The General Pharmaceutical Council (GPhC) has unveiled its Carbon Net Zero Action Plan for sustainable pharmacy regulation, aiming to achieve carbon net zero by 2040. The action plan focuses on three main elements: setting an example, incorporating sustainability into regulatory standards, and fostering collaboration for sustainable pharmacy care. Roz Gittins, Chief Pharmacy Officer at the GPhC, emphasised the urgent need for pharmacy sector to embrace sustainable healthcare practices. "Climate change is a global emergency and has been declared the biggest threat to public health," she said, adding that it not only poses health risks but also amplifies existing health inequalities, with immediate consequences for patients, the public and health services. She highlighted that the GPhC is actively engaged in addressing climate change, both as an employer and regulator. "Our aim is to positively influence the pharmacy sector to embrace climate action and sustainable healthcare principles in line with our vision to have safe and effective pharmacy care at the heart of healthier communities," Gittins said.

NHS England has commissioned the Royal Pharmaceutical Society (RPS) to develop guidance that helps community and hospital pharmacy teams across Britain to reduce the impact of pharmacy services, pharmaceutical care and medicines on the environment. The RPS said the Greener Pharmacy Guidance will enable pharmacies to self-assess their impact against the standards, benchmark and improve through evidence-based activities and actions. "I'm delighted our strong commitment to helping pharmacy reduce its environmental impact can now be taken to the next level through developing guidance and accreditation for pharmacy teams," RPS president Professor Claire Anderson said. "Medicines account for 25 per cent of carbon emissions within the NHS and this initiative underscores our commitment to promoting sustainable healthcare and supporting the NHS's goal of achieving 'net zero' emissions by 2040." Peter Morgan, medicines assistant director at NHS England, commented: "Pharmacy staff are involved in the purchasing and dispensing of almost every medicine used in the NHS and the new Greener Pharmacy Guidance and Self-accreditation scheme will provide support for pharmacy professionals by outlining clear actions to deliver more environmentally sustainable pharmacy practices." The RPS said the guidance and digital self-assessment toolkit will integrate with carbon calculator tools to help pharmacy teams to measure their carbon footprint, action plan to reduce use of carbon and improve sustainability.

Thanks to increasing environmental awareness among people worldwide, there's a growing commitment to sustainable practices across industries. The healthcare sector, including pharmacies, is no exception. Recognising the fact that 'climate crisis is a health crisis', 124 countries signed a new Declaration on Climate and Health at the first-ever Health Day at the United Nations climate change conference, COP28 Dubai, which was held on 3 December 2023. The declaration acknowledged the need to prepare healthcare systems to cope with climate change and accelerate actions to protect communities. Welcoming the declaration, Dr Tedros Adhanom Ghebreyesus, Director-General, World Health Organization (WHO) emphasized the need to build "climate-resilient and low-carbon" health systems to protect the health of both the planet and people. Pharmacies play a crucial role in providing essential medications and healthcare services to the community, but their operations can have environmental implications.

The NHS is facing a range of long term challenges which needs to be countered with prevention, enhance personalised care and sustainable performance, health and social care secretary Sajid Javid said on Tuesday (March 8). In his speech on healthcare reform at the Royal College of Physicians in London, he said: "We face some long-term challenges: how to keep the NHS focused on delivery while futureproofing it for changing demographics and disease; how to meet rising patient expectations and address the injustices of widespread disparities; and how to deal with an unsustainable financial trajectory while backing the brilliant people who work in health and care." Javid emphasised on three key points for healthcare reform - prevention, personalisation and performance. He said, prevention is not just about building a 'national hospital service' but a true 'National Health Service'.

Released on Monday morning, the election manifesto shared by the Liberal Democrats pledges to "strengthen patients' rights" by tackling public health inequalities and providing early access to community services. The Liberal Democrats Party's manifesto promises to address the issues at both the "front and back doors" of the National Health Service (NHS), which the party claims has been plunged into crisis by the Conservatives. Pledging to invest in public health and early access to community services, the party aims to "work towards a fairer and more sustainable long-term funding model for pharmacies and build on the Pharmacy First approach to give patients more accessible routine services and ease the pressure on GPs." The pledge also includes making prescriptions for people with chronic mental health conditions free on the NHS, as part of a wider commitment to review the entire schedule of exemptions for prescription charges.

