The International Pharmaceutical Federation (FIP) has offered a new resource to help countries self-assess readiness of their regulations to enable pharmacist-delivered vaccination services and, where needed, to implement such regulations. Developed by FIP's Regulators Advisory Group, it covers broad pharmacist-led roles related to vaccination from supply chain management to administration and prescribing. "This new resource from FIP is based on the experiences of professional regulators in countries where pharmacy-led vaccination has been successfully introduced and regulated. With it, we aim to support regulators, policymakers and national organisations in developing strategies or enabling regulatory frameworks in countries where the pharmacy workforce has a limited role in vaccination strategies so that it can contribute further to the global imperative of improving immunisation coverage," said Brett Simmonds, group chair. Where they contain the Home Office approved wording "Please dispense instalments due on pharmacy closed days on a prior suitable day" then you will need to plan to make these supplies at an appropriate time. This wording would also permit the supply of the initial dose of a prescription starting on the Bank Holiday Monday where the date of the signature is before this." However, PSNC is aware that not all prescriptions will contain such a direction to support the supply, in advance, of a dose for Monday 19 September where the supplying pharmacy is closed. "We recognise that this client group are often vulnerable with multi-morbidities and that the risk to them may be significant. You should therefore exercise your professional discretion in determining when to make an instalment supply for the Bank Holiday Monday. The UK Health Security Agency (UKHSA) has suggested that the Live Attenuated Influenza Vaccine (LAIV) vaccine that offers protection to children against flu may also help reduce the rate of group A strep infections. LAIV is a nasal spray offered each season to most children aged 2 and 3 years old, and to school-aged children, to help protect against flu. The new study looked back at data from 2013 to 2017, comparing rates of group A strep (GAS) infections in pilot areas and comparing them to other areas where the vaccine was not being offered as widely. It was found that incidence of GAS was lower in pilot areas where the LAIV vaccine was being offered to all primary school children, compared to areas where it was being incrementally rolled out. The study revealed, in 2 to 4 year olds, rates of GAS were 73.5 per 100,000 children in pilot areas, compared to 93 per 100,000 children in non-pilot areas. In 5 to 10 year olds, rates of GAS were 50.3 per 100,000 children in pilot areas, compared to 57.8 per 100,000 in non-pilot areas.

National Institute for Health and Care Excellence (NICE)'s diagnostic advisory committee has recommended the use of HM-JACKarc or OC-Sensor quantitative faecal immunochemical tests (FIT) in a draft guidance release for consultation on Wednesday (5 July). Under existing NICE guidance, FIT was already offered to some people presenting to primary care with symptoms suggestive of colorectal cancer, while others were immediately referred on the suspected cancer pathway. The new draft guidance will now see everyone receive a FIT. A sample is sent in the post to a laboratory where the amount of blood in the faeces is measured. The results are usually available within a week and people with 10 or more micrograms of haemoglobin in their faeces should then be referred for further investigation. Further assessment using colonoscopy, or CT colonography, is required to diagnose cancer. The tests cost between £4 and £5 per sample, and can correctly identify about 9 out of 10 people with colorectal cancer. The committee agreed it is important that GPs can refer people for colonoscopy without a positive FIT result if they think it is necessary and where symptoms persist. The institute believes that the recommendation of the tests should reduce the number of unnecessary colonoscopies, thus freeing up appointments for more non-urgent referrals. This should lead to 50% fewer referrals for urgent colonoscopies being made by GPs in primary care settings each year.

It added: "The range and availability of health-care services must increase to reflect citizens' demands and their increasingly complex needs. Nearly 70 years ago the UK Government recognised the imbalance of power between commercial landlords and tenants and passed the Landlord and Tenant Act 1954 (the Act), giving most commercial tenants a right of security of tenure. What is security of tenure? The Act grants tenants of business premises (so this would include pharmacy tenants) the right when the fixed term of their lease has ended to remain in occupation of the premises and the right to apply for the grant of a new lease. The landlord can only object to the grant of the new lease of the premises to the tenant, and therefore regain possession of the property on certain grounds set out in the Act. The parties to a lease can contract outside the provisions of the Act and where this is the case, a tenant would not have security of tenure. Most landlords will insist on no security of tenure where a lease is granted for a short term. As pharmacy leases have tended to be granted for longer terms (usually between 10 and 15 years) pharmacy tenants often have security of tenure. The Act sets out strict procedures which need to be followed to both contract outside the provisions of security of tenure and also to exercise the security of tenure rights granted by the Act. Pharmacists should take legal advice before agreeing to a pharmacy lease being excluded from the Act, and also at the end of their lease term whether or not they have security of tenure. If the lease is contracted outside of the Act, advice should be sought on agreeing new lease terms, if the pharmacist wishes to remain, as there will be no right to do so beyond the end of the lease term. If the lease is protected by the security of tenure provisions of the Act, advice should still be sought, as the Act prescribes a formal notice procedure that both the landlord and tenant must adhere to, before a new lease can be granted.
