The Department of Health and Social Care (DHSC) has commenced a consultation on allowing pharmacists to dispense prescription medicines in original packaging. The consultation, closing on December 13, 2021, proposes changes to the Human Medicines Regulations (HMRs). It would allow pharmacists to dispense more or less than the 10 per cent of the prescribed quantity in original packs, except where this would affect the patient's clinical treatment. The change would not apply to controlled drugs. The second proposed change is to supply sodium valproate in original packaging regardless of the conditions set around Original Pack Dispensing (OPD). Currently, contractors have to supply the exact quantity prescribed, unless certain exceptions apply.

Prime minister Boris Johnson announced on Wednesday (January 19) that restrictions re-imposed in England last month would be lifted by from next Thursday (January 27). In the latest development, the government has decided to end the guidance on working from home, asking employers to make adequate arrangements to return safely to work. New mask mandates and other measures aimed at curbing the spread of the Omicron coronavirus variant came into England today, as Prime Minister Boris Johnson eyes an expanded booster programme to help increase protection against Covid-19. From Tuesday morning (November 30), face masks are compulsory on transport and in shops, banks and hair salons. All international travellers must take a PCR test by the end of the second day after they arrive, and self-isolate until they get their result. That is in addition to restrictions on arrivals from 10 southern African countries, who have to enter hotel quarantine. Britain has reported 11 cases of the Omicron variant so far, and while the government says this number will rise, it says it is important to slow its spread until more is known about the variant's transmissibility and impact on vaccines.

The regulatory amendments proposed will enable pharmacists to dispense medicines in their original packaging for private prescriptions starting this Autumn, according to the Department of Health and Social Care. In the recently published draft OPD regulations, the DHSC has indicated that the alterations, permitting pharmacists to vary the dispensed quantity by up to 10 per cent to avoid splitting medicine packs, will be implemented for private prescriptions 'immediately upon the enforcement of the Human Medicines Amendment Regulations in the autumn,' as highlighted in a briefing by Community Pharmacy England. CPE stated that the regulations related to NHS prescriptions will come into effect when the pharmaceutical terms of service regulations expressly apply the OPD amendments. Moreover, new directive mandating the dispensing of sodium valproate products solely in their original packaging (except when an assessment of risk necessitates an alternative approach) will align with the rollout of the private prescription regulations during the autumn. CPE further noted that these regulations are currently in draft, indicating that they are not currently in effect and may undergo revisions prior to their implementation.

Pharmacy bodies are "bitterly disappointed" that the latest deal on the national contract makes no commitment to "fresh funding", with one organisation calling it "the biggest dis-service ever done" to community pharmacy. The only commitment made in monetary terms was one in which NHS England agreed to write off a sum of £100m in excess margin earned by contractors in previous years. This allowance, which can't be seen as new cash injection, was said to have been made in recognition of the pressures facing the sector. Welcoming the attendees, she assured everyone that the committee was well aware of the pressures the sector was facing. "I heard how contractors are feeling and their frustrations over growing pressure and lack of financial support from the government. They confirmed that many now are unable to deliver the full range of services, and others are struggling to maintain core levels of services. And the ongoing impact of capacity and workforce crisis is critical, leading to temporary closures.

The National Pharmacy Association (NPA) has partnered with training provider Skills 4 Pharmacy to provide Level 2 and Level 3 pharmacy apprenticeships to its members in England. Skills 4 Pharmacy is an experienced pharmacy apprenticeship training provider dedicated to initiating and developing the careers of pharmacy support staff and technicians. They manage the recruitment of apprentices, as well as their enrolment and training. The apprenticeship package offers a Level 2 Apprenticeship - Pharmacy support worker and Level 3 Apprenticeship - Pharmacy technician. In a recent interview with The Telegraph, health secretary Steve Barclay stated that he has asked his officials within DHSC to look at a "pharmacy first" approach to alleviate pressures on A&E departments in order to avoid the widely predicted NHS winter crisis. On the face of it, this a welcome if long overdue recognition that community pharmacy is an essential part of our national healthcare infrastructure alongside our GP and A&E colleagues. But let's not get carried away - we have had lots of praise from politicians in the past which have not then been backed by firm commitments for a sustainable future for the network. Could this be a turning point? I hope so, but I am not confident it will be. I fear this may turn out to be another emergency stop-gap measure which does nothing to secure the long-term viability of the sector in England. The role of community pharmacy during the recent Covid pandemic demonstrated clearly how important we are to ensure people have easy access to essential healthcare support, advice and services. The NHS winter crisis can only be avoided or at least mitigated if the potential of the community pharmacy network to provide more patient care services is unlocked and that Barclay requires you to end pharmacy funding austerity and start investing. The Treasury will no doubt say there is no more money, but what then the alternative other than a NHS winter crisis? And, of course, treating people in secondary care settings is far more costly than community pharmacy based interventions.

