A Theoretical Model of The Transition to the Primary Care Nurse Practitioner Role

These are the Adult Gero Acute Care NP competencies that were developed by the National Organization of Nurse Practitioner Faculties (NONPF) in 2012. Interesting that these were only developed 3 years ago!

This article addresses the importance of Nurse Practitioners in the work force. Currently, there is an ever growing healthcare worker shortage, especially in light of health reform coverage expansions in 2014. This article goes on to address the disparities found in state licensing and the scope of practice of Advanced Practice Registered Nurse.

This article discusses the effects of states' scope of practice laws and payment policies on the role of NPs in the primary care setting.

The following is the abstract as stated by the author: "Purpose: To describe factors that ensure a smooth transition from nurse practitioner (NP) student to independent NP during the first year of practice. Data sources: Two contrasting case examples, the authors' experiences, and selected professional literature. Conclusions: Key points to consider upon searching or taking a first NP job include patient, colleague, and clinic factors. Patient factors include mix of patient presentations, complexity of patient presentations, insurance status, and population. Colleague factors include mentorship, charting, mix of providers, and availability of providers. Perhaps most importantly, clinic factors include productivity expectations, practice mission, charting systems, on-call requirements, supervision of other staff, and teaching status. Together, these factors can largely determine whether an NP's days are satisfying or frustrating in a new job. Implications for practice: The transition from NP student to independent NP can be daunting. Although nursing schools and practice sites have responsibilities to ensure the transition is smooth, the new NP is ultimately responsible for the transition. [ABSTRACT FROM AUTHOR]"

Research article that discusses practice setting characteristics that were either helpful or stressful to new grad NPs during their first year of practice. Primary care settings in Canada were sites used in this study, but the results would likely apply to acute care settings as well.

Discusses challenges faced by new NP grads as they transition from student to NP.

This article describes feeling like a fraud once in a new role as an advanced practice nurse. It also speaks to how this is a normal feeling and suggests different ways to cope with these feelings to make the transition easier.

This article examines the documented positive impact of full practice legislation on patient outcomes. Potentially avoidable hospitalizations, readmission rates after inpatient rehabilitation, and nursing home resident hospitalizations were explored. States with full practice of nurse practitioners have lower hospitalization rates in all examined groups and improved health outcomes in their communities.

This article highlights the changes in policy that are needed to enable nurse practitioners their full scope of practice in a way that benefits patients and families at end-of-life. Nurse practitioners need the ability to practice to their full scope of practice, including providing autonomous care, receiving adequate and equal reimbursement for services, and having the authority to prescribe controlled substances II through V. Nurses also need to endorse policies that require specialized education so they receive the knowledge and skills necessary to be leaders in end-of-life care.

This article is very relevant to our discussion today in lecture. It speaks to the consensus model, our primary certification after graduation, and then speciality certification. Additionally the articles outlines what is required to work in the ED. A concise and easy to follow article.

The orientation described in this article is something that sounds appealing to me as a new grad. It exposes practitioners to didactic and simulated experiences, focused peer support, formalized resources, and includes a preceptor.

With increasing interaction between governing bodies and cultures this article explores the APRN scope of practice on a global scale. It also examines access to care as a global issue postulating the need to standardize the scope of the Nurse Practitioner.

Role of FNP

This article discusses the difficulties APRNs experience while attempting to expand their scope of practice. It explores arguments between nurse practitioners and physicians on how APRNs will contribute to the shortage of primary care providers.

This article describes entry-level competencies for NPs practicing in emergency care - it was the first to define these competencies which is key since the roles of APRNs have expanded. A Nurse Practitioner (NP) Validation Work Team was formed and a Delphi study was conducted to verify and gain consensus on professional and clinical competencies for NPs in emergency care.
