Zap survey is an online Tool where you can create all kind of different surveys. It is very easy to use and divided into three steps: Create an Online-survey, send them to students via Mailing list and inteprate the reusults fast and easyl via online-tool. You have to login bevore using it. It is rather a tool for trainers and teachers than for students. It can also be used as an tool to spread Evaluation surveys and exams. I have used different survey tools, and I like Google Forms (part of Google Docs) because it has no restrictions and a very intuitive interface. My Diigo account has an overview of survey tools if you search for the tag "survey" (on my account)
