WARNING: Graphic and disturbing photos between 38:40 and 40:45. U.S. Army Ranger John Needham, who was awarded two purple hearts and three medals for heroism, wrote to military authorities in 2007 reporting war crimes that he witnessed being committed by his own command and fellow soldiers in Al Doura, Iraq. His charges were supported by atrocity photos which, in the public interest, are now released in this video. John paid a terrible price for his opposition to these acts. His story is tragic.  CBS reported obtaining an Army document from the Criminal Investigation Command suggestive of an investigation into these war crimes allegations. The Army's conclusion was that the "offense of War Crimes did not occur." However, CBS also stated that the report was "redacted and incomplete; 111 pages were withheld."  This video is placed with the context of Vice President, Dick Cheney, insistence that this nation's efforts "must go to the dark side;" which included ignoring the Geneva Conventions.  John's story is told, here, by his father, Michael Needham. It is produced in the spirit of the public interest and towards promoting justice and the rule of law.

The IOM's 1999 landmark report, "To Err is Human," dropped the first bombshell, reporting that between 44,000 and 98,000 Americans die in hospitals each year from medical mistakes, costing an estimated $17 billion to $29 billion annually. HHS' new finding that medical mistakes kill 15,000 Medicare patients a month equates to 180,000 Medicare deaths per year - more than the IOM's estimate, which attempted to cover all patients in the United States. That means that the annual death toll in this country caused by mistakes in hospitals is well over 250,000 deaths a year! But perhaps the most startling finding by HHS is that a significant number of patients suffered injuries or died needlessly, as 44 percent of the medical errors were preventable.

The text of the budget deal reached by President Barack Obama and congressional leaders contains few specific public disclosure provisions for the committee. The standing committees of Congress are allowed to send suggestions for ways to reduce the debt to the super committee members, but there is, as yet, no provision for the disclosure of those reports. The final report is required to be publicly disclosed upon completion, however there is no requirement that the report be placed online. There are also no official requirements for web-casting of committee meetings.

On November 2, 2016, the Securities and Exchange Commission ("SEC") Division of Corporation Finance released two new compliance and disclosure interpretations ("C&DIs") addressing eligibility to use Form S-3 registration statements and submission of annual reports to the SEC.

The Department of Justice has just released its annual report on its surveillance activities for 2010 including its use of secret court orders, National Security Letters (NSLs) and electronic and physical surveillance - and boy were they busy. Buried in the depths of the National Defense Authorization Act for FY2011 (H.R. 5136) is the latest salvo in the war on lawyers. In particular, section 1037 of the Act [page 403 of the PDF], titled "Inspector General Investigation of the Conduct and Practices of Lawyers Representing Individuals Detained at Naval Station, Guantanamo Bay, Cuba," instructs the Department of Defense IG to "conduct an investigation of the conduct and practices of lawyers" who represent clients at Guantánamo and report back to the House and Senate Armed Services Committees within 90 days.

In a new report (PDF) by the Open Society Institute, human-rights researcher Jonathan Horowitz contrasts the official prison system that the Pentagon has constructed in Afghanistan-where they often arrange press briefings and invite journalists on tours-with the super-secret facility run on the periphery of Bagram Air Base, the "Tor" or "Black Jail."

Today, hundreds of thousands of people comprising a Main Street Movement - a coalition of students, the retired, union workers, public employees, and other middle class Americans - are in the streets, demonstrating against brutal cuts to public services and crackdowns on organized labor being pushed by conservative politicians. These lawmakers that are attacking collective bargaining and cutting necessary services like college tuition aid and health benefits for public workers claim that they have no choice but than to take these actions because both state and federal governments are in debt.

The Big Money economy is booming. According to a new Commerce Department report, third-quarter profits of American businesses rose at an annual record-breaking $1.659 trillion - besting even the boom year of 2006 (in nominal dollars). Profits have soared for seven consecutive quarters now, matching or beating their fastest pace in history. Executive pay is linked to profits, so top pay is soaring as well. Higher profits are also translating into the nice gains in the stock market, which is a boon to everyone with lots of financial assets. And Wall Street is back. Bonuses on the Street are expected to rise about 5 percent this year, according to a survey by compensation consultants Johnson Associates Inc. But nothing is trickling down to the Average Worker economy. Job growth is still anemic. At October's rate of only 50,000 new private-sector jobs, unemployment won't get down to pre-recession levels for twenty years. And almost half of October's new jobs were in temporary help.

Speaking at the 25th anniversary celebration of the national media watch group Fairness and Accuracy in Reporting, world-renowned political dissident and linguist Noam Chomsky analyzes the U.S. response to the popular uprisings sweeping the Middle East and North Africa. "Across the [Middle East], an overwhelming majority of the population regards the United States as the main threat to their interests," Chomsky says. "The reason is very simple... Plainly, the U.S. and its allies are not going to want governments which are responsive to the will of the people. The NAACP has just passed a historic resolution demanding an end to the War on Drugs.  The resolution comes as young Black male unemployment hovers near 50 percent and the wealth gap's become a veritable gulf. So why is the forty-year-old "War on Drugs" public enemy number one for the nation's oldest civil rights organization? Well here's why:  it's not extraneous - it's central: the war on drugs is the engine of 21st century discrimination - an engine that has brought Jim Crow into the age of Barack Obama.     Author Michelle Alexander lays out the statistics -- and the stories --  of 21st Century Jim Crow in her ought-to-blow-your-socks off book: "The New Jim Crow: Mass Incarceration in an Age of Colorblindness." I had a chance to sit down with Alexander earlier this summer.
