This site is for teachers wanting to promote strong study skills in their classrooms. They have lots of information on the home page and will send out a free digital copy of their program. However, they really want you to sign up and pay for the whole thing. It's a web-based program designed to help struggling students work on their skills at school and from home. Worth a look.
