The Dispensing Doctors' Association (DDA) is advising patients left deserted due to pharmacy closures in rural areas to consult their general practice about accessing the GP dispensing service. This recommendation comes in response to concerns that England's rural areas are increasingly becoming pharmacy 'deserts', posing challenges for residents to access essential medications and healthcare services. DDA chairman Dr Richard West said: "The GP dispensing service is designed to ensure NHS medicine supply in areas where people have little or no access to a pharmacy service. "People in rural areas who now live more than a mile from a pharmacy should speak to their GP about whether they can receive the NHS GP dispensing service."

Paydens Pharmacy Group, one of the first multiple pharmacies to adopt hub and spoke model last year, revealed that the move has resulted in significant time savings in store. Centralising dispensing of repeat prescriptions relieves pressure in pharmacy branches and releases time to deliver revenue generating services, the group said. The group created a hub in Maidstone, Kent, to service its highest dispensing branches. The hub uses Centred Solution's FLOWRx Hub Auto to dispense their original pack repeat prescriptions. The group put in the first FLOWRx Hub production line in spring 2022 and then added in a second line in the autumn as they increased production and moved more stores to the hub and spoke model. The innovative solution interfaces with EMIS PMR, Omnicell's Robotic Dispensing System and Universal Logistics Management software as well as Victoria OS ordering software. The hub operation runs during the day Monday to Friday and produces an average of 33,289 packs per week for just under 9,000 patients, leaving plenty of scope to ramp up. The hub is currently dispensing 79.7% of repeat prescription original packs requested by the group's busiest stores.

Recognising the changing practices in healthcare, the Royal Pharmaceutical Society (RPS) has revised and updated its position on the separation of prescribing and dispensing or administration of a medicine by the same healthcare professional. It's previous guidance, published in March 2020, restricted prescribing and dispensing by the same healthcare professional only in exceptional circumstances. The new guidance, jointly developed with the Royal College of Nursing (RCN), has made prescribing and dispensing of medicines more flexible. It clarifies that "where there is a risk assessment in place and in the best interests of the patient, the same healthcare professional can be responsible for the prescribing and dispensing / supply / administration of medicines." The change in the position statement follows an extensive year-long engagement exercise with medical associations, members, fellows, expert advisory groups, patient groups and the RPS Country Boards.

Centred Solutions has been at the forefront of the pharmacy hub and spoke dispensing in England over the last few years. We have already demonstrated that hub and spoke dispensing is a realistic model of dispensing for pharmacy groups of all sizes. We've seen the massive difference that hub and spoke can make to community pharmacy. With pharmacies struggling to survive, the time has come to stop delaying and to now push ahead with model one of hub and spoke dispensing. In our experience, there is no risk with moving ahead with model one of hub and spoke legislation which would allow medicines to be returned from the hub to the pharmacy ready to be dispensed to the patient. We do however understand the need for more policy discussion around the second model of hub and spoke, where the hub sends the medication directly to the patient. This should not delay the implementation of model one. This model would immediately level the playing field for smaller and independent pharmacies, allowing them to use a hub and spoke model of their choice now to create capacity for clinical services. We feel strongly about this issue and that's why we are working alongside HubRx and PillTime to raise awareness.

Talk of hub and spoke models has been rumbling on for as long as I can remember. The topic has once again hit the headlines as the government announced a fresh consultation for its proposed changes, calling on all interested parties to submit their views by 8th June. The argument for hub and spoke is to allow independents to have the same level playing field as the larger groups who have been operating this model for some time. The government also sees significant efficiency gains in centralisation, arguing this will free pharmacists time away from dispensing to focus on clinical services. The latest consultation advocates two models being created. In the first model, the dispensed medication is sent back to the spoke. In the second model, the completed medication can be sent out directly to the patient. The latter is a new concept which could open up new opportunities in how services are delivered. Pharmacies could effectively outsource the entire dispensing and delivery to housebound patients and care homes allowing pharmacies to take on new business without having to worry about logistics and geography.

Prevalence and consequences of medication errors Medication errors are incidents involved with the administering, prescribing, dispensing or monitoring of medicine to patients. It can happen at many different steps in the healthcare process and by any medical professional in the system. Many cases are avoidable. According to analysis from BMJ, there are an estimated 237 million medication errors made in England every year. The majority of these are minor errors, but 1 in 4 cases has the potential to cause moderate to serious harm to patients. Not only do these errors cost the NHS significantly, at almost £100 million every year, but there is a shocking cost to public health. Lives are being lost because of medication errors which is unforgivable and tragic for the families involved. Drains on NHS resources have widespread impacts on public health and the operations of healthcare organisations up and down the country. Individual errors and mistakes may seem inconsequential (in minor cases), but they all add up in the big picture to a significant concern for policymakers.

