The Department of Health (DoH) in Northern Ireland has published a new Strategic Plan for Community Pharmacy, highlighting the vision and key priorities for the sector over the next 10 years. Co-developed by the DoH and Community Pharmacy Northern Ireland (CPNI), the representative body for community pharmacy owners, the plan aims to fully realise the potential of community pharmacy services to support better health outcomes from medicines and prevent illness. The vision outlined in the plan sees community pharmacies offering more clinical services, leveraging the skills of their teams to provide the public with safe, convenient, and faster access to care. According to DOH, the vision will be realised through four major reform programmes focusing on services, legislation, workforce, and digital enablers, alongside six strategic priorities. Together, these initiatives are anticipated to transform the role of community pharmacy within the Health and Social Care (HSC). Health Minister Robin Swann has extended his support to the new Community Pharmacy Strategic Plan, which builds on the previously agreed 3-year community pharmacy commissioning plan.

Calling it "a severe blow to an already financially strained sector", McQuillan emphasised that the additional burden has the potential to threaten the viability of many pharmacies. "Community pharmacies are the frontline of our healthcare system, by penalising these pharmacies for an error not of their making, the Department of Health is jeopardising the foundation of local healthcare delivery," McQuillan criticised. Here are the potential implications: 1. Prevention Focus Streeting's vision of a shift from a reactive, hospital-based model to a preventive healthcare system presents a promising future for community pharmacies. These changes could significantly reduce pressures on GPs and hospitals, integrating pharmacies more closely into primary care networks and enhancing their role in preventive care. 2. Digital Transformation The move from an "analogue to digital" NHS could see community pharmacies adopting more digital tools and systems for service delivery. This would likely involve greater use of digital prescribing, telehealth consultations, and AI-driven services. Pharmacies will need to invest in digital infrastructure and ensure that pharmacy professionals are trained to operate within a more tech-centric environment. 3. In its closing oral statement to the UK Covid Inquiry Module 3 on Wednesday (27 November), the National Pharmacy Association (NPA) urged the government and NHS to "recast community pharmacy in their minds as a valuable partner in the post-pandemic recovery, and not as a cost center." The Inquiry was also asked to recommend sufficient government investment in the network and infrastructure needed to integrate community pharmacy into the broader health system and support effective cooperation across the health service. Additionally, the NPA requested that the significant contribution of community pharmacy to the pandemic response be reflected in the Module 3 findings to "redress the lack of recognition they received throughout the pandemic." The NPA's closing statement addressed four key issues - the role of community pharmacy, the impact on pharmacy teams, the resilience of the pharmacy network, and the lack of recognition given to the sector during the pandemic and since. Lee John-Charles, speaking for the NPA, told the Inquiry that "Community pharmacy became the first port of call for patients seeking health advice during the pandemic." "Many community pharmacies went to great and heroic lengths to ensure that their services were maintained during the pandemic and demonstrated the value of the network of community pharmacies across the country."

The Welsh government has reached an agreement with the NHS and Community Pharmacy Wales (CPW)-the statutory representative body for all community pharmacies-on a six per cent funding increase as part of the 2024/25 Community Pharmacy Contractual Framework (CPCF). The £9.9 million increase will bring the total CPCF funding to £175 million per year, representing a 24 per cent rise in funding since 2017. In a statement published on 20 December 2024, Jeremy Miles, cabinet secretary for health and social care, said that the additional investment will be used to address "both immediate pressures facing community pharmacies and to maintain momentum on our longer-term ambitions for reform." Miles also said that the government would continue to invest in integrating pharmacies in primary care clusters, developing the skills and scope of practice of the wider community pharmacy workforce including pharmacy technicians, and the Choose Pharmacy IT system. In contrast, community pharmacies in England are still awaiting progress on CPCF negotiations for 2024/25. Last month, Community Pharmacy England (CPE) warned of the sector's worsening financial situation, urging ministers to provide immediate funding relief.

