The Pharmacists' Defence Association (PDA) has secured the right to negotiate and agree on proposed changes to the NHS Agenda for Change (AfC) terms and conditions for its members. This milestone follows an agreement with other NHS trade unions, enabling the PDA Union to join the NHS Staff Side. As part of this group, the PDA can now actively participate in the NHS National Joint Council, which oversees changes to AfC. Participation in the NHS Staff Side brings significant benefits for PDA members, especially those employed in the NHS under AfC. In addition to providing the PDA the right to negotiate and agree proposed changes to AfC, it empowers the trade union to advocate for pharmacists at the highest levels in the NHS, participate fully in the NHS Social Partnership Forum, and collaborate with other unions on critical issues such as NHS pay. It also gives PDA members the right to be represented at local trust level on matters handled by local staff sides.

In September, the Royal Pharmaceutical Society (RPS), the professional leadership body for pharmacists and pharmaceutical scientists in Great Britain, announced its proposal to transition into a Royal College. This significant step, which came following an 18-month independent review of the RPS constitution and governance, aims to strengthen the RPS's leadership role, with the ultimate goal of delivering greater benefits to patients and the public. In an exclusive interview with Pharmacy Business, Tase Oputu, chair of the RPS English Pharmacy Board, discusses the key reasons behind the proposed changes, the potential impact on pharmacists and patients, and her personal journey as the first Black Chair of the RPS Board. Benefits for pharmacists and patients When asked how this transition would benefit pharmacists and patients, Tase says, "I'm excited about RPS' proposals for change. In my view, becoming a Royal College would elevate the status of the profession and strengthen our ability to raise the profile of pharmacy, advocate for change and advance the safe and effective use of medicines for the benefit of patients and the public."

The British Medical Association (BMA) has urged non-GP partners and contractors, including pharmacists, nurses, and physiotherapists, to support their "Protect Your Patients, Protect Your GP Practice" campaign, which was launched to protest changes to the 2024/25 GP contract. GP contractors and partners across England have strongly opposed the modifications to the GMS contract, leading to an ongoing dispute with the NHS. The BMA highlights that general practice in England currently receives just 6 pence of every NHS pound and an average GMS 'global sum' of £107.57 per registered patient annually. Since 2018/19, CPI erosion to the GMS contract has resulted in a loss of £659 million, or 6.6%. This financial strain has contributed to the closure or merger of over 1,300 practices over the past decade. To address these issues, the BMA is urging all GP contractors and partner members to vote YES in the GPC England non-statutory ballot, which will be open until July 29.

To combat the effects of climate change, the Integrated Care System (ICS) in South East London (SEL) has initiated a pioneering inhaler recycling project. Supported by NHS England and spanning 20 community pharmacies across the region, the project marks the first nationally-funded pilot of its kind in England, targeting the reduction of greenhouse gas emissions from improperly disposed inhalers. The project, which commenced at King's College Hospital in February 2024, focuses on the recycling of pressurised metered-dose inhalers. These inhalers, due to their propellant content, contribute significantly to environmental harm, emitting gases equivalent to driving 71.8 miles in a standard petrol car.

Hallo Healthcare has announced its decision to exit from its hospital pharmacy sites, currently operated by LloydsPharmacy Healthcare Services (LP HCS Limited), which primarily provides outpatient services. Pharmacists and their teams at its 64 pharmacy sites were informed on Monday, 19 August 2024, about the company's plans to withdraw from this sector. The Pharmacists' Defence Association (PDA), the recognised trade union representing pharmacists at these sites, was also notified. The association has assured its support for members impacted by the decision, although it noted that the process of branch transfers could take several months. "It is expected to be several months before all such branch transfers are agreed and undertaken," the PDA said. The association noted that contracts to provide such services "do change hands periodically with some hospital outpatient departments (OPDs) operated by pharmacy multiples and others operated by hospital-owned subsidiaries" and highlighted its experience in supporting pharmacists when employers change in this way. PDA Union Director Paul Day is hopeful that there will be no job losses in outpatient services and that all pharmacists will be able to continue in their current roles.