Lo's Pharmacy, a Yorkshire-based pharmacy chain is adopting a new approach to hub and spoke in a first of a kind model to be used in the UK. The pharmacy will centralise its repeat prescription dispensing service for original packs with the support of its medication wholesaler. The independent group, which has 25 NHS community pharmacies across Yorkshire, will install Centred Solution's Automated FLOWRx Hub product at its central dispensing facility in Wath-Upon-Dearne, South Yorkshire. But unlike other FLOWRx customers, who pick packs from a robotic dispensing system or internal warehouse, Lo's Pharmacy will receive patient-specific flow totes directly from their wholesaler which can then be processed by their hub. This workflow has not been used anywhere else in the country until now and it is a revolutionary approach to hub and spoke dispensing, showing what can be achieved when key players decide to collaborate. It further demonstrates that there are range of variations of hub and spoke models available and accessible to independent and multiple community pharmacies across the UK.

Our Totton branch has an incredibly high volume of original pack dispensing, roughly 45,000 items. As we became busier and busier in the dispensary we were finding that our pharmacists, dispensers and other technicians simply didn't have capacity to deliver the services that we needed them to. The Department of Health and Social Care (DHSC) has finally published its response to the 2022 consultation on hub and spoke dispensing. Considering the consultation evidence and further discussions, the government has expressed its intention to progress the proposals for enabling hub and spoke models across different legal entities as soon as possible. This will be achieved by using the enabling powers outlined in Part 2 of the Medicines and Medical Devices Act 2021 (MMDA) to amend the Medicines Act 1968 and the HMRs. Furthermore, the DHSC has decided to proceed with the implementation of the two models of hub and spoke dispensing that it consulted on. The government response to the consultation reads: "Having considered the responses, the government intend to proceed to implement the necessary changes to medicines legislation to remove the current restrictions that prevent the hub and spoke dispensing models from operating across different legal entities found in section 10 of the Medicines Act 1968.

It's no secret that as pharmacists we are navigating a new and more demanding healthcare landscape. Our role has changed significantly since the start of the 21st century due to increased expectations from the public, advances in science and technology and changing workforce pressures. The Community Pharmacy Contractual Framework for England has placed a higher focus on clinical service. In addition, the Covid-19 pandemic resulted in significant acceleration in changing how we work day-to-day and the public perception of our role. Community pharmacy owners and staff have been under relentless pressure for the last two years. Dispensing volumes are continually increasing but dispensing revenues are being replaced by launching new services. We must find solutions to reduce the time it takes to dispense medication so we can focus on these new revenue generating services. To not just succeed, but to thrive, it is becoming increasingly important to understand and embrace the technology available.

Many community pharmacies are under severe financial strain due to chronic underfunding and a mismatch between medicine costs and reimbursement rates. A report by the National Pharmacy Association (NPA) revealed that NHS underpays pharmacies by up to £75 a pack for common medicines, resulting in losses of thousands of pounds each month. The NPA analysis found that, in many instances, NHS funding covers only one-fifth of their purchase costs for medicines. Nemesh Patel, Managing Director of the Southdowns Pharmacy Group, said the current pharmacy funding contract is "completely broken and beyond farcical." Sharing their struggles, he tells Pharmacy Business: "Our teams are wasting hours and hours trying to just source medication for patients when they could be better using that time to clinically support our patients, and then when it comes to reimbursement, the drug tariff prices or concessionary prices are structured such that on hundreds of medications we dispense per month, we will be making a significant financial loss, each and every single time our pharmacies dispense that medication."

The National Pharmacy Association (NPA), representing independent community pharmacies, has called for tougher regulation of the online sale of weight loss jabs to protect patients amid a prediction of a new year's boom in demand. The association argues that current regulation "leaves the door open for medicines to be supplied without appropriate patient consultation and access to patient records." It has urged the General Pharmaceutical Council (GPhC) to require greater consultation with patients before dispensing weight loss jabs and other high-risk medication online. The association stressed that online sellers should not just rely on information provided in online questionnaires but also gather important historical medical information before a full two-way consultation between prescriber and patient. Draft guidelines on weight loss medication The GPhC proposed changes to their current guidance for online pharmacies to improve patient safety. It launched a consultation on the revised guidance on 17 September, inviting feedback until 9 October 2024 from the public, carers, pharmacists, pharmacy technicians, other healthcare professionals, and pharmacy owners. In its response to proposed guidelines on prescribing weight loss medication, the NPA highlighted critical gaps, stating that proposed safeguards "still leaves the door open for medicines to be prescribed/supplied without appropriate two-way direct patient consultation and access to patient records for a full clinical picture particularly where high-risk medicines are involved, and the risk to patient safety remains".