With the upcoming general election on the horizon, Proprietary Association of Greater Britain (PAGB) is leading the charge to integrate self-care into the core of all political party manifestos. The aim is to ease strains on the NHS and forge a sustainable healthcare system for the future. In a significant move, PAGB unveiled a comprehensive general election toolkit on Wednesday, laying out a compelling argument for the prioritisation of self-care among stakeholders. The toolkit underscores the importance of public interest, potential NHS cost savings, and the economic advantages of the over-the-counter (OTC) sector. Aimed at unlocking the full potential of self-care and nurturing an enabling environment for its implementation, PAGB's toolkit advocates for greater integration of pharmacy services into primary care.

In a recent interview with The Telegraph, health secretary Steve Barclay stated that he has asked his officials within DHSC to look at a "pharmacy first" approach to alleviate pressures on A&E departments in order to avoid the widely predicted NHS winter crisis. On the face of it, this a welcome if long overdue recognition that community pharmacy is an essential part of our national healthcare infrastructure alongside our GP and A&E colleagues. But let's not get carried away - we have had lots of praise from politicians in the past which have not then been backed by firm commitments for a sustainable future for the network. Could this be a turning point? I hope so, but I am not confident it will be. I fear this may turn out to be another emergency stop-gap measure which does nothing to secure the long-term viability of the sector in England. The role of community pharmacy during the recent Covid pandemic demonstrated clearly how important we are to ensure people have easy access to essential healthcare support, advice and services. The NHS winter crisis can only be avoided or at least mitigated if the potential of the community pharmacy network to provide more patient care services is unlocked and that Barclay requires you to end pharmacy funding austerity and start investing. The Treasury will no doubt say there is no more money, but what then the alternative other than a NHS winter crisis? And, of course, treating people in secondary care settings is far more costly than community pharmacy based interventions.

A new survey has highlighted growing concerns about pharmacists' increasing workload and their often-overlooked role within the healthcare systems. In the survey commissioned by consumer health company Kenvue, over 90 per cent of pharmacists reported a workload increase over the past year, with only 37 per cent considering their current workload sustainable. Their primary concerns include the impact of workload on wellbeing, expanding clinical roles and services, and challenges with staff recruitment and retention. Pharmacists reported spending most of their time advising patients on common ailments such as coughs, colds, pain, and allergies. However, they spend significantly less time on preventive services, which could further reduce pressure on the wider healthcare system. For instance, only seven pharmacists focus primarily on smoking cessation, and just four per cent on oral health. The survey also highlighted pharmacists' sense of being undervalued, with only 22 per cent feeling valued by policymakers, despite their significant contributions to primary care provision.

The Independent Pharmacies Association (IPA), in collaboration with the Association of Optometrists (AOP), the British Dental Association (BDA) and the NHS Confederation has written to health secretary Wes Streeting, urging him to ensure sustainable investment in primary care to strengthen the NHS and better serve patients. In their letter, the group congratulated Streeting on his recent appointment as Secretary of State for Health and Social Care and requested a meeting to provide their joint constructive input towards improving health outcomes. They expressed concern that recent administrations have failed to follow through on promises to properly integrate and fund primary care, leading to deteriorating health outcomes. "Our collective experience is that our National Health Service (NHS), and health outcomes generally, have suffered at the hands of recent administrations that have talked much about the importance of properly integrating and funding primary care but not followed through with action," they wrote. The associations emphasised the need to transition the NHS to a Neighbourhood Health Service, where more care is delivered within local communities to identify problems earlier.

The English health secretary has fumbled the opportunity to prevent a crisis in the NHS this winter. She either does not understand or value the role of community pharmacy as the third pillar of patient access to essential healthcare. Her announcement that she wants community pharmacy to provide more services to take the strain off A&E departments and GP surgeries comes on the same day DHSC announces no new long-term investment to sustain the sector. Does she not understand that as a result of years of government underinvestment in England the network is in decline with random closures across the country? Too many pharmacies are temporarily closed every day due to workforce shortages beyond the control of pharmacy owners. Adding a new service here and there, even with some additional funding, does not address the longer term viability of the network which needs to know which patient services it will be expected to provide over the next 10 years - not just the next few months - and how those will be adequately remunerated. Asking more from our sector with no new investment is a strategy which is bound to fail. The pharmacy contract remains economically illiterate. The sector's finances need open heart surgery not a couple of paracetamol tablets.

