    Recent NHS data sheds light on the evolving landscape of fit note certification, indicating a gradual expansion of roles beyond traditional medical practitioners to pharmacists and nurses. This uptake in numbers coincides with the Department of Work and Pensions's (DWP) call for evidence in relation to fit note reform which will remain open till 8 July 2024. According to figures published by NHS Digital, during the nine-month period from April 2023 to December 2023, pharmacists certified a significant number of fit notes. Approximately 1.4 per cent of fit notes of the 8 million were issued by pharmacists while the rest were issued by the GP surgeries reflecting the impact following legislative amendments in July 2022, which extended certification authority to pharmacists, nurses, occupational therapists, and physiotherapists. In June 2022, the Pharmacists' Defence Association (PDA) advised pharmacists not to sign fit notes where the required therapeutic, diagnostic, or examination skills lie outside their scope of competence. The association remarked that healthcare professionals needed mandatory training under the Health Education England before they are fit to issue fitness notes to the public.

Swann announced that 'fit notes' were also set to go fully digital in the future, meaning they would no longer need to be signed in ink. He said: "I am delighted to introduce this legislation in Northern Ireland. This change will make it easier for people and employers to get the advice they need so people can stay in work and will help free up doctors' time. "Too often we see people being faced with unnecessary challenges to get a fit note. More professionals being able to offer this vital service will speed up the process and support people to return to or remain in work."

It added that since "fit notes should only be issued following a full assessment of a patient's fitness for work", it "would be an unsuitable service to be provided 'over the counter'.

It also highlighted that completing a fit note may have an impact on the income received by the patient and therefore the consequences of supplying the document could mean a direct financial consequence for them, such as eligibility for sick pay or insurance claims, as well as a health outcome. The PDA expects the mandatory training to cover any factors which will need to be considered in this regard so that pharmacists understand any limitations on when, how, and for who they might complete the fit notes.

New NICE Guidance For Colorectal Cancer Diagnosis - 0 views

    National Institute for Health and Care Excellence (NICE)'s diagnostic advisory committee has recommended the use of HM-JACKarc or OC-Sensor quantitative faecal immunochemical tests (FIT) in a draft guidance release for consultation on Wednesday (5 July). Under existing NICE guidance, FIT was already offered to some people presenting to primary care with symptoms suggestive of colorectal cancer, while others were immediately referred on the suspected cancer pathway. The new draft guidance will now see everyone receive a FIT. A sample is sent in the post to a laboratory where the amount of blood in the faeces is measured. The results are usually available within a week and people with 10 or more micrograms of haemoglobin in their faeces should then be referred for further investigation. Further assessment using colonoscopy, or CT colonography, is required to diagnose cancer. The tests cost between £4 and £5 per sample, and can correctly identify about 9 out of 10 people with colorectal cancer. The committee agreed it is important that GPs can refer people for colonoscopy without a positive FIT result if they think it is necessary and where symptoms persist. The institute believes that the recommendation of the tests should reduce the number of unnecessary colonoscopies, thus freeing up appointments for more non-urgent referrals. This should lead to 50% fewer referrals for urgent colonoscopies being made by GPs in primary care settings each year.

The Parliamentarians acknowledged that pharmacies have the potential to do more to support the NHS and they called for the government to urgently consider a commitment to working with the sector to create a fair and fit for purpose future funding model that ensures sustainability and properly funds pharmacies for what they do. All signatories also called for the introduction of a fairly-funded Pharmacy First service.