Omnicell Limited, a wholly owned subsidiary of Omnicell - an American healthcare technology company, has signed an agreement with the shareholders of Hub and Spoke Innovations Limited to acquire Hub and Spoke Innovations. Hub and Spoke Innovations Limited is distributor of the Pharmaself24 prescription collection kiosk in Great Britain. The move will complement Omnicell's total solution technology portfolio for retail pharmacy in the UK, including a pack-pick dispensing robot, automated MDS filling machine and eMAR solution, thereby helping pharmacies to improve workflows and offer a 24/7 access to medications and patient care. It will also help community pharmacies achieve their goals, as they increasingly look at ways to make their businesses more efficient and free up staff to offer more pharmaceutical care and services to patients. In short, the deal would enable Omnicell to offer a wider range of digital technologies to streamline retail pharmacy operations, allowing pharmacists to spend more time with patients and focus on value-added, revenue-generating healthcare services, such as vaccinations.

Parliamentarians continue to highlight the serious challenges faced by community pharmacists, calling for increased support for the sector. On Wednesday, January 8, during Prime Minister's Questions, Marie Goldman, Liberal Democrat MP for Chelmsford, drew attention to the issue of pharmacies dispensing medications at a loss. Goldman cited the case of a pharmacy owner in her constituency who is 'sometimes forced to pay over 100 times more' for a particular mental health drug than the contract reimburses. Dipak, the pharmacy owner, has been serving his community since opening his pharmacy in 1991, often greeting patients by name. However, he is now struggling to keep his business afloat as his NHS contract no longer covers the cost of the drugs he dispenses. "Dipak is dipping into his life savings to keep the pharmacy afloat. I am sure many other pharmacies are doing the same," Goldman stated. She asked the prime minister whether he agreed that "no pharmacist should be forced to use their own money to keep their pharmacy viable."

Accord Healthcare UK Ltd has announced that Disulfiram, an alcohol deterrent compound, will be made available to all UK pharmacists and dispensing doctors for prescription to suitable patients. Following its launch to wholesalers in December 2024, the medication will now be accessible for the treatment of alcohol dependence under appropriate medical supervision. Disulfiram is indicated as an adjuvant in the treatment of carefully selected and co-operative patients with drinking problems. The medication works by inhibiting the enzyme responsible for breaking down acetaldehyde-a substance produced when alcohol is metabolised in the body. This results in a buildup of acetaldehyde in the blood, triggering unpleasant reactions such as headaches, heart palpitations, and nausea. The experience of such reactions acts as a deterrent to further alcohol intake.

The NHS Business Services Authority (NHSBSA) has published its latest quarterly report on medicines used in mental health in England, specifically those used to treat anxiety, depression, psychosis, attention deficit hyperactivity disorder (ADHD), and dementia. Data from April to June 2024 show that 800,000 central nervous system (CNS) stimulants and ADHD drugs were prescribed to 230,000 identified patients. Both prescribing and dispensing of ADHD drugs continue to rise, with a 4.8 per cent increase in prescribed items and a 4.7 per cent rise in identified patients compared to the previous quarter. CNS stimulants and ADHD medications saw the largest increase in costs among the five drug categories, rising by 6.4 per cent to £36 million. The report reveals a modest increase in the prescribing of antidepressants, with 23 million antidepressant items prescribed to an estimated 6.9 million identified patients between April and June 2024. This represents a small increase of 1.5 per cent for prescribed items and a less than one per cent rise in identified patients compared to the previous quarter.

The Department of Health and Social Care (DHSC) and the Medicines and Healthcare Products Regulatory Agency (MHRA) have authorised community pharmacies throughout the UK to dispense original full packs of Valproate-containing medicines upon prescription. The necessary changes in regulations came into effect on 11 October which was strongly supported by 85 per cent of respondents concerned about risks to the unborn baby if valproate-containing medicines are used in pregnancy. The decision was made to ensure patients receive safety warnings and pictograms, including a patient card and the Patient Information Leaflet (PIL), contained in the manufacturer's original full pack. Valproate is a treatment for epilepsy and bipolar disorder but is also associated with birth defects and neurological disabilities.

Christie & Co, a specialist business property adviser, recently finalised the sale of two pharmacies-one located in Wales and the other in Hampshire. Sylvia Williams Chemist, a well-performing community pharmacy located on the main high street of the Vale of Glamorgan market town of Cowbridge, has been sold after an impressive 50 years of ownership. Dispensing an average of 12,500 items monthly, the business has been owned by Kate Thomas and her family for nearly 50 years. It was put on the market to enable her retirement and allow her to focus on other interests, the broker said. The pharmacy has been purchased by Zaid Salih, making it his second pharmacy acquisition. Jonathan Board, Director - Medical at Christie & Co, who handled the sale, said that Sylvia Williams Chemist attracted a wide range of buyers.