The Department of Health and Social Care (DHSC) has announced adjustments to the reimbursement rates for several Aptamil products dispensed earlier this year, following a pricing error that left pharmacies out of pocket. The changes, which apply to products dispensed between January and April 2024, come after Community Pharmacy England (CPE) raised concerns about discrepancies in the pricing data used by the NHS. The issue stemmed from recent price increases by Nutricia Ltd, the manufacturer of Aptamil, which were not reflected in the NHS Dictionary of Medicines and Devices (dm+d). To address the shortfall, the DHSC has retrospectively increased the reimbursement prices for the following Aptamil products: - Aptamil Anti-Reflux powder (800g): From £14.29 to £16.08 - Aptamil Comfort milk (800g): From £14.29 to £16.08 - Aptamil First milk powder (200ml): From £0.92 to £1.06 - Aptamil Follow On milk (200ml): From £0.92 to £1.06 - Aptamil Lactose Free powder (400g): From £7.70 to £8.39 These changes, effective from January 2024, are intended to ensure that pharmacies are appropriately reimbursed for the cost of these products.

Pharmacy professionals due to submit their revalidation records on or before May 31, 2022 only need to present a reflective account until June 1, 2022, the General Pharmaceutical Council (GPhC) has said. GPhC has extended the deadline, considering the continuation of pressures related to the Covid-19 pandemic and the Omicron variant, that pharmacy professionals are facing this winter. Changes to revalidation requirements were first introduced in March 2020 due to pressures on pharmacy professionals during the pandemic. The changes were then extended until March 1, 2022, with the GPhC remaining committed to keep the situation under review. "Our current intention is that full revalidation will not resume until the emergency powers for the pandemic introduced by the secretary of state for health and social care are removed, but we will continue to keep this under active review," GPhC stated.

"If we stand still, we will die. We have to keep moving, and we have to keep changing" said Patrick Gompels, co-owner of Gompels Limited, while emphasising the need for innovation in community pharmacy services, during the recent Pharmacy Business Conference 2024. Centered on the theme 'Pharmacy of Tomorrow,' the conference, held at the Hilton Wembley on April 28, focused on adopting and adapting innovative technology as well as new ways of working and thinking to enhance patient services. Patrick was joined by Mayank Patel of Pearl Chemist Group and Michael Lennox, CEO of Community Pharmacy Somerset (LPC), during the panel discussion on innovation in community pharmacy services, moderated by Reena Barai, pharmacist and owner of S G Barai Pharmacy. On the changes he made at Gompels, Patrick said: "The first thing we did was a complete overhaul of all of our systems. Pretty much everything was stripped down." He shared that Gompels has embraced innovation by partnering with forward-thinking companies like Titan PMR, Drug Comparison, and Real World Analytics to enhance their processes and data analysis capabilities.

At last Friday's Clinical Congress Conference, the NHSE Chief Pharmaceutical Officer for England at NHS England, David Webb, announced the upcoming launch of the Pharmacy Stakeholder Forum- an initiative led by NHS England aimed to improve pharmacy professional leadership and foster collaboration within the sector. Webb highlighted the forum's significance as a platform for driving transformative change in pharmacy leadership. "The aim is to deliver collaboration to develop a stronger and more united voice for pharmacy professionals," Webb emphasised. Highlighting the need to drive transformative change in pharmacy leadership, he underscored that "the overarching goal is to address key challenges facing the pharmacy profession and chart a course for future development and innovation." Scheduled to commence in June, the Pharmacy Stakeholder Forum will provide a space for diverse organisations and networks with a vested interest in pharmacy professional leadership to converge.

Pharmacies in England will soon be allowed to dispense up to 10 per cent more or less than the quantity prescribed for NHS prescriptions, thanks to the introduction of new original pack dispensing (OPD) rules. These changes are part of regulatory amendments laid on Tuesday, 3 September, and will come into effect over the coming months. The amendments are being made to the National Health Service (Pharmaceutical and Local Pharmaceutical Services) Regulations 2024 (PLPS). Starting 1 January 2025, pharmacies will have the option to adjust the prescribed quantity by up to 10 per cent for NHS prescriptions so medicines can be supplied in their original manufacturer's pack. Earlier in October 2023, changes to the Human Medicines Regulations (HMRs) permitted pharmacies to dispense up to 10 per cent more or less than the prescribed quantity for private prescriptions of Prescription Only Medicines (POMs).

