health 101 Award was established to support creative efforts to improve and promote projects in health promotion on college campuses that have measurable results. The college environment is an important place and opportunity to engage young adults and influence them to establish life-long healthy habits, while avoiding behaviors with adverse consequences. Health promotion delivered in creative ways with measurable outcomes is an invaluable tool. The Student-Health 101 Award is designed to fund the development of a creative initiative that involves student peers in an effort to promote healthy behaviors.

The United States (U.S.) Centers for Disease Control and Prevention (CDC) is working to help the Côte d'Ivoire Ministry of Health meet key International Health Regulation (IHR) requirements through implementation of priority Global Health Security Agenda (GHSA) activities. The objectives of this program are to strengthen Côte d'Ivoire's health system capacities to prevent, detect and respond to infectious disease outbreaks and other public health emergencies. This program emphasizes further improving core GHSA priority capacities in disease surveillance, laboratory diagnostics, emergency preparedness and response, public health workforce development, and strengthening of anti-microbial resistance stewardship. This program includes a particular focus on strengthening disease surveillance through the establishment of an event-based surveillance system in high-risk border districts, reinforced through public health staff training and supervision. Surveillance and response capacities will be bolstered through improved laboratory point of care diagnostics and the integration of epidemiologic and laboratory data. This program will also support building Côte d'Ivoire's capacity to respond to disease outbreaks and public health emergencies through the training and mentoring of a cadre of public health workers in each of the 101 districts in Côte d'Ivoire in field epidemiology and emergency preparedness and response, reinforcing these capacities via field simulation exercises.

The Communities Building Healthier Environments for a Stronger Nation Initiative ('Communities Initiative') intends to demonstrate the effectiveness of community-based networks in improving health outcomes among racial and ethnic minority and/or other disadvantaged populations. This program seeks to improve health outcomes through the establishment of integrated networks that collaboratively employ evidence-based disease management and preventive health activities; build the capacity of communities to address social determinants and barriers to healthcare access; and increase access to and utilization of preventive health care, medical treatment, and supportive services. The Communities Initiative specifically targets the unmet healthcare and supportive service needs of racial and ethnic minority populations at highest risk for poorer health outcomes. Health services provided under the Communities Initiative will not be denied to any person based on race, color, or national origin. Populations at highest risk include, but are not limited to, individuals who are newly diagnosed and lack a medical home; individuals who experience difficulty in adhering to a prescribed medical treatment plan; individuals with a chronic disease that is not well managed; and individuals that are unstably housed. Specific health areas to be addressed by the Communities Initiative include asthma, cardiovascular disease, diabetes, HIV/AIDS, Hepatitis B or C, obesity/overweight, and mental disorders. Community health programs are required to address social determinants of health, and improve coordination of health, social, and supportive services to significantly improve health outcomes among minority and/or disadvantaged communities. This notice announces the opportunity to apply for funding under the Rural Health Care Services Outreach Program (Outreach Program). The Outreach Program is a community-based grant program aimed towards promoting rural health care services by enhancing health care delivery to rural underserved populations in the local community or region. Through consortia of local health care and social service providers, communities can develop innovative approaches to challenges related to the specific health needs in rural areas that expand clinical and service capacity in rural communities. The overarching goals for the Outreach Program are to: * Expand the delivery of health care services to include new and enhanced services exclusively in rural communities; * Deliver health care services through a strong consortium, in which every consortium member organization is actively involved and engaged in the planning and delivery of services; * Utilize community engagement and evidence-based or innovative, evidence informed model(s) in the delivery of health care services; and * Improve population health, and demonstrate health outcomes and sustainability In addition to funding Outreach programs through the Regular Outreach track, in FY 21, FORHP will also afford applicants a unique opportunity to take part in a national effort that targets rural health disparities through a second track called the "Healthy Rural Hometown Initiative."

The Robert Wood Johnson Foundation Health Policy Fellows program provides the nation's most comprehensive fellowship experience at the nexus of health science, policy and politics in Washington, D.C. It is an outstanding opportunity for exceptional midcareer health professionals and behavioral and social scientists with an interest in health and health care policy promoting the health of the nation. Fellows participate in the policy process at the federal level and use that leadership experience to improve health, health care and health policy. The fellowship requires, at a minimum, a 12-month residential experience in Washington, D.C., with additional support for health policy leadership development activities. The program will select up to six fellows.

