This article discusses the benefits and pit-falls to autonomous primary care Nurse practitioner practice and advocates for cost effective improvements to our healthcare system. The research suggests that Nurse practitioners who practice independently report greater job satisfaction, improved patient outcomes and provide low cost, high quality services that are equal or superior to primary care physicians. Unfortunately, current healthcare reimbursement models, policies and reluctant physicians have created barriers to autonomous NP practices. The author suggests that autonomous practice requires further research across the U.S, the establishment of a well-defined model and the evaluation of patient outcomes, in order to determine whether the present day Nurse Practitioner requires future collaborative agreements with physicians, in order to provide quality patient care across the United States.

I found this article to be completely fascinating as I was almost completely unaware of the history of the APRN role. I have heard of Lillian Wald and Mary Breckenridge but didn't know the full extent of their backgrounds. I enjoyed the history provided in this article and feel that it was a perfect reading to begin my APRN transition. It provided some important information on how the role of the nurse changed and was diminished by ourselves (ANA) before being re-expanded in the 1960s with the role of the NP. I'd love to hear what others think and if you have other interesting historical articles on the background of the APRN role!

This article looked at the relationship between physician oversight and APRN autonomy and empowerment. It surveyed APRNs in Montana, to determine which factors most influence APRN empowerment and autonomy. Surprisingly, it found that high levels of physician oversight improve APRN empowerment, regardless of whether the oversight is regulatory or simply collaborative. A next step for this study would be to learn more about why physician empowerment increases APRN autonomy - are there certain qualities to the physician/APRN relationship that must be established?

This article details the researchers' efforts to describe the ability of nurse practitioners to reduce health disparities. By reviewing the literature, the researchers were able to create a "Nurse Practitioner Health Disparities Model" that outlines the many factors that influence a NP's ability to reduce health disparities. Outside actors, such as policymakers and insurance companies, can also influence the ability of nurse practitioners to reduce health disparities. Thus, the model is a useful tool in promoting full practice for APRNs.

This resource is a research study analyzing the differences in cost and pediatric patient outcomes between hospital teams run by residents alone, PNP's alone and PNP/MD teams. Overall, the researches concluded that incorporating PNP's into hospital care can reduce costs for patients requiring certain treatments as well as provide care that is comparable to the care given by medical residents.

This article explores how APRNs in the state of Iowa, as part of a state-sanctioned task force, identified barriers to practicing at the top of their licensure. This article provides a brief history of the APRN role and the advancement from a CRNA, to midwives, then to FNPs. The focus is the family nurse practitioner role. The author shows the connection with Erik Ericsson's stages of development and provides examples of how the role of the NP progressed through those stages. Examples are also provided from a world development perspective, as each country has also had to move through the same phases of development before moving on to the next. The author concludes that we (the U.S.) are still in the final stage of development (Integrity-older adult).

This article explores how restrictions on CNM scope of practice affect health outcomes. Scope of practice laws were found to be neither helpful nor harmful regarding health outcomes. However, states without scope of practice barriers for CNMs had lower rates of induced labor and cesarean deliveries.

When I typed in "APRN history" into Google, this was the first item to populate. The article is from the American Association of Nurse Practitioners website and is a timeline of significant events that have occurred over the course of APRN history. I was surprised to see that one of the first NP programs was created at Boston College. My mother is a graduate from their BSN program! The AANP was developed in 1985 and has since helped pioneer the career. Enjoy!