Community pharmacy is stuck in a vicious circle. Pharmacies are closing at an alarming rate whilst the government is talking about moving care away from hospitals and into the community to focus on preventative care. While the government talks about this vision for the future, the stark reality is that pharmacies are already struggling to meet the current Pharmacy First thresholds. And now we discover that one of the key components that could create capacity in pharmacy to deliver more clinical services has been put on hold…indefinitely. The recently published Darzi review, which was commissioned after the new government came to power to assess the state of the NHS, praised the value of community pharmacies and preventative services. But it recognised the level of pharmacy closures across the country and warned pharmacy access could be 'at risk'. Community pharmacy is ideally placed to deliver the government's vision of preventative care in the community. But let's all be clear, moving preventative services into community pharmacies has, so far, been nowhere near as successful as it could have been due to the current climate pharmacies find themselves in. The Pharmacy First scheme is a testament to this. Just this week it was announced that consultation thresholds for the Pharmacy First scheme have been reduced from 30 consultations to 20. It's the second time in three months that the thresholds have been reduced and it follows an intervention from Community Pharmacy England who wanted to "save many pharmacies from missing out on a vital payment this month." In June alone, 3,269 participating pharmacies failed to meet the threshold - that's almost a third of all community pharmacies in England.

Prime Minister Rishi Sunak, during today's (15th May ) Prime Minister's Questions (PMQs), reiterated his commitment to supporting community pharmacies amidst growing concerns over pharmacy closures. Addressing questions regarding the Pharmacy First initiative, Sunak assured the House of Commons of his dedication to bolstering resources for these vital healthcare providers. In response to a query raised by Member of Parliament, St Ives MP Derek Thomas, citing statistics from the National Pharmacy Association (NPA) about "committing to directing funds towards Community Pharmacy to alleviate financial pressure and prevent closures, Sunak remarked that he "cares deeply about the future of community pharmacies." Sunak acknowledged the indispensable role played by community pharmacists in alleviating pressure on urgent care services, emphasising the importance of initiatives like Pharmacy First in government's commitment to supporting community pharmacies. "There are over 10 and a half Community pharmacies across the country working incredibly hard to serve their patients," Sunak said praising the community pharmacists and highlighted the concerning trend of pharmacy closures, citing a significant increase compared to the previous year.

Health secretary Wes Streeting has pledged to stabilise the community pharmacy system before enabling it to take on a broader role in healthcare, as he addressed the Health and Social Care Select Committee in Parliament on Wednesday (18 December). "Now that we have got the budget set, we will be consulting with Community Pharmacy England shortly in the New Year," Streeting stated. "I do want to reassure pharmacists across the country that we are taking into account the enormous pressures they are under when thinking about allocations so that we can stabilise the system and create the foundations from which to recover." The health secretary emphasised that his top priority is to "stabilise the [community pharmacy] system" before expanding initiatives such as Pharmacy First and independent prescribing. He also expressed a vision for community pharmacies to play "a bigger role… as part of the shift from hospital to community," but stressed that immediate steps are required to address the sector's current financial strain. The announcement that Community Pharmacy Contractual Framework (CPCF) negotiations will commence early in the New Year was reaffirmed by Streeting, echoing a statement made earlier in the week by minister for care Stephen Kinnock during a Westminster Hall debate.

Backed by a £22.5 million of government funding, community vaccine champions will tap into their local networks and encourage people in their communities to get vaccinated against the Covid-19. The funding, allocated today (January 13) to 60 councils, will help deliver bespoke projects in these regions, including: In Derby and Newham roving vaccinators and a vaccine bus will visit workplaces, places of worship and local highstreets.Videos shared on social media featuring local sporting stars, TV actors, local community and faith leaders and local NHS staff in Southampton.Developing a community transport to support access to vaccination sites/pop-ups and considering 'Grab a Jab' taxis scheme.As per NHS statistics, over 4.6 million people aged 18 and above remained unvaccinated in the 60 councils, selected to be a part of the Community Vaccine Champions programme. Vaccines minister Maggie Throup said: "Thanks to the pace and scale of our Covid-19 vaccination programme, we are in a much better position than this time last year."

Stephen Kinnock, Minister of State at the Department of Health and Social Care (DHSC), discussed all primary care contracts, including those related to community pharmacy, during his meeting with Community Pharmacy England (CPE) Chief Executive and two members from the Negotiating Team. The pharmacy minister reiterated that community pharmacies will be central to the new government's health and wider missions. CPE Chief Executive Janet Morrison and her team updated Kinnock on the community pharmacy sector, highlighting the pressures throughout the summer. They underscored the critical challenges facing the sector and the urgent need for a funding lift. They also discussed Pharmacy First and emphasised the broader potential for pharmacies to further support the health of their local communities.