The NHS claims that it's been able to save taxpayers £1.2 billion in just three years by procuring hundreds of hospital medicines at a better price. The adoption of cheaper versions of a single drug - adalimumab - which is used to treat more than 45,000 patients with rheumatoid arthritis, inflammatory bowel disease and psoriasis, has accounted for about one third of the savings. After the exclusive patent on the drug - originally known as its brand name Humira - expired in 2018, the NHS struck cost-saving deals to bulk-buy generic versions, which have the same quality, safety and efficacy of a branded one. Since then, tens of millions of pounds have been saved by buying cheaper generic versions of other medicines for conditions ranging from severe skin infections to aggressive blood cancers. Four in five medicines prescribed in the NHS are now non-branded, helping the NHS to achieve significant savings while ensuring the continuity of high-quality patient care. NHS chief executive Amanda Pritchard said: "Smart deals by the NHS mean patients are getting the best medicines and taxpayers are getting best value.

The National Pharmacy Association (NPA) has called the future prime minister to deliver on the commitments made by the previous health secretaries to reform primary care through a 'pharmacy first' approach and fairly fund community pharmacies to avoid closures. In an open letter, the NPA has urged Rishi Sunak and Liz Truss, the two leadership contenders of the Conservative Party and prime ministerial candidates, to think radically about the role of England's 11,000 pharmacies, which are on the frontline of healthcare across the country. NPA also highlighted how pharmacies could help clear the NHS backlog, free up millions more GP appointments, dramatically improve access to primary health care and do more to prevent ill health and support people with long term medical conditions. "Because of our unparalleled presence in deprived areas, we also have great potential to level up access to healthcare and address health inequalities. We set these ideas out in How We Can Help, which is an improvement plan already welcomed by many of your colleagues."

Pharmaceutical Services Negotiating Committee (PSNC) has expressed its support for alterations in the Human Medicines Regulations 2012 to enable original pack dispensing (OPD) and supply of medicines containing sodium valproate by pharmacists. PSNC's support for changes was in response to the consultation on OPD, initiated by the Department of Health and Social Care (DHSC). The consultation had proposed regulatory changes to allow pharmacists to dispense (up to 10 per cent) more or less than the prescribed quantity of a given medicine in the manufacturer's original packs. Key aims of the proposal are to "support increased patient safety by ensuring patients receive the necessary information that is included in the original manufacturer's packaging about the safe and effective use of a product," DHSC said.

A pharmacist in a leading independent pharmacy chain shares his myth-busting Ramadan advice on managing health and the use of medication while fasting. While the holy month means total fasting in terms of oral consumption, it's still ok, says Ifti Khan, superintendent pharmacist at Well Pharmacy, to have vaccinations and blood tests and to use eye drops or other medical items that are not ingested through the mouth. It's also important for people to keep taking their prescribed medication over the Ramadan period. Anyone who is concerned should speak to their pharmacist about whether they can take their medicine at different times, as stopping it could have serious consequences. Mr Khan, who will be observing Ramadan himself, said: "Confusion can sometimes arise when it comes to medication and managing health conditions whilst fasting", adding it is important for patients to keep managing health during Ramadan. Having injections or vaccinations is fine, as are blood tests, using eye or ear drops, and using anything that patients don't consume orally - so nicotine and other transdermal patches, pessaries and dialysis, for example. It's also ok to eat and drink as a result of a condition which causes forgetfulness.

Britain almost doubled the number of visas awarded to foreign migrants coming to work in the health and care sector last year, government data showed, adding to pressure on prime minister Rishi Sunak over immigration ahead of the next general election. Immigration is one of the top three concerns for voters as the country gears up for an election expected later this year. Trailing the opposition Labour party in the polls, Sunak has vowed to bring it down. But data released on Thursday by Home Office was the latest set of statistics to show the scale of the challenge he faces. Britain granted 337,240 work visas in 2023, 26 per cent higher than in 2022, with the rise largely driven by those coming to the country to work in the care home sector. That follows figures released in November which showed annual net migration to the UK hit a record 745,000 in 2022 and has stayed high since. Visas under the 'Skilled Worker - Health and Care' route almost doubled, Thursday's data showed, rising by 91 per cent to 146,477 last year.
