Typeform allows users to easily build, design and publish 'typeforms'. Really beautiful and easy tool to use for small questionnaires or forms. Very nice interface.

Zap survey is an online Tool where you can create all kind of different surveys. It is very easy to use and divided into three steps: Create an Online-survey, send them to students via Mailing list and inteprate the reusults fast and easyl via online-tool. You have to login bevore using it. It is rather a tool for trainers and teachers than for students. It can also be used as an tool to spread Evaluation surveys and exams.

I have used different survey tools, and I like Google Forms (part of Google Docs) because it has no restrictions and a very intuitive interface. My Diigo account has an overview of survey tools if you search for the tag "survey" (on my account)
