In response to concerns raised by pharmacy owners, the government has decided to adjust the August threshold for the Pharmacy First service from 20 to 15 clinical pathways. The ability of pharmacy owners to achieve the thresholds for the monthly Pharmacy First payments has been a topic of great concern. Community Pharmacy England (CPE) has been actively discussing this issue over the last few weeks with the Department of Health and Social Care and NHS England. Ministers have now decided to adjust the August threshold for the Pharmacy First service to 15, CPE announced today (July 31). Commenting on this decision, Janet Morrison, CEO of Community Pharmacy England said: "We are pleased that Ministers have listened to our concerns and acted quickly to make an adjustment so soon after coming into office."

In its closing oral statement to the UK Covid Inquiry Module 3 on Wednesday (27 November), the National Pharmacy Association (NPA) urged the government and NHS to "recast community pharmacy in their minds as a valuable partner in the post-pandemic recovery, and not as a cost center." The Inquiry was also asked to recommend sufficient government investment in the network and infrastructure needed to integrate community pharmacy into the broader health system and support effective cooperation across the health service. Additionally, the NPA requested that the significant contribution of community pharmacy to the pandemic response be reflected in the Module 3 findings to "redress the lack of recognition they received throughout the pandemic." The NPA's closing statement addressed four key issues - the role of community pharmacy, the impact on pharmacy teams, the resilience of the pharmacy network, and the lack of recognition given to the sector during the pandemic and since. Lee John-Charles, speaking for the NPA, told the Inquiry that "Community pharmacy became the first port of call for patients seeking health advice during the pandemic." "Many community pharmacies went to great and heroic lengths to ensure that their services were maintained during the pandemic and demonstrated the value of the network of community pharmacies across the country."

NHS England announced at the Pharmacy Show this morning that Ministers have decided to amend the Pharmacy First thresholds for the remainder of the year. This decision follows concerns raised by Community Pharmacy England (CPE) regarding difficulties faced by pharmacy owners in meeting the required number of clinical pathway consultations for monthly Pharmacy First payments In response to CPE's representations to the Department of Health and Social Care (DHSC) and NHS England, the threshold was earlier reduced from 20 to 15 in August, and from 30 to 20 in October. At the Pharmacy Show on Monday, Ali Sparke from NHS England announced the revised thresholds, setting them at 20 for November and December 2024. Starting in January and February 2025, the number of clinical pathway consultations needed for the monthly Pharmacy First payment will rise to 25, with a further increase to 30 in March.

Pharmacy minister Stephen Kinnock has pledged to enhance the role of community pharmacies, particularly in deprived areas, by making better use of the skills of pharmacy teams. In a video message at the SIGMA UK Community Pharmacy Conference 2024 on Sunday, Kinnock also announced plans to expand the Pharmacy First initiative and introduce an independent prescribing service, making prescribing an integral part of the service delivered by community pharmacies. He said: "Since taking on the role of minister of state for care in government, my message has been very clear: we must get our fantastic primary care sector back on its feet to be able to deliver for our patients. "This government is committed to shifting care from hospital to community, from analogue to digital, and from sickness to prevention, and pharmacies will play a vital role in that." Kinnock acknowledged the pressures faced by the sector and expressed concerns that so many community pharmacies have exited the market over the past decade.

The decline in the number of pharmacies, particularly within the Managed Chains channel, underscores the volatility of the market. The number of active pharmacy contractors in England has dropped significantly, with more closures than openings each month, exacerbated by the exit of Lloyds Pharmacy from the UK market which completed at the end of 2023. In the year ending June 2024, there was a net closure of 440 sites within NHSE which equates to a 4% decline in available pharmacies1. The increase in closures from June 2023 was driven by the closures of Lloyds in Sainsburys and the ongoing divestment of the Boots estate.

