Troops are coming home from Iraq despite the best efforts of the Obama administration to keep them there. "We're relieved to know all the troops will be home at the end of the year. This would not have been possible without the courageous actions of Wikileaks and the alleged participation of Bradley Manning" says Executive Director of Iraq Veterans Against the War, Jose Vasquez. Until their abrupt reversal a few weeks ago, the Obama administration had been working hard to maintain between 8000 and 20,000 troops in Iraq after the current Status of Forces agreement was set to expire at the end of this year. Fortunately, the Iraqi government refused to renew the judicial immunity, under which the U.S. Armed Forces had been operating. Establishment news outlets have largely ignored the reason for this sudden change of plans. A few weeks before the negotiations between Washington and Baghdad reached an impasse, WikiLeaks released a cable providing clear evidence that U.S. forces had been engaged in a cover up of the heinous execution of civilian non-combatants following a raid of a suspected insurgent stronghold. "A U.S. diplomatic cable made public by WikiLeaks provides evidence that U.S. troops executed at least 10 Iraqi civilians, including a woman in her 70s and a 5-month-old infant, then called in an airstrike to destroy the evidence, during a controversial 2006 incident in the central Iraqi town of Ishaqi." Seattle photographer Joshua Trujillo captured what may become the defining image of this week of Occupy unrest - an elderly woman being led away from the mayhem, her face covered with pepper spray. A pregnant woman and a priest were also hit with pepper spray during a march on Tuesday night. You see more photos of the confrontation at SeattlePI.com. (More photos here as well.) The Seattle branch of the Occupy movement, which has been camped out near Seattle Central Community College, held the march in support of the New York camp, which faced a day long eviction battle with the city yesterday. On Monday, Occupy Oakland was the scene of another attempt by police to drive campers out of a city park. There were reports that both Occupy San Francisco and Occupy Cal (on the Berkeley campus of the University of California) are being raided on Wednesday morning. The week of police crackdown comes amid reports that the federal government and is coordinating with multiple on legal strategies that can shut down the Occupy protests. The woman in the picture is not just any elderly woman, however, as she is well known to Seattle residents. Dorli Rainey is a former school teacher who has been active in local politics since the 1960s. In 2009, she ran for mayor, but eventually dropped out by saying, "I am old and should learn to be old, stay home, watch TV and sit still." We guess she didn't learn. Rainey emailed The Stranger, Seattle's alternative paper, to say she stopped by the march to see what was happening when her group got pinned in by police and nearly trampled in the chaos.

The IOM's 1999 landmark report, "To Err is Human," dropped the first bombshell, reporting that between 44,000 and 98,000 Americans die in hospitals each year from medical mistakes, costing an estimated $17 billion to $29 billion annually. HHS' new finding that medical mistakes kill 15,000 Medicare patients a month equates to 180,000 Medicare deaths per year - more than the IOM's estimate, which attempted to cover all patients in the United States. That means that the annual death toll in this country caused by mistakes in hospitals is well over 250,000 deaths a year! But perhaps the most startling finding by HHS is that a significant number of patients suffered injuries or died needlessly, as 44 percent of the medical errors were preventable.

Back in March, Sen. John McCain (R-AZ) astoundingly claimed that the iPad and iPhone are "built in the United States of America." This news must have been a great surprise to the Chinese workers who work for Taiwanese-based manufacturing giant Foxconn, which is notorious for the poor conditions at its factories and the wave of suicides at its plants. After much of the international media covered the abuses at Foxconn's factories, the company, along with the major American corporations it supplies - like Apple and HP - announced that it would be reforming its practices. Yet a new report from Students & Scholars Against Corporate Misbehaviour (SACOM), a Hong Kong-based advocacy and research group, finds that many of the practices that led more than a dozen workers committ suicide continue to live on. SACOM conducted a comprehensive study of practices at several Foxconn factories over the months of March and April and found that a number of shocking policies are in place.
