The Gallatin Valley Resource Guide for Parents of Children with Disabilities is a reference source that has been created for parents and those who serve children with developmental delays and disabilities age birth to 18 in Gallatin County, specifically Bozeman and Belgrade.  The guide contains over 250 resources for parents of children with disabilities, 95 specific to Gallatin Valley and an additional 58 unique to Montana. This guide was created by me as a final project (May 2011) for a Master’s of Science degree in Health and Human Development at Montana State University.  The internet version of the guide will be maintained by the Bozeman chapter of the Special Education Parent Teacher Association (SEPTA). Portfolios are also used by youth to introduce themselves at their IEP meetings and to develop self-advocacy skills. During this webinar, you will learn how Portfolios can be an important piece of the health care transition for young adults. Through a Portfolio's concise text and photos, individuals with disabilities can describe the impact of their disability; list current medications and side effects they are experiencing; discuss seizures (what type, how often, etc.); introduce and explain medical equipment; share information about helpful accommodations and supports; and articulate what they want and need from their medical provider. Younger children can use Portfolios to learn to describe their own health care needs at an early age. Presenters will also share links to health care transition resources such as toolkits, web sites, and iPad apps that attendees may wish to explore. What: This webcast will introduce information for professionals on some of the emerging trends in biomedical research and health care environments that present new opportunities for partnership, collaboration, and support, as well as: Provide an overview of translational science, e-science and health information resources in electronic health records, including the related trends and issues affecting information professionals. Identify skills that information professionals need to work with researchers, information technology professionals, clinicians and community partners in these environments. Learn about community engagement in translational science research and roles for clinicians and information professionals in this arena. Address meaningful use of health information by patients accessing their electronic health records, as required by recent legislation to facilitate quality improvement at the point of care. This knowledge path directs readers to current, high-quality Spanish-language health resources for health professionals and families. The knowledge path has been developed by the MCH Library at Georgetown University and will be updated periodically.

This knowledge path about Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) services in Medicaid has been compiled by the Maternal and Child Health Library at Georgetown University. It offers a selection of current, high-quality resources about providing and strengthening EPSDT services. The path includes guidelines for the frequency, timing, and content of health-promotion and disease-prevention services for infants, children, and adolescents. Separate sections present resources for professionals (health professionals, program administrators, policymakers, and researchers) and for families. A special topics area lists resources about oral health services as part of the EPSDT benefit. The knowledge path will be updated periodically.

This knowledge path about caring for children and youth with special health care needs has been compiled by the Maternal and Child Health Library at Georgetown University. It offers a selection of current, high-quality resources that analyze data, describe effective programs, and report on policy and research aimed at developing systems of care that are family-centered, community-based, coordinated, and culturally competent. A separate section lists resources for families. The final part of the knowledge path presents resources that address specific aspects of care and development, such as advocacy, early intervention and special education, financing services, rehabilitation, screening, and transition. This knowledge path for health professionals, program administrators, policymakers, educators, researchers, and families will be updated periodically.

The toolkit offers best-practice tools and information regarding specific medical and behavioral concerns of adults with IDD, including resources for patients and families. The website is divided into four categories: General Issues; Physical Issues; Health Watch Tables; and Behavioral and Mental Health Issues.

What: This group is designed for parents & caregivers of children with mental health issues. The group meets once a month for the purpose of supporting one another and sharing resources as parents/caregivers of children with mental health issues. DPHHS has produced a community health program guide to make it easier to find health resources that are available across Montana.

What: Supplemental Security Income (SSI) and Social Security Disability Insurance (SSDI) are disability income benefits administered by the Social Security Administration (SSA) that also provide Medicaid and/or Medicare health insurance to individuals who are eligible. The application process for SSI/SSDI is complicated and difficult to navigate. Nationally, about 37 percent of individuals who apply for these benefits are approved on initial application and appeals take an average of two years to complete. For people who are homeless or who are returning to the community from institutions (jails, prisons or hospitals), access to these programs can be extremely challenging. Approval on initial application for people who are homeless and who have no one to assist them is about 10-15 percent. For those who have a mental illness, substance use issues, or co-occurring disorders that impair cognition, the application process is even more difficult – yet accessing these benefits is often a critical first step in recovery. SSI/SSDI Outreach, Access and Recovery (SOAR) is an approach that helps states increase access to mainstream benefits for people who are homeless or at risk of homelessness through: Strategic Planning Meeting(s) to establish collaboration among key state and/or local stakeholders and to develop an action plan for implementing the SOAR approach (e.g. SSA and Disability Determination Services (DDS); State Mental Health Agency and Department of Corrections leadership; and community homeless, health and behavioral health providers). Training of case managers using SAMHSA's Stepping Stones to Recovery curriculum that includes a step-by-step explanation of an improved SSI/SSDI application process. A Train-the-Trainer program allows for expansion and sustainability. Four years ago the Helena School District received a $5.3 million grant for the Safe Schools Healthy Student Initiative. Since the grant is in its last year, Youth Connections in Helena wants to get the word out to the community about all the programs the grant has allowed them to establish.

WASHINGTON, DC (October 10, 2013) -- Autism Speaks today issued autism-specific resources to help families understand the Affordable Care Act and the new changes that will affect health insurance coverage, beginning in 2014. These resources are a prelude to the “Autism Speaks Insurance Link,” a new interactive web tool Autism Speaks will unveil shortly to help families determine whether they are eligible for autism benefits under their current coverage.