The new Labour government's first budget has not been well received by community pharmacies, who are 'deeply worried' about the increase in National Insurance and the national living wage. They have also voiced their dissatisfaction with the Autumn Budget 2024 for lacking measures to address pharmacy closures. Commenting on the Chancellor's Budget, Paul Rees, CEO of the National Pharmacy Association (NPA) said: "Millions of people who depend on local pharmacies will be holding their breath today, hoping that the £22.6bn increase in health spending announced by the Chancellor will include money to stem the devastating closure of local health services in the past decade. "There's absolutely no mention in the Budget of action to halt the closure of our vital NHS pharmacy network, which has been shrinking at the rate of seven a week as pharmacies are forced to close through underfunding." According to the NPA, around 700 pharmacies have shut since just the start of 2022 due to the impact of a 40 per cent cut to funding coupled with rising demand.

"If we stand still, we will die. We have to keep moving, and we have to keep changing" said Patrick Gompels, co-owner of Gompels Limited, while emphasising the need for innovation in community pharmacy services, during the recent Pharmacy Business Conference 2024. Centered on the theme 'Pharmacy of Tomorrow,' the conference, held at the Hilton Wembley on April 28, focused on adopting and adapting innovative technology as well as new ways of working and thinking to enhance patient services. Patrick was joined by Mayank Patel of Pearl Chemist Group and Michael Lennox, CEO of Community Pharmacy Somerset (LPC), during the panel discussion on innovation in community pharmacy services, moderated by Reena Barai, pharmacist and owner of S G Barai Pharmacy. On the changes he made at Gompels, Patrick said: "The first thing we did was a complete overhaul of all of our systems. Pretty much everything was stripped down." He shared that Gompels has embraced innovation by partnering with forward-thinking companies like Titan PMR, Drug Comparison, and Real World Analytics to enhance their processes and data analysis capabilities.

Pharmacies are valuable healthcare assets, meeting increasing public demand. However, a substantial portion of their capacity is consumed by unfunded work. According to Community Pharmacy England's (CPE) 2024 Pharmacy Advice Audit, community pharmacies conduct over 1.3 million unfunded consultations each week, equating to 69 million annually. The average pharmacy completes around 22 of these consultations daily, each taking approximately 5.6 minutes of staff time -adding up to over two hours daily. This represents nearly 50 per cent more consultations than in 2020 and a 78 per cent increase in time spent on unfunded healthcare advice since the 2020 audit. The audit, which surveyed over a third (3,916) of community pharmacies in England earlier this year, also highlights that pharmacies handle nearly 150,000 informal referrals from GPs and NHS 111 each week. All these referrals could have been routed through the Pharmacy First service, which would have potentially secured over £115 million in funding for the sector, the CPE noted.

With the UK General Election set for 4 July, pharmacy bodies are actively engaging with all political parties to secure support for the community pharmacy sector. They are pressing parties to address urgent issues such as pharmacy closures and medicine shortages. "The upcoming general election is a critical opportunity for us to strengthen supporters of community pharmacy in readiness for the new Parliament," said Janet Morrison, chief executive of Community Pharmacy England (CPE). She stressed the need for unity within the sector in advocating for community pharmacy and raising awareness of the issues with election candidates from all parties. As the general election approaches, CPE has called upon all political parties to support pharmacies by "introducing a long-term sustainable funding model and reviewing the medicines supply chain, including much-needed short-term relief measures."

Janet Morrison, CEO of Community Pharmacy England (CPE) has emphasised that stabilization of the sector is essential before any solutions can be delivered to fix the "struggling" health service. Speaking at the recent SIGMA UK Community Pharmacy Conference 2024, Janet described the current contract as "unaffordable" and warned that pharmacies are becoming "unsustainable." "Without an urgent funding uplift, the sector is facing a 'house of cards' collapse that will seriously impact communities, patients, and the safety of medicine supply. "So, first of all, we have to secure the stabilization. We have to make sure you can keep the lights on and the doors open, and that's critical," she stated. Janet pointed out that if the sector is stabilized and a fair deal is achieved, community pharmacies could effectively deliver solutions for the government that meet their policy goals. However, she noted that negotiations for the Community Pharmacy Contractual Framework (CPCF) for 2024-2025 cannot resume until the budget is published on October 30.