The General Pharmaceutical Council (GPhC) is inviting feedback on proposed changes to its guidance for online pharmacies, aimed at enhancing patient safety. These changes are designed to tackle concerns associated with receiving medicines from online pharmacies, particularly those that may not be clinically appropriate and could potentially harm patients. GPhC has identified concerns relating to inappropriate supplies of weight loss medications that could pose risk to patients' health. Duncan Rudkin, Chief Executive of the GPhC, underscored the importance of addressing risks in online pharmacy services. "We know that there can be significant benefits for people using online pharmacy services to get medicines and treatment, but there are also additional risks that need to be managed, to make sure medicines and other pharmacy services are provided safely to patients and the public.

A harsh funding regime that hasn't adapted to changing macroeconomic realities means the viability of large parts of the pharmacy sector is at risk unless the model changes. Very simply, across the sector, revenue has remained largely fixed while costs have increased significantly, making it impossible for many pharmacies to sustain their business models. The sector has seen the closure of more than 1,500 community pharmacies since 2015, with 700 of these closures occurring since 2021. Larger pharmacy operators such as Lloyds, Boots, and Rowlands have seen the biggest decline in numbers, with smaller businesses operating between one and five pharmacies now accounting for almost 50% of the sector. Despite rising inflation and business costs, the NHS pharmacy funding model has remained fixed. Over the period of the current Community Pharmacy Contractual Framework (CPCF) - 2019-2024 - pharmacies have experienced a 30% real terms cut in core funding leading to an annual shortfall of over £750 million, equivalent to £67,000 per pharmacy in England. The current CPCF is due to end in 2024, and there is still no arrangement in place for future funding, leading to delayed and inefficient spending decisions and hampering the ability of the sector to plan and attract much-needed investment.

The British Generic Manufacturers Association (BGMA), the trade body for off-patent medicines, has made several recommendations to alleviate medicine shortages, which not only have serious consequences for patients but also increase workloads for pharmacists. England is currently experiencing twice as many medicine shortages as it did two years ago, with some products remaining out of stock for many months, the BGMA reveals in a report, noting that "the vast majority are commonly used generic medicines." "We have witnessed the number of products facing supply challenges double in the past two years and remain at a high level since September 2023," says BGMA chief executive Mark Samuels. In its report titled 'Solutions to UK medicines shortages' published on 1 October 2024, the BGMA proposes policy changes to tackle medicine shortages, including incorporating supply chain resilience and past performance in hospital medicines tenders. "Tender scoring should include whether supply issues were reported with adequate notice. Suppliers should be informed of changes in prescribing more quickly," the report says.

New amendments to the Human Medicines Regulations will soon allow registered pharmacy technicians to supply and administer medicines under Patient Group Directions (PGDs). Laid before Parliament at the end of May, the legislative changes will come into effect from June 26, 2024, the Community Pharmacy England (CPE) announced today. The introduction of this new legislation follows an announcement made by the Department of Health and Social Care (DHSC) earlier this year. The regulatory adjustments provide the legal framework for pharmacy technicians to engage in the supply and administration of medicines under PGDs in the course of their professional practice. However, there will be no immediate change in the provision of community pharmacy services, the CPE noted

Community Pharmacy England (CPE) has notified pharmacy teams that starting 1 August 2024, three Hormone Replacement Therapy (HRT) medicines will be discontinued and will no longer be covered under the NHS HRT Prescription Prepayment Certificate (HRT PPC) scheme. These are Bedol® 2mg tablets, Climanor® 5mg tablets and Clinorette® tablets. Since Climanor® 5mg tablets have been discontinued, Medroxyprogesterone 5mg tablets will also be removed from the HRT PPC list, as this generic product is no longer available as a licensed HRT medicine. The CPE also noted that Provera® 5mg tablets, being an unlicensed HRT medicine, are not covered by the HRT PPC. Pharmacy staff are advised to check the NHS Business Services Authority (NHSBSA) website for the most current list of HRT medicines covered by the HRT PPC. The CPE has updated its HRT PPC medicines list to reflect these changes, which will also be updated in Part XVI of the August 2024 Drug Tariff.