Parliamentarians continue to highlight the serious challenges faced by community pharmacists, calling for increased support for the sector. On Wednesday, January 8, during Prime Minister's Questions, Marie Goldman, Liberal Democrat MP for Chelmsford, drew attention to the issue of pharmacies dispensing medications at a loss. Goldman cited the case of a pharmacy owner in her constituency who is 'sometimes forced to pay over 100 times more' for a particular mental health drug than the contract reimburses. Dipak, the pharmacy owner, has been serving his community since opening his pharmacy in 1991, often greeting patients by name. However, he is now struggling to keep his business afloat as his NHS contract no longer covers the cost of the drugs he dispenses. "Dipak is dipping into his life savings to keep the pharmacy afloat. I am sure many other pharmacies are doing the same," Goldman stated. She asked the prime minister whether he agreed that "no pharmacist should be forced to use their own money to keep their pharmacy viable."

A group of 20 MPs from the different political parties, including Dame Priti Patel and Judith Cummins, have written a joint letter to health minister Andrea Leadsom, calling for urgent action to address the pressures faced by community pharmacies. Taking note of rising pharmacy closures, they have highlighted the need for more investment and support from the government. Recent NHS figures showed that around 1,400 pharmacies have closed permanently since October 2016, and many more are withdrawing services or reducing their opening hours, particularly in deprived areas. The MPs have urged the pharmacy minister to commit to making essential changes including a fair and sustainable core funding model, an effective implementation of the Pharmacy First service and an expansion into other clinical service areas.

Community Pharmacy England (CPE) is gearing up to conduct a vital Pharmacy Advice Audit, calling on pharmacy owners and their teams to help measure the increasing reliance on pharmacies within communities. With years of financial strains impacting the sector, despite the much-needed £645 million recovery plan funding, the sector finds itself on the edge. The launch of Pharmacy First sparked a notable surge in people seeking advice from community pharmacies, both within and beyond the funded service, as revealed by recent sector polling. To accurately assess this trend and advocate for sustainable funding, CPE is initiating an audit to gather data crucial for showcasing the extent of unfunded care provided by community pharmacies. Scheduled to commence this week, the Pharmacy Advice Audit will entail pharmacy teams documenting informal healthcare advice dispensed over a single day.

The International Pharmaceutical Federation (FIP) has issued a new policy statement which encourages a "life-course" approach to vaccination by pharmacists. In the statement, published on September 27, FIP backed expansion of vaccine schedules and strategies so that patients of all age groups could be vaccinated throughout their entire lifespan, from infancy to old age. A life-course approach in public health and healthcare focuses on an individual's health and well-being, including vaccination, administered throughout their entire lifespan, from infancy to old age. FIP urged governments and policymakers to eliminate regulatory barriers, enabling pharmacists to prescribe and administer all relevant vaccines throughout the life-course. Additionally, FIP recommended that policymakers develop remuneration models for pharmacies to deliver sustainable life-course vaccination services within the private and public sectors.

Nick Kaye, chair of the National Pharmacy Association (NPA), gave evidence to the UK's official COVID-19 Inquiry on Monday (4 November), highlighting the importance of building a "sustainable and resilient community pharmacy network" to enhance preparedness for future pandemics. Kaye also recommended utilising the existing community pharmacy infrastructure for public health initiatives, such as vaccination programmes, and stressed the need to recognise community pharmacists and their teams as integral members of primary care. "Seven pharmacies a week are closing across the four nations, and that is tragic for any future response. So, a resilient community pharmacy network that exists is going to be key," he told the Inquiry. He noted that 90 per cent of most NPA members' income comes from providing NHS services, underscoring the critical role pharmacies play in healthcare delivery. However, Kaye highlighted that pharmacies were often overlooked and underappreciated during the pandemic.

As the general election campaign heats up, major political parties - including Labour, the Conservatives, the Liberal Democrats - have released their manifestos, detailing their plans and visions for the future of community pharmacy. While the Conservative Party promises to expand Pharmacy First initiative, Labour pledges support for small businesses, with plans to overhaul the business rates system. The Liberal Democrats' election manifesto commits to developing a fairer and more sustainable long-term funding model for pharmacies. Announcing their plans recently, Prime Minister Rishi Sunak stated that they are investing in community care services to secure the future of the NHS and make healthcare more accessible. The Conservatives plan to fund these initiatives by cutting NHS managerial positions back to pre-pandemic levels and halving the government's management consultancy expenditure.