A worrying rise in pharmacy closures across England is disproportionately affecting older people and rural communities, according to a new report by Healthwatch England. Findings obtained through Freedom of Information (FoI) requests have exposed the scale of the issue, with over 436 permanent pharmacy closures recorded in 2023 and more than 13,800 temporary closures resulting in nearly 47,000 hours lost. The report, published on Thursday, paints a bleak picture of access to vital healthcare services, particularly in rural areas where pharmacy closures have hit hardest. The report found that Integrated Care Boards (ICBs) in rural areas recorded a higher level of temporary pharmacy closures compared to ICBs in predominantly urban areas. Additionally, ICBs with a higher proportion of people over 60 years of age recorded a higher number of hours lost per pharmacy. Healthwatch chief executive Louise Ansari expressed concern about the impact on elderly populations, noting that the closures were undermining the government's flagship Pharmacy First programme, designed to relieve pressure on GP practices by allowing patients to seek care for common conditions at local pharmacies.

The Community Pharmacy Walk-In Consultation Service (WICS), offering treatment for various minor ailments, recorded 19,567 consultations in 2024. This marks a 61 per cent increase compared to the 12,138 consultations recorded in 2023, according to data released by Community Pharmacy Cornwall (CPC) and obtained by The Pharmaceutical Journal. Launched in community pharmacies across Cornwall and the Isles of Scilly in January 2021, the minor ailment scheme is commissioned by the NHS and CPC. It enables locally accredited community pharmacists to supply prescription-only-medicines (POMs) for the treatment of several common conditions utilising patient group directions (PGDs) that Pharmacy First does not currently cover. These include vaginal candidiasis (vaginal thrush), dermatitis, migraine, conjunctivitis, and emergency hormonal contraception. Since its launch, the service has facilitated over 38,000 consultations, with 80 per cent of them resulting in advice or treatment that concluded the consultation in the pharmacy. WICS data also indicates that the service has helped avoid an estimated 30,000 GP appointments and more than 500 inappropriate A&E visits since its inception.

The annual UK-wide public awareness campaign, Ask Your Pharmacist Week, organised by the National Pharmacy Association (NPA), has begun today (4 November). The campaign theme, "Professional and Convenient Healthcare, from Your Local Pharmacy Team", aims to highlight the skills and knowledge of community pharmacists and the wider pharmacy team, and their track record of safely delivering clinical services. Furthermore, it emphasises that community pharmacists work closely with GPs and the rest of the local healthcare team to make sure patients get the best possible care. The central message of this year's campaign is: "Your local pharmacy team is highly skilled and well equipped to provide convenient clinical care, including treatment for common illnesses, as part of the wider local NHS team." Throughout the week, pharmacies across the country will promote their expertise and services, including NHS-supported treatments for common ailments.

It was an unprecedented move, to call for the first ballot in the 103-year history of the National Pharmacy Association (NPA), and now the organisation has warned that it's more than just a scare tactic and pharmacies will go through with action in January if there isn't an acceptable resolution forthcoming from the government. "We're looking for the government to consider our ballot, digest the results, and then come up with a proper proposal for community pharmacy - a fair deal," chief executive officer Paul Ree told Pharmacy Business. "If that's not forthcoming, we've got a board meeting in a couple of weeks, we then need to decide when to take action, but we would be looking at taking action in January." The NPA announced the results of the collective action ballot on 14 November in which nearly all pharmacy owners, who participated in the ballot, voted to limit their services, insisting that a lack of additional funding is jeopardising patient health and safety. "Our members have voted, they've spoken clearly, and unless the government come up with a fair deal soon, we will regrettably have to move to the next step, which is calling for action," said Rees.

The Company Chemists' Association (CCA) has urged NHS England to expand the Pharmacy First service by increasing treatment options, adding more conditions, and integrating Independent Prescribing into the service. This would further promote pharmacists as antimicrobial stewards while enhancing patient access to urgent care, it said. Managing patient demand for antibiotics remains a challenge for healthcare professionals, including pharmacists. However, a new report by CCA confirms that pharmacists are acting as responsible providers of antibiotics, in line with Antimicrobial Resistance (AMR) guidelines. The service allows pharmacies to provide advice and treatment, including prescription-only medication, for seven common conditions: earache, uncomplicated urinary tract infections (UTIs) in women, sore throat, sinusitis, infected insect bites, impetigo, and shingles. Patients can access the service via GP referrals or by visiting or contacting a pharmacy directly.

