By drawing highlights across different selections of similar texts within the same page, I hoped to identify something within the page that might cause the problem. Unfortunately not.
In the screen shot below (taken before reloading the page):
* after seeing what's missing from my library I set the colour of all missing highlights to pink
— I assume that all these will be lost when I reload the page
* for highlights that are consistent, I set the colour to green
* for the strange single-word/single-sentence issue described at comment #5 above, I left the colour set to blue.
Amongst the items at http://www.wuala.com/Diigo-4/978866 there's a WebArchive that contains the page resources but I'm not sure whether it's an issue with the content of the page. The main problem here is that no warning is given when highlights fail.
(It would be nice to know the cause of the failure, but failure without warning is a PITA.) For the moment, I view this topic as separate from the bundle (of six or so topics) in which highlighting and annotations are unreliable.

Jokes aside, this is seriously embarrassing. What I'm highlighting is related to a private web working group that was set up yesterday to turn things around in a very tight timescale. Hey ho.