Janet Morrison, CEO of Community Pharmacy England (CPE) has emphasised that stabilization of the sector is essential before any solutions can be delivered to fix the "struggling" health service. Speaking at the recent SIGMA UK Community Pharmacy Conference 2024, Janet described the current contract as "unaffordable" and warned that pharmacies are becoming "unsustainable." "Without an urgent funding uplift, the sector is facing a 'house of cards' collapse that will seriously impact communities, patients, and the safety of medicine supply. "So, first of all, we have to secure the stabilization. We have to make sure you can keep the lights on and the doors open, and that's critical," she stated. Janet pointed out that if the sector is stabilized and a fair deal is achieved, community pharmacies could effectively deliver solutions for the government that meet their policy goals. However, she noted that negotiations for the Community Pharmacy Contractual Framework (CPCF) for 2024-2025 cannot resume until the budget is published on October 30.

There is a clear opportunity for community pharmacists and their teams to lead best practice when it comes to supporting the ongoing health needs of older people. As the global population continues to age, the demand for specialised healthcare services tailored to the elderly is rapidly increasing. Community pharmacists, with their accessible locations and frequent interactions with patients, are uniquely positioned to play a pivotal role in addressing this growing need. The ageing population presents a complex set of health challenges, including multiple chronic conditions, polypharmacy, and increased susceptibility to adverse drug reactions. Community pharmacists, armed with their extensive knowledge of medications and their effects, can serve as the first line of defence in managing these issues. By leveraging their expertise, they can implement best practices that not only improve the quality of life for older individuals but also reduce the burden on the broader healthcare system. One of the primary areas where community pharmacists can make a significant impact is in medication management. As people age, they often find themselves taking multiple medications for various conditions. This polypharmacy can lead to dangerous drug interactions, side effects, and compliance issues. Community pharmacists can lead the way in implementing comprehensive medication reviews, ensuring that each patient's drug regimen is optimised for their specific needs and minimising potential risks.

We're approaching that time of year where the sun is supposed to be shinning and things are supposed to quieten down over the summer holidays (note I said supposed to. . . well we have the sun at least). The elections are over, and a new party is in power. Wes Streeting is the Secretary of State for Health and Social Care. He's met with GPs and Dentists and has great ambitions for Primary Care but when will he meet with community pharmacists? To be fair to Wes, he did visit a local pharmacy in his constituency, with CPNEL last year as Shadow Health Secretary (and was a guest speaker at the Pharmacy Business Awards 2023). He has also had meetings with NPA and CPE, so we know he understands the value of Community Pharmacy. The question is, do we as a sector and profession need to shout louder so that we aren't forgotten about, as healthcare plans are being drawn up? Community Pharmacy is a humble sector, not unlike most other HC sectors. The difference is that we often get forgotten. I have lost count of the times I've heard in meetings or conversations "Primary care and community pharmacy. . ." totally disregarding the fact that we are part of primary care.

To maximise the benefits of the Pharmacy First service for patients, the NHS, and community pharmacies, Community Pharmacy England (CPE) has advised the community pharmacists to to establish a robust relationship between community pharmacies and local general practices. The news comes after several reports highlighted issues regarding GP referrals for the seven minor ailment conditions under the Pharmacy First services. According to the reports, many pharmacists claimed that "many GPs in England are not sending patients to them to be treated - and that some are refusing to participate at all in the "groundbreaking" Pharmacy First scheme". In response to foster better relationship with the GPs, CPE has said that strong local collaborations will enable general practice teams to confidently refer patients to the Pharmacy First and other community pharmacy services where appropriate. Moreover, the organisation emphasised implementation of long-term changes in practice will take time. The current focus on integrating GP referrals to Pharmacy First will likely require sustained effort over several months or longer.

The National Pharmacy Association (NPA) views the COVID vaccination programme as broadly successful, largely due to the efforts of healthcare workers and volunteers. However, it cited some areas that could be improved, notably the early involvement of community pharmacies in planning, operational challenges, and recognising the role of community pharmacy in addressing vaccine hesitancy and inequalities. Submitting its evidence to the Covid-19 Inquiry, the NPA highlighted that the community pharmacy should have been consulted and involved earlier in the planning process, considering their two years of experience and expertise in administering annual influenza vaccination programs and the reach and resources of the 14,000 community pharmacies across the UK. "Instead, community pharmacy was initially given a gap in service role, which failed to fully utilize their experience, expertise and resource," Lawyer Brian Stanton told the Inquiry on behalf of the NPA during the Module 4 Preliminary Hearing on 22 May.