At this year's pharmacy show and many recent conferences, I have given presentations on behavioural economics. People's health outcomes are greatly determined by behaviours outside the pharmacy or other health settings. Whether they take their medicines, give up smoking or eat healthily, these choices are made at home in the course of their daily lives. How many minutes of the 168 hours in a week, are your customers in contact with you? For many pharmacists, behavioural economics is a 'new' discipline and often flies in the face of what we learned at university years ago. People had symptoms, often traced back to a disease, which led to a treatment, which the patient took (as instructed) and got better. If only! In the context of healthcare, behavioural economics shows that people do not always make decisions based purely on rational cost-benefit analysis. Instead, our choices are often shaped by factors like cognitive biases, social influence, and environmental cues. Once you've left university and started practicing in the real world, this should not come as a surprise. The fact is people do not always act in their own best interest and people do not always do the "right" thing, even if they know what the right thing is. Intention can be a poor predictor of actual health behaviour change-now termed the intention-behaviour gap. In other words, although patients intend to change and maintain their behaviour, the data suggest that many will not follow through with their intention.

In significant development, the Pharmacists' Defence Association (PDA) has advised its Boots members to challenge the "trustees' decision to end the ability to take an unreduced pension from age 60 by submitting complaints through the scheme's two-stage internal disputes procedure". The issue centers around the alterations made to pension scheme rules without the adequate disclosure to scheme members or their representatives. The PDA has drafted template letters to assist scheme members in response to undisclosed changes during a buyout process, sparking legal challenges and demands for transparency from Boots and scheme trustees. Paul Moloney, National Officer of the PDA Union leading the campaign, expressed concern over the sudden withdrawal of the right to an unreduced pension from age 60. "We have always maintained that members had a right under the rules to draw an unreduced pension from age 60," Moloney stated.

The General Pharmaceutical Council (GPhC) has unveiled its Carbon Net Zero Action Plan for sustainable pharmacy regulation, aiming to achieve carbon net zero by 2040. The action plan focuses on three main elements: setting an example, incorporating sustainability into regulatory standards, and fostering collaboration for sustainable pharmacy care. Roz Gittins, Chief Pharmacy Officer at the GPhC, emphasised the urgent need for pharmacy sector to embrace sustainable healthcare practices. "Climate change is a global emergency and has been declared the biggest threat to public health," she said, adding that it not only poses health risks but also amplifies existing health inequalities, with immediate consequences for patients, the public and health services. She highlighted that the GPhC is actively engaged in addressing climate change, both as an employer and regulator. "Our aim is to positively influence the pharmacy sector to embrace climate action and sustainable healthcare principles in line with our vision to have safe and effective pharmacy care at the heart of healthier communities," Gittins said.

Pharmacy minister Stephen Kinnock highlighted the role of community pharmacies in the Labour government's proposed healthcare reform during his speech at the 24th annual Pharmacy Business Awards on Wednesday. He outlined the government's vision for three strategic shifts: from hospital to community, from analogue to digital, and from sickness to prevention. Emphasising the crucial role that community pharmacies will play in achieving these changes, he said: "Pharmacies are based in, and recruit from, and are part of their communities, and we have to put you right at the heart of those three shifts if we are to deliver the change that our country so desperately needs." Kinnock also acknowledged the contribution of pharmacies in safeguarding the nation's health. He stated: "I've already heard, just in the short time that I've been in government since 4 July, of many examples of just how much patients and communities rely on your services and the lengths to which you go to deliver care."

As per a statement from Watford-based industry leader, they raised £20,000 for Citizens Advice Watford, their selected charity of the year, during the recent Sigma annual conference held in Sun City, South Africa. Additionally, Sigma played a vital part in raising more than £52,000 for 'Team George' at the London Marathon Charity run held on Sunday, 21 April 2024 to support cancer charities. Throughout the year, Sigma will continue to partner with Citizens Advice Watford, a local charity that provides frontline service to the community, giving free, expert and impartial advice and support to 8,000 people a year, many of whom are facing crisis situations. Applauding the work done by this charity, Dr Bharat Shah CBE, Founder and Managing Director at Sigma, said: "It has been eye-opening to learn about the life-changing impact the charity has on the lives of so many people. "We have seen how their expert team works day in and day out to speak up for people who have nowhere else to turn, ensuring they receive the support and services they're entitled to."
