Systematic Review of the Effects of Exercise on Activities of Daily Living in People Wi... - 0 views

  •
This is a systematic review that looks at the evidence for the effects of exercise on ADL functioning of people with AD. This review also looked at the effects of exercise on cognition and mood and on physical performance. The initial search for articles for the systematic review began with over 2000 articles, and the reviewers narrowed it down to 6 articles. Overall, they found that exercise can, in fact improve ADL function in people with AD. Exercise can also improve physical performance, and may slow the decline in cognition and boost mood in those with AD.

This article used 6 other articles to review effects of exercise on ADL performance of those with Alzheimer's Disease. Occupational therapy intervention that includes aerobic and strengthening exercises may help improve independence in ADLs and improve physical performance in people with Alzheimer's. More research needs to done to identify specific components of intervention and optimal dosage to develop clinical guidelines. At my SNF placement, I have worked with many residents with Alzheimer's. We do use strength training with most patients is they are able to do it, and we also work to increase endurance as well. It is good to see evidence to back up treatment

Effectiveness of Interventions to Prevent Falls in People With Alzheimer's Disease and ... - 1 views

  •
This is a systematic review of the effectiveness of interventions to prevent falls in people with the psychosocial disorders of Alzheimer's and dementia. I picked this article because my fieldwork is currently in a skilled nursing facility and I read about the many falls that happen that lead to the patients coming to the facility. The article focuses on three types of interventions including exercise, nursing staff interventions, and multidisciplinary interventions. the article mentioned that although all three intervention showed some data of effectiveness the studies used had many limitations. In addition, occupational therapy was seldom mentioned in the studies, the article mentions the big opportunity there is for occupational therapist to address this issue.

Effective Fieldwork Supervision: Occupational therapy student perspectives - 0 views

  •
This article examines the relationship between the fieldwork educator and the occupational therapy student; specifically, effective fieldwork supervision from the student perspective. It looks at the characteristics of effective fieldwork supervisors as well as the limitations on effective fieldwork supervision. Major characteristics emerged: well-developed interpersonal skills and the use of collaborative and facilitatory teaching/learning strategies. An environmental issue that emerged was lack of time for student supervision.

I am using this study as an example of clinical reasoning in the field. The systematic review of k tape effectiveness for musculoskeletal injuries found 6 studies that met inclusion, exclusion, and quality criteria. Only 3 of the 6 studies found K tape to have significant positive outcomes, 2 studies concluded that K tape had positive short-term benefits for shoulder injuries and 1 study found to have positive short-term benefits for whiplash injuries. The review concluded that there is insufficient evidence for or against K tape effectiveness on musculoskeletal injuries, however, the patient may perceive a benefit from its use. After reading this article a therapist has to use there best clinical reasoning when treating there patients with K tape . A therapist has to consider that K-taping may provide a perceived pain reduction to the patient and should be used in conjunction with other modalities and therapy approaches. However K- tape is not meant to be a stand-alone treatment. Patients experience and clinical observation by skilled therapists should be taken into account for K-tape use.

In this letter to the editor in AJOT, an OT working in a mental health setting describes the difficulty of proving the effectiveness and cost-effectiveness of OT in mental health. When asked to provide such evidence at her work place, even with the help of some of the prominent mental health OTs she was unable to find enough research. She makes a call for those interested in the mental health profession to begin to do more research in the area in order to protect OTs role in the setting.

Effects of a soft prefabricated thumb orthosis in carpometacarpal - 1 views

  •
This article examines the effect of a prefabricated soft thumb-base orthosis on pain, hand strength, and activity performance in persons with osteoarthritis of the CMC joint. At my facility, I see quite a bit of CMC arthritis and wanted to explore the use of an orthotic since most of my pt.'s say it helps with pain and protection of their thumb. Much to my surprise, this study found that the soft prefabricated orthosis seems to have an immediate pain-relieving effect during use, but no effects in terms of less hand pain, or improved strength or activity performance when not worn. There were no significant difference found between the experimental (orthosis+exercises) and control (exercises only) groups.

