Improving Healthcare Systems - December 2013 Cycle | Patient-Centered Outcomes Research... - 0 views

    PCORI is seeking applications to study the comparative effectiveness of alternate features of healthcare systems (e.g., innovative information technologies, personnel deployments, incentives, service designs, etc.) designed to optimize the quality, outcomes, and/or efficiency of care for the patients they serve.  Healthcare systems include:  private and public health insurance plans; physician groups; hospitals, nursing homes and assisted living facilities; academic medical centers; integrated delivery systems; community-based and safety-net clinics; federal, state, and municipal providers; and other entities organized to deliver, arrange, purchase, or coordinate healthcare services.  PCORI seeks studies that will provide information of value to patients, their caregivers and clinicians, as well as to healthcare leaders, regarding which features of systems lead to better patient-centered outcomes.

    The purpose of this FOA is to solicit applications for the AD/ADRD Health Care Systems Research Collaboratory ("Collaboratory", henceforth) to improve care for persons with dementia (PWD) and their caregivers through health systems. Health and long-term systems for this FOA are defined broadly and include organizations providing care across settings to include primary and specialty outpatient care, acute inpatient care, skilled nursing and other rehabilitation facilities, residential long-term care, and home and community-based services. Organizations may be traditional health care systems (e.g., health maintenance organizations, or HMOs), health insurance companies, managed care plans, home health care providers, memory clinics, nursing homes, assisted living facilities, outpatient clinics, federally-designated health centers, hospitals, and other providers of acute and long-term care.

Systems Biology Approaches using Non-Mammalian Laboratory Animals to Uncover Causes of ... - 0 views

    The National Institute on Aging is seeking applications on systems biology approaches using non-mammalian laboratory animal models to increase our understanding of the basic biology underpinning neurodegeneration. It is expected that research supported under this FOA will provide new insights into molecular networks that might be involved in causing, amplifying or protecting against neurodegeneration, and that, in turn, might ultimately contribute to Alzheimer's disease or related dementias. Importantly, a major goal of this FOA is to use interaction and regulatory networks produced and analyzed using systems biology to gain these new insights. Because this FOA is directed toward discovery, currently employed genetically modified laboratory animals used to study AD are not required, although they may be used. Because this FOA requires systems biology approaches, data used to build interaction or regulatory networks may also come from humans or other mammals in which AD, related dementias, or aging-related cognitive decline have been observed. This FOA will only support studies using non-mammalian laboratory animal models; studies involving humans or experiments with mammals will not be allowed under this FOA.

FY2017 Lifespan Respite Care Program: Advancing State Lifespan Respite Systems - 0 views

    The Lifespan Respite Care Act’s purpose is threefold: expand and enhance respite care services to family caregivers; improve the statewide dissemination and coordination of respite care; and provide, supplement, or improve access and quality of respite care services to family caregivers, thereby reducing family caregiver strain. Since 2009, ACL has been making grants to states to develop or enhance their ability to provide coordinated, high quality respite services to caregivers of persons of all ages with disabilities. States who have received grants under this program have made considerable progress in building or strengthening their capacities to provide respite services. With this new funding opportunity, grantees will be asked to carefully examine the extent to which their state has a Lifespan Respite System and propose approaches that enable greater realization of such a system.

    The objective of this funding opportunity announcement is to build upon work previously done in this area by promoting continued and expanded engagement in national peer-to-peer networks, furthering the design and implementation of small grants demonstration programs at the community level, strengthening and refining evaluation and continuous improvement processes, and expanding dissemination activities. 2019 Evidence-Based Falls Prevention Program - 0 views

    The Administration on Aging (AoA) within the Administration for Community Living (ACL), U.S. Department of Health and Human Services (HHS) forecasts the possible availability of Fiscal Year (FY) 2019 funds to make three-year grants to approximately 10 entities to develop capacity, bring to scale, and sustain evidence-based falls prevention programs that will help to reduce the number of falls, fear of falling, and/or fall-related injuries in older adults. ACL aims to compete these 10 grants via two options (both with 36-month grant periods): (A) Sustainable Systems Grants: approximately six grants of $400,000 to $600,00 focused on developing integrated, sustainable systems for delivering falls prevention programs; (B) Capacity-Building Grants: approximately four grants of $50,000 to $150,000 to build capacity to introduce and deliver falls prevention programs within underserved areas and/or populations.

    This FOA invites Research and Development Center (P30) grant applications in demography, economics and health services research relevant to Alzheimer's Disease and Alzheimer's Related Dementias (AD/ADRD). Areas of focus that are especially encouraged are: a) national and international population trends in cognitive aging and AD/ADRD; b) demography of dementia care and caregiving; c) economic burden of AD/ADRD; d) impact of health care systems and long-term supports and services on outcomes for persons with dementia and their care providers; e) impact of health care financing policies on outcomes for persons with dementia and their care providers; f) how regulatory and economic incentives affect access, quality and health outcomes in health and long-term care systems for persons with dementia; g) disparities in quality and access to dementia care; h) effects of population-level health delivery and care interventions on outcomes of persons with dementia; and i) national and international projections of dementia caseload, incidence and prevalence. Center grant applications must include two mandatory Cores and may choose among three optional Cores. The Alzheimer's Disease (AD) Patient Registry is a population-based longitudinal cohort study of AD and dementia that is linked to an HMO-medical record system or a similar record system.
