The Pharmaceutical Services Negotiating Committee (PSNC) has issued a revised guidance for the dispensing of controlled drugs in instalments on 19 September that has been declared as a bank holiday for Her Majesty The Queen's state funeral. PSNC said: "Many instalment prescriptions have already been issued in advance of the upcoming Bank Holiday. Where they contain the Home Office approved wording "Please dispense instalments due on pharmacy closed days on a prior suitable day" then you will need to plan to make these supplies at an appropriate time. This wording would also permit the supply of the initial dose of a prescription starting on the Bank Holiday Monday where the date of the signature is before this." However, PSNC is aware that not all prescriptions will contain such a direction to support the supply, in advance, of a dose for Monday 19 September where the supplying pharmacy is closed. "We recognise that this client group are often vulnerable with multi-morbidities and that the risk to them may be significant. You should therefore exercise your professional discretion in determining when to make an instalment supply for the Bank Holiday Monday. Covid-19 booster jab can now be administered sooner than six months after the second dose to certain vulnerable people as guideline in the UK Health Security Agency (UKHSA) Green Book has been updated. With the change in the guidance, care home residents who have been given their second vaccine dose at different times will be able to be jabbed in the same session, as long as it has been at least five months since their last dose. It may also help with other vulnerable groups, such as housebound patients to have their flu and Covid-19 vaccines at the same time. The move will also benefit those who are about to receive immunosuppressive treatment as they can get the booster from a minimum of four months after their second dose. Health secretary Sajid Javid said: "This updated guidance will ensure healthcare professionals have the necessary flexibility in the booster programme, allowing more vulnerable people to be vaccinated where it makes operational sense to do so." Vaccines minister Maggie Throup also encouraged eligible people to book their booster jab and secure protection.

Usually as we head into winter, we can all predict what our patients will be asking us about. But, just like the rest of the year has been like no other in community pharmacy, I'd expect this winter to be different too. This will present us with challenges, but also some great opportunities too. One shift we have seen over the last seven months of Covid is that more and more people in the community are turning to their pharmacy for help, support and advice. With our doors remaining open throughout the height of the pandemic, we've established ourselves firmly at the heart of local healthcare - a trend I'd expect to see continue over the winter, especially now that a second wave, it seems, is upon us. I think there's three different types of patients who we'll be seeing this winter coming to us for help and advice. First, we'll see those with a simple winter illness - for example, a common cold. For these patients our advice will be focused around self-care and home treatments. There is a wide range of products that we can recommend in the pharmacy from decongestants to combination products such as Night Nurse and nasal sprays.

Talk of hub and spoke models has been rumbling on for as long as I can remember. The topic has once again hit the headlines as the government announced a fresh consultation for its proposed changes, calling on all interested parties to submit their views by 8th June. The argument for hub and spoke is to allow independents to have the same level playing field as the larger groups who have been operating this model for some time. The government also sees significant efficiency gains in centralisation, arguing this will free pharmacists time away from dispensing to focus on clinical services. The latest consultation advocates two models being created. In the first model, the dispensed medication is sent back to the spoke. In the second model, the completed medication can be sent out directly to the patient. The latter is a new concept which could open up new opportunities in how services are delivered. Pharmacies could effectively outsource the entire dispensing and delivery to housebound patients and care homes allowing pharmacies to take on new business without having to worry about logistics and geography.

The Department of Health and Social Care aims to amend the Human Medicines Regulations 2012, thereby enabling pharmacy technicians to provide and administer specific medicines through a patient group direction. In line with this initiative, the Department introduced a six-week consultation to assess whether registered pharmacy technicians should be authorised to dispense medicines using a PGD. The proposals, backed by all four Home Nations, apply exclusively to registered pharmacy technicians. Meanwhile, this coverage will extend to include pharmacy technicians in Northern Ireland upon their achievement of registered professional status. "The proposed statutory instrument, presently applicable in England, Scotland, and Wales would offer 'clear benefits,' especially in areas such as vaccination and the provision of oral contraception and EHC," the DHSC said. According to the Department, facilitating registered pharmacy technicians to execute PGDs might amplify their workload, yet it could simultaneously enhance overall efficiency and cost-effectiveness for pharmacy teams by optimising the utilisation of skill diversity.