This announcement solicits applications for the Rural Health Network Development Planning Grant Program (¿Network Planning¿).   The purpose of the Network Planning program is to assist in the development of an integrated healthcare network, if the network participants do not have a history of formal collaborative efforts.  Health care networks can be an effective strategy to help smaller rural health care providers and health care service organizations align resources and strategies, achieve economies of scale and efficiency, and address challenges more effectively as a group than as single providers.  The Network Planning program promotes the planning and development of healthcare networks in order to: (i) achieve efficiencies; (ii) expand access to, coordinate, and improve the quality of essential health care services; and (iii) strengthen the rural health care system as a whole.  The health care system is undergoing a significant amount of change and this can be particularly challenging for small rural providers.  The goals of the Network Planning program represent ways to help rural providers better serve their communities given changes taking place in health care, as providers move from focusing on the volume of services to focusing on the value of services.  This program brings together key parts of a rural health care delivery system, particularly those entities that may not have collaborated in the past under a formal relationship, to work together to establish and improve local capacity and coordination of care.  Furthermore, this program supports one year of planning with the primary goal of helping networks create a foundation for their infrastructure and focusing member efforts to address important regional or local community health needs.

The mission of the Office on Womens Health (OWH) is to provide national leadership and coordination to improve the health of women and girls through policy, education and innovative model programs. The Department of Health and Human Services has identified violence prevention as a major goal for improving health across several initiatives, including Healthy People. Further efforts are needed to ensure that all aspects of the health care system respond appropriately to women and girls who are victims of interpersonal violence. According to data from the CDC, women are disproportionately affected by intimate partner violence, sexual violence, and stalking; and victims experience many negative impacts and health consequences. Preliminary work supported by OWH highlighted the need for more collaborative violence prevention and health-related programs and projects.Previous work sponsored by OWH has illustrated the need to connect health care providers and public health programs to IPV programs. Strategies have included train-the-trainer methods, clear messaging, and development of services. OWH work also supports the importance of including geographically and ethnically diverse sites to maximize impact and social responsibility to underserved populations. Across all strategies and sites, the over-riding goal has been to develop the connection and support the linkage between systems that provide health care services and IPV programs for women. The next step is to test models that implement the linkages using robust methodologies to test interventions and evaluate programs.

This project will: 1. Enhance and strengthen ongoing regional cooperation related to the creation of a shared surveillance information platform. 2. Contribute to efficiencies the small Central American countries are seeking to improve laboratory capacity by creating networks of reference laboratories. 3. Develop and implement regional guidelines for biosafety, as well as other guidelines that are appropriately developed regionally. It will strengthen the network of epidemiologists and regional epidemiological training. 4. Strengthen regional communication and the capacity of the countries of the region to respond in a coordinated manner to epidemiological and public health threats. This program addresses the issues of Health Communication; Immunization and Infectious Diseases; Public Health Infrastructure; Respiratory Diseases; and Sexually Transmitted Diseases, and is in alignment with HHS/CDC performance goal(s) to protect Americans from infectious diseases by providing global health promotion, health protection and health diplomacy. Measurable outcomes of the program will be in alignment with one (or more) of the following performance goal(s) the Center for Global Health’s priority areas identified in "Protecting the Nation’s Health in an Era of Globalization: CDC’s Global Strategy for Addressing Infectious Diseases". Priority areas for this cooperative agreement include: 1) implementation of proven disease prevention and control interventions, 2) application of proven public health tools, 3) identification of potential global initiatives for disease control and, 4)public health training and capacity building.

This notice announces the opportunity to apply for funding under the National Rural Health Policy, Community, and Collaboration Program. The purpose of this program is to identify, engage, educate, and collaborate with rural stakeholders on national rural health policy issues and promising practices in an effort to improve the health of people living in rural communities nationwide. Program objectives include: (1) Identifying and educating rural stakeholders about national policy issues and promising practices for rural health. (2) Maintaining projects that will help support engagement of rural communities in a broad range of activities. (3) Facilitating partnerships and collaborations at the local, regional, state, and national levels to improve the exchange of information and promising practices that support rural health. (4) Identifying and promoting broader collaborative federal efforts to support, promote, and address unique rural health issues. A successful program will continue to build upon the federally funded resources that currently exist, leverage the broad network of rural health organizations and individual subject matter experts, and provide a mechanism for the Federal Office of Rural Health Policy (FORHP) to engage with public rural health stakeholders beyond Health Resources and Services Administration (HRSA) funded award recipients and current partners to add value, information, and support to rural communities.

