If you wait long enough, does the beach ball disappear? Depending on your hardware and the nature of this issue, a wait of some minutes may be required; I saw a video of the 'fiasco' outlined above in which the user had to wait a ridiculous length of time before Firefox started.
In the annotations area of the sidebar of Diigo for Firefox: do you allow all annotations, or do you prefer to not see some?
Have you tried clearing your Firefox cache and other caches?

Here using Diigo 3.1.6.16 in Firefox 3.5 on Mac OS X, I can't reproduce the problem.
Re Troubleshooting extensions and themes if you disable all extensions except Diigo, does the problem occur?