The Royal Pharmaceutical Society (RPS) has announced the results of the elections to vacant positions on the RPS Scottish and English Country Boards that took place at the meeting on June 18. The Scottish Pharmacy Board elected Jonathan Burton, FRPharmS, as their new Chair and Audrey Thompson to the RPS Assembly, the organisation's governing body. Jonathan is a community pharmacist and an independent prescriber affiliated with the Right Medicine Pharmacy group in Stirling. He has played an integral role in the inception, development and delivery of Pharmacy First services in Scotland. He is an RPS Fellow and received an MBE in 2018 for services to healthcare. Jonathan is also the recipient of the 2023 RPS Charter Award. He previously served as Chair of the Scottish Pharmacy Board from 2019 to 2021. "Serving as Chair is a great privilege," said Jonathan while expressing his delight at being elected as the Chair of the Scottish Pharmacy Board.

The Company Chemists' Association (CCA) has responded positively to the Labour Party's manifesto, which outlines plans to bolster community pharmacy services and shift the NHS towards a more localized, prevention-focused model. Key proposals include the creation of a Community Pharmacist Prescribing Service and the enhancement of care delivery in local communities. Malcolm Harrison, Chief Executive of the CCA, expressed strong support for these initiatives and said: "We welcome the Labour Party Manifesto's clear focus on moving to a 'Neighbourhood Health Service', prevention, and the return of the 'family doctor'." "Harnessing community pharmacy is crucial to delivering each of these," Harrison added.

Britons paying privately for obesity drugs are increasingly choosing Eli Lilly's Mounjaro over Novo Nordisk's Wegovy, online pharmacies say, in a sign the US drugmaker is gaining ground on its European rival. Mounjaro is appealing to people because of its greater efficacy, six online pharmacies and two patients told Reuters, showing Novo's first-mover advantage is being challenged in the UK even though some pharmacies sell starter doses for the weekly injection for up to 40 per cent more than Wegovy. "Mounjaro is now vastly outstripping Wegovy," said Chemist4U CEO James O'Loan, who said for the past three to four months, Mounjaro has won about 70 per cent of its sales. Chemist4U and another online seller, Simple Online Pharmacy, estimate that as many as 500,000 people in the UK currently take either Mounjaro or Wegovy via prescriptions from private online pharmacies. Mounjaro, unlike Wegovy, is not available through the National Health Service (NHS) though it likely will be next year. Wegovy is only available through the NHS at specialist obesity clinics and in limited circumstances. There is no public data on prescription numbers but the government said last year it had capacity to treat about 35,000 patients.

The event, organised in response to feedback from PDA locum members, addressed a range of significant topics including training for the recent Pharmacy First Services, respect for professional autonomy, and the need for adequate rest breaks. However, the issue of rate pressure and pharmacy closures was a focal point. Several locum pharmacists reported that some employers were leveraging the possibility of pharmacy closures to coerce them into working for lower pay. This unethical practice has raised alarm within the locum community, highlighting the precarious working conditions many face.

As the general election campaign heats up, major political parties - including Labour, the Conservatives, the Liberal Democrats - have released their manifestos, detailing their plans and visions for the future of community pharmacy. While the Conservative Party promises to expand Pharmacy First initiative, Labour pledges support for small businesses, with plans to overhaul the business rates system. The Liberal Democrats' election manifesto commits to developing a fairer and more sustainable long-term funding model for pharmacies. Announcing their plans recently, Prime Minister Rishi Sunak stated that they are investing in community care services to secure the future of the NHS and make healthcare more accessible. The Conservatives plan to fund these initiatives by cutting NHS managerial positions back to pre-pandemic levels and halving the government's management consultancy expenditure.
