When I found http://www.diigo.com/user/grahamperrin?domain=www.ncsu.edu with no annotations following a browsing session, I assumed a problem with my Internet connection during the previous session. I hadn't been paying attention to the status within the toolbar of Diigolet 3.1b538 after each highlight.
Please, are there service issues affecting reliability/consistency of annotations?
Second only to creation of bookmarks, I treat reliability of annotations as a top priority. It's the core feature within Diigo's tag line "Highlight and Share the Web!".
I can't bear the repeated, unpredictable losses so until further notice I'll probably:
* mark the vast majority of bookmarks as unread * read and annotate only when the need is great.
I'll punctuate annotations with repeated reloads of My bookmarks, repeated reloads of the All annotations view of the bookmark on which I'm working. This may occasionally put strain on the service, affecting some other users, but it's a necessary routine to watch for dataloss.
http://www.diigo.com/06rvy fails to show at least two highlights:
* one of which was applied a few hours ago
- reapplied at least twice since then
* one of which was applied then reapplied this morning
- but it's impossible to stick a public note to the highlighted string

Highlights are also missing when I view the bookmark in my library.
Comparison
http://www.diigo.com/bookmark/http%3A%2F%2Fjoelfightsback.com seems to be more comprehensive, and proves that public sticky notes were previously possible.

Highlights lost and/or failing to appear and/or duplicated as a result of failure to appear
So I reloaded http://www.ncsu.edu/mac/software/webXkiosk.html and re-applied all missing highlights.
Back to my bookmarks, again all highlights are missing.
Now I find that most are duplicated at http://www.diigo.com/bookmark/http%3A%2F%2Fwww.ncsu.edu%2Fmac%2Fsoftware%2FwebXkiosk.html and http://about.diigo.com/about/http%3A%2F%2Fwww.ncsu.edu%2Fmac%2Fsoftware%2FwebXkiosk.html
PITA :-(

Most recently: my highlight and public sticky note missing from http://help.twitter.com/portal
* missing from http://about.diigo.com/about/help.twitter.com%2Fportal
* missing from http://www.diigo.com/bookmark/http%3A%2F%2Fhelp.twitter.com%2Fportal
* missing from http://www.diigo.com/user/grahamperrin/Diigo+48676
Whenever the bug bites, I tend to carelessly repeat annotations in the hope that eventually, they'll appear.
Postscript
Possible workaround
Recreate all annotations using a different version of Diigo.
Annotations lost, again
