    Home modifications aim to enhance safety and occupational performance in the home. However, given the complexity and unique meaning of the home, they can impact aspects other than function. This qualitative descriptive study aimed to explore the impact of home modifications on clients and their family's experience of home. The study found that home modifications impacted positively and negatively on five dimensions of the home environment, the personal, occupational, physical, temporal and social dimension. The outcomes of the modifications and the home modification process were influenced by three themes, workmanship, consultation or involvement in decision-making, and the societal dimension of the home environment. Conclusions: The negative outcomes and poor consultation experiences suggest the need for occupational therapists to understand their client's personal experience of home and to comprehensively follow-up and evaluate these following home modifications.

    This assessment is based on observation of a patient in a more natural environment/setting and looks at if they initiate expected activities such as, turning off appliances, closing doors when they leave, doing dishes, prepping dinner.. etc within 15 min of expected time. This is looked at and recorded each day of the week. In acute care this is just not very applicable, as we don't watch patients all day, every day of the week and the Vets are in an unfamiliar setting where they likely won't initiate such activities even if cognition is unimpaired. Even the activities that do occur in the hospital setting, such as eating lunch, don't apply very well because the score is acquired by adding up how they did each day of the week and it is not very practical to see a patient every day for the entirety of their lunch for a week straight. This may be a nice assessment for the nursing home to use in some instances.

This included the development of a personal factors guideline to aid OT practitioners' clinical reasoning in home modification interventions. Factors to consider range from course of diagnosis and patient's preference to compliance and resources. This article showed how holistic OT is because it looked at home modification based on all client factors/environment to guide clinical reasoning for interventions.

    As the title suggests, this article discusses occupational therapy interventions aimed at modifying the environment for persons with Alzheimer's disease as well as all forms of dementia. The article is a systematic review which provides the reader with insight into the different types of environmental modifications commonly utilized by occupational therapy practitioners as well as their effectiveness. This particular article is certainly a good read for those of us who are currently completing our level II fieldwork in a SNF, as Alzheimer's disease and related dementias are very common in such settings.

Occupational therapists are beginning to address community well-being more frequently. Currently, the majority of occupational therapy interventions occur within the home for this population. However, the results of this study indicate that there is a potential to successfully address some of these needs on a societal level.

    The authors of this systematic review sought to gather the available evidence base in order to determine best practice in terms of service delivery models and methods used in early intervention. The results of the initial search were categorized into one of 3 categories of service delivery: family centeredness, service setting, and type of parent participation. Through analysis of 18 articles, the authors determined that: parents' perceptions of efficacy and satisfaction increased when interventions were embedded in family routines and settings; these studies support the use of combined approaches across environments, though they did not specifically compare differences in outcomes in varying environments; and parenting classes were found to decrease stress, improve parents' relationships, as well as lead to improved cognitive scores in children when added to early intervention services. The implications of this review on OT practice support the continued use of family-centered approaches, individually tailored interventions, post-intervention assessments to determine effectiveness of interventions, and therapist feedback to increase outcomes.
