A new legislation has been introduced to allow authorised pharmacists along with other healthcare professionals in Northern Ireland to certify 'fit notes', health minister Robin Swann has announced. Pharmacists and some other healthcare professionals in England, Scotland and Wales are already allowed to sign 'fit notes'. Swann announced that 'fit notes' were also set to go fully digital in the future, meaning they would no longer need to be signed in ink. He said: "I am delighted to introduce this legislation in Northern Ireland. This change will make it easier for people and employers to get the advice they need so people can stay in work and will help free up doctors' time. "Too often we see people being faced with unnecessary challenges to get a fit note. More professionals being able to offer this vital service will speed up the process and support people to return to or remain in work."

The Department of Health and Social Care has clarified that community pharmacists will not be among those allowed to issue sick notes, calling the service unsuitable to be provided over the counter. In response to a question from Pharmacy Business, a spokesperson for the department said that the policy change was "aimed at pharmacists working within general practices or hospital settings", adding: "The intention is that 'where' the fit note is issued will remain the same but 'who' issues it will change. The DHSC went on to explain that fit notes, known technically as 'Med3', were "not part of the NHS commissioned services" and therefore private providers, such as high street pharmacies, were unable to provide it under government provision. It added that since "fit notes should only be issued following a full assessment of a patient's fitness for work", it "would be an unsuitable service to be provided 'over the counter'.

The General Pharmaceutical Council (GPhC) has initiated the evaluation of its five-year strategy for changing the way it manage concerns about pharmacy professionals. It has urged people to share their experience of fitness to practice. The evaluation will look at how the strategy has improved the experience of people involved in fitness to practice (those who raise concerns, professionals, witnesses etc.); impacted the culture amongst pharmacy professionals and their attitudes to openness and honesty; improved the process and made it fairer and changed perceptions of fitness to practice. The first phase of the evaluation process will take place from July to September 2023. The council said: "We are committed to making our ambitions a reality for the benefit of everyone involved and your contribution will help us to make sure patients and the public continue to be protected while being fair to pharmacy professionals." The General Pharmaceutical Council (GPhC) Fitness to Practise Committee has removed a pharmacy technician from Register after been found guilty of possessing 'indecent photographs of a child'. Graeme Arthur, a pharmacy technician first registered with GPhC on 1 September 2019, under the registration number 5039154, was found guilty at Peterlee Magistrate's Court in August 2022. He received an 18-month Community Order, and is subject to a Sexual Harm Prevention Order for 5 years, He was also ordered to undertake a period of rehabilitation activity for 40 days. In the remote video linking hearing held on 2 - 3 May, the Fitness to Practise Committee found Arthur's fitness to practise to be currently impaired on grounds of protection of the public and in the wider public interest of declaring and upholding the Standards of the profession and maintaining public confidence in the reputation of the profession. The committee considers this to be a serious conviction for an offence which has included possession of the most serious category of images of sexual abuse of children. It said: "Although Mr Arthur had no direct contact with the children concerned, the nature of the abuse is that it thrives on the demand from those who search for and view the images online. As such, children come to actual harm indirectly through the activity of someone viewing and possession of images of sexual abuse."

The Pharmacists' Defence Association (PDA) has called for a mandatory training for those who wish to sign fit notes. The union said it will be seeking urgent clarification around the rollout of guidance and timescales of when this will be available to PDA members working in primary care and hospitals, who will be at the forefront of the implementation. "As with any activity, regulated health professionals will be expected to ensure they have received adequate training and guidance before taking on new elements of practice and to ensure they are acting within the limits of their professional competency," said the association. It also highlighted that completing a fit note may have an impact on the income received by the patient and therefore the consequences of supplying the document could mean a direct financial consequence for them, such as eligibility for sick pay or insurance claims, as well as a health outcome. The PDA expects the mandatory training to cover any factors which will need to be considered in this regard so that pharmacists understand any limitations on when, how, and for who they might complete the fit notes.

National Institute for Health and Care Excellence (NICE)'s diagnostic advisory committee has recommended the use of HM-JACKarc or OC-Sensor quantitative faecal immunochemical tests (FIT) in a draft guidance release for consultation on Wednesday (5 July). Under existing NICE guidance, FIT was already offered to some people presenting to primary care with symptoms suggestive of colorectal cancer, while others were immediately referred on the suspected cancer pathway. The new draft guidance will now see everyone receive a FIT. A sample is sent in the post to a laboratory where the amount of blood in the faeces is measured. The results are usually available within a week and people with 10 or more micrograms of haemoglobin in their faeces should then be referred for further investigation. Further assessment using colonoscopy, or CT colonography, is required to diagnose cancer. The tests cost between £4 and £5 per sample, and can correctly identify about 9 out of 10 people with colorectal cancer. The committee agreed it is important that GPs can refer people for colonoscopy without a positive FIT result if they think it is necessary and where symptoms persist. The institute believes that the recommendation of the tests should reduce the number of unnecessary colonoscopies, thus freeing up appointments for more non-urgent referrals. This should lead to 50% fewer referrals for urgent colonoscopies being made by GPs in primary care settings each year.

The General Pharmaceutical Council (GPhC) has revealed it has received 25 whistleblowing disclosures between April 2021 and March 2022. The pharmacy regulator said, in its annual report, that it has concluded enquiries on 22 disclosures so far with a further three still under review. "We also concluded two qualifying disclosures that were raised during the previous reporting period," it said, adding: "The action GPhC took included a full investigation through established fitness to practise processes and follow-up action through its inspection network. "The former can result in any available outcome throughout the fitness to practise process. The latter can include guidance, a follow-up visit or an unexpected inspection." 12 concerns were investigated and concluded with no further action. Three were signposted to another organisation. The remaining seven cases were concluded by sharing information with inspection colleagues for follow up action. Of the two concerns from the previous reporting period, one was concluded with no further action and the other remains under investigation.

The Scottish government has announced funding to help community pharmacy contractors purchase air extraction equipment for their premises. The funding will be available to pharmacies that have fitted or committed to fit air extraction equipment on or after 1 April 2020.