Individuals experiencing mental health disorders are susceptible to impairments in occupational functioning due to decreased concentration, memory, thought processes, insight, judgement, decision-making, reality orientation, regulating emotions, understanding perception, motivation, and psycho-motor activity. This is a quantitative study out of the South American Journal of Occupation Therapy that studied the effects of slow vs. fast tempo of music on symptoms of psychosis during occupation. There were 160 individuals with diagnosed psychological disorders in a psych hospital in South Africa. The subjects participated in a leatherwork activity while listening to either fast paced/upbeat music or low tempo/slow music. Attention/focus, ability to follow instructions, directedness towards the activity, and motivation were measured in groups of individuals who had psycho-motor agitation or psycho-motor inhibition psychosis. The study found that listening to music during an activity has the ability to improve all three variables in both agitation and inhibited-motor psychosis. Fast-paced tempo significantly improves attention in individuals with inhibited motor-psychosis. The ability to follow instructions improved in both fast and slow tempo music for both agitated and inhibited individuals. Directedness and motivation did not significantly improve in any of the groups. This study indicates that music can improve the ability to perform occupations and activities in individuals with mental health disorders. Music is very individualized and it may be difficult for OT practitioners to work with groups of individuals when introducing music. IT is important for an OT to gather a good understanding of a patient's preferences and determine if they would be a good candidate for the use of music during therapy. More research is recommended in this area to further determine the effects of music on occupation in this population.

Effectiveness of the California Tri-Pull Taping Method for Shoulder Subluxation Poststr... - 1 views

  •
I was very excited to talk talk about this article. During my rotation, I have found that my FE uses kinesiotape fairly often with patients, depending on condition and circumstance, obviously. I know in our MOT class/faculty, we definitely had some "doubters" in the effects kinesiotape can have. One of my patients was a lady in her 70's who had a CVA more than 10 years ago, and has since suffered from significant shoulder subluxation. She came in to therapy after trialing botox. When she came to us, she was very discouraged that botox had not helped, and also discouraged because past OT had not helped her at all, and was now dealing with significant pain from subluxation. After research and discovering this article, I discussed with my FE and we agreed that this California Tri-Pull taping method would be appropriate to try with our patient. In short- the patient found that the tape really helped her, and felt like it was the first thing that had provided her a little bit of relief in a long time. -The taping method itself actually is not too complicated, and did not take long to practice and learn before completing on a patient. In short- this article discusses the methodology for taping, as well as research that backs the effectiveness of the taping method. Overall, the study finds some mixed results in effectiveness in terms of reducing subluxation and reducing pain, but overall can be a promising adjunct to the management of the hemiplegic subluxed shoulder

Horseback Riding as Therapy for Children with Cerebral Palsy - 0 views

  •
A systematic review of the literature on horseback riding therapy as an intervention for children with cerebral palsy (CP) was carried out. There is contradicting evidence on how effective hippotherapy is for children with disabilities such as cerebral palsy. The article found that there is evidence that hippotherapy is effective for treating muscle symmetry in the hips and trunk. Horseback riding is also effective for gross motor function when compared to regular therapy. I found this article interesting since my fieldwork educator complete hippotherapy out at a clinic and is always talking about the various benefits she sees.

In this article researches assessed the effectiveness of sensory-enhanced Hatha Yoga on symptoms of combat stress for deployed military personnel. In this study the researchers compared the anxiety and sensory processing abilities of the deployed military personnel with stateside civilians using the State-Trial Anxiety Inventory scales and the Adolescent/Adult Sensory Profile Quadrants. There was a total of 70 participants, 35 received 3 wks of treatment and 35 did not. The sensory-enhanced hatha yoga was shown to be effective in reducing state and trait anxiety. Researchers also found positive correlations with all test measures excepts sensory seeking, which was negatively correlated with all measures except low registration. These results support the use of sensory enhanced hatha yoga as a proactive approach for stress management for deployed military personnel.