Consolidating and organizing public health expertise and systems within a National Public Health Institute (NPHI) is a successful way for countries to carry out essential public health functions and improve their accountability and efficiency. The CDC, with over 70 years of experience as the U.S. NPHI, is helping countries around the world increase surveillance, laboratory, and outbreak response capacity to improve recognition of and response to health threats. An NPHI coupled with stronger local outbreak detection and response capabilities offers a sustainable solution for building global public health capacity, achieving global health security, and increasing public health impact. This Notice of Funding Award (NOFO) seeks to build upon activities supported by CDC through partnerships with Ministries of Health, National Public Health Institutes, and organizations that have capacity by providing technical assistance supporting the establishment and development of strong NPHI's globally. The CDC seeks to continue strengthening NPHI's so that they are credible, technically expert, apolitical and prioritize the protection of the publics' health.

The Centers for Medicare & Medicaid Services (CMS) is announcing the availability of funds under this grant program to assist researchers in conducting health services research for 2013. The purpose of the Hispanic grant program is to implement Hispanic health services research activities to meet the needs of diverse CMS beneficiary populations. The grant program is designed to: 1) encourage health services and health disparities researchers to pursue research issues which impact Hispanic Medicare, Medicaid, and Children Health Insurance Program (CHIP) health services issues, 2) conduct outreach activities to apprise Hispanic researchers of funding availability to conduct research-related issues affecting Hispanic communities to expand the pool of applicants applying for such grants, 3) assist CMS in implementing its mission focusing on health care quality and improvement for its beneficiaries, 4) support extramural research in health care capa city development activities for the Hispanic communities, 5) promote research that will be aimed at developing a better understanding of health care services issues pertaining to Hispanics, and 6) foster an network for communication and collaboration regarding Hispanic health care issues.

The mission of the American Association on Health and Disability is to contribute to national, state, and local efforts to promote health and wellness in people with disabilities and identify effective intervention strategies that reduce health disparities between people with disabilities and the general population. AAHD accomplishes its mission through research, education, and advocacy at the national, state, and community levels. To that end, the AAHD Frederick J. Krause Scholarship on Health and Disability is awarded annually to deserving students with a disability who are pursuing undergraduate/graduate studies (at least a sophomore in college) at an accredited university in an area related to health and disability, including but not limited to public health, health promotion, disability studies, disability research, rehabilitation engineering, audiology, disability policy, special education, or other majors that impact quality of life for persons with disabilities. Scholarships of up to $1,000 will be awarded in late January/early February 2019. To be eligible, applicants must be enrolled full time as an undergraduate (freshman or beyond) or part or full time in graduate school, have a documented disability, and be able provide documentation of their disability. (Applicants who have not yet graduated from high school will not be considered.) In addition, applicants must be a citizen or legal resident of the United States enrolled in an accredited U.S. university. Preference will be given to students majoring in public health, disability studies, disability research, health promotion, or a field related to disability and health.

In September 2000, the Pew Environmental Health Commission issued a report entitled America’s Environmental Health Gap: Why the Country Needs a Nationwide Health Tracking Network. In this report, the Commission documented that the existing environmental health systems were inadequate and fragmented and recommended a “Nationwide Health Tracking Network for disease and exposures.” In response to the report, Congress appropriated funds in the fiscal year 2002’s budget for the CDC to establish National Environmental Public Health Tracking Program and Tracking Network and has appropriated funds each year thereafter to continue this effort. Environmental Public Health Tracking Network (Tracking Network) is the integrated surveillance of health, exposure, and hazard information and data from a variety of national, state, and local sources. The Tracking Network is unique in that it provides the United States with accurate and timely standardized data and supports ongoing efforts within the public health and environmental sectors to improve data collection, accessibility, and dissemination as well as analytic and response capacity. Data that were previously collected for different purposes and stored in separate systems are now available in a nationally standardized format allowing programs to begin bridging the gap between health and the environment.

This program is financed in part by FY18 Prevention and Public Health Funds. The purpose of this notice of funding opportunity (NOFO) is to build on the National Center for Chronic Disease Prevention and Health Promotion’s (NCCDPHP) efforts to strengthen the capacity and capabilities of the states’ chronic disease prevention and health promotion public health workforce.