The Company Chemists' Association (CCA) has responded positively to the Labour Party's manifesto, which outlines plans to bolster community pharmacy services and shift the NHS towards a more localized, prevention-focused model. Key proposals include the creation of a Community Pharmacist Prescribing Service and the enhancement of care delivery in local communities. Malcolm Harrison, Chief Executive of the CCA, expressed strong support for these initiatives and said: "We welcome the Labour Party Manifesto's clear focus on moving to a 'Neighbourhood Health Service', prevention, and the return of the 'family doctor'." "Harnessing community pharmacy is crucial to delivering each of these," Harrison added.

A group of 20 parliamentarians has written to health secretary Wes Streeting MP, calling for urgent government action to address the growing financial crisis facing community pharmacies. In the cross-party letter, supported by Community Pharmacy England (CPE), the MPs warned of a real risk of more pharmacy closures, with one in six pharmacy owners saying that they are unsure if their pharmacy will survive the year. They have urged the Department of Health and Social Care (DHSC) and NHS England (NHSE) to prioritise long-term planning for pharmacies, including the implementation of the Community Pharmacist Prescribing Service. The MPs have also highlighted the need for more effective marketing for Pharmacy First to increase both walk-ins and referrals to this critical service. CPE chief executive Janet Morrison welcomed the support of 20 cross-party parliamentarians and recognising the vital role pharmacies play in the local community. She stressed the urgent need for immediate government intervention to ensure the survival of the sector and to maximise its potential in providing accessible care.

In a stirring address at the recent conference on Pharmacy at the Westminster Health Forum, Director of NHS Services at Community Pharmacy England (CPE), Alastair Buxton presented a stark assessment of the community pharmacy sector's challenges. Highlighting years of financial constraints imposed by the NHS and government, Buxton warned of a sector teetering on the edge of survival. "There have been no years of financial and operational squeezes applied by the NHS and government to the community pharmacy sector, so as a consequence, there are a lot of pharmacies that are on the on the brink of survival -battling for survival," he said. Buxton underscored the critical and indispensable role of community pharmacies in delivering essential NHS services, from dispensing medications to innovative initiatives like the Discharge Medicine Service and Contraception Service.

Maxine McCabe has made history as the first community pharmacist in the UK to be credentialed as a core advanced pharmacist by the Royal Pharmaceutical Society (RPS) in Scotland. Announcing McCabe's achieving, NHS Education for Scotland (NES) stated that this credential demonstrates her expertise across the four pillars of advanced practice: clinical, leadership and management, educating others and research. Maxine has worked in the community pharmacy sector since qualifying as a pharmacist in 2008, consistently showing a keen interest in developing the wider pharmacy team and enhancing clinical services for the local patient population. Throughout her career, she has managed various community pharmacies, including Boots and Parkhead Health Centre Pharmacy, where she played a crucial role in developing pharmacy practice and nurturing pharmacy support roles and trainee pharmacists. In 2023, Maxine took on a part-time role as a Senior Educator in the Prescribing & Clinical Skills teams at NHS Education for Scotland (NES). She continues to practice as a pharmacist, leading the Teach and Treat Training Hub at Burnside Pharmacy in Lanarkshire. McCabe expressed her excitement about the recognition, saying, "I am delighted to obtain my Advanced Practice credential and showcase that this is achievable in community pharmacy."

With the UK General Election set for 4 July, pharmacy bodies are actively engaging with all political parties to secure support for the community pharmacy sector. They are pressing parties to address urgent issues such as pharmacy closures and medicine shortages. "The upcoming general election is a critical opportunity for us to strengthen supporters of community pharmacy in readiness for the new Parliament," said Janet Morrison, chief executive of Community Pharmacy England (CPE). She stressed the need for unity within the sector in advocating for community pharmacy and raising awareness of the issues with election candidates from all parties. As the general election approaches, CPE has called upon all political parties to support pharmacies by "introducing a long-term sustainable funding model and reviewing the medicines supply chain, including much-needed short-term relief measures."
