    This article, published by AJOT in July 2006, is titled Occupational Therapy Activities and Intervention Techniques for Clients With Stroke in Six Rehabilitation Hospitals. As the title suggests, this article discusses various occupational therapy intervention strategies, as well as their effectiveness, for the treatment of patients with CVAs. Control of upper extremity exercises as well as activities of daily living practice were the most common form of occupational therapy intervention techniques utilized.

    I chose this article because I treat many CVA patients in my setting. I was curious to see how the typical VA treatments related to other hospitals with their treatments. The results were very similar. Mainly we work on functional activities and we add in some exercising to strengthen and further enhance the functional abilities.

    This article looks at the efficacy of a technique called tapping facilitation for CVA patients with hemiplegic UEs. I got to learn about this technique with an inpatient who had a massive CVA when she went in for open heart surgery. Her right arm and hand were completely flaccid. We used this to work on regaining elbow flexion & extension, as well as wrist flexion and extension. This technique involves putting the targeted muscle group on a stretch (i.e. elbow/wrist flexors or extensors) and firmly tapping the muscle bellies until there was a palpable to visually noticeable "twitch" in those muscles. After this twitch was felt or observed we would passively take the elbow or wrist through the ROM we were working on so that the brain could reform the connections.

    this articles reviews the literature on interventions used for treating people who have had a stroke. The most typical approach for treating strokes is using NDT (nuerodevelopmental therapy). This approach includes electrical stimulation, EMG biofeedback, and motor learning approaches. This article discusses other therapy approaches that aren't so common, but do have literature on them. I am currently a neuro site and this article is very helpful.

    This article briefly explains several evidence-based interventions for stroke, including more traditional types of interventions, newer treatment techniques, and complementary treatments. The article points out, however that the evidence is not pointing to one particular treatment that is superior to the others, and therefore, more rigorous research is needed.

It provides an overview of the best available evidence for stroke intervention.     I was very excited to talk talk about this article. During my rotation, I have found that my FE uses kinesiotape fairly often with patients, depending on condition and circumstance, obviously. I know in our MOT class/faculty, we definitely had some "doubters" in the effects kinesiotape can have. One of my patients was a lady in her 70's who had a CVA more than 10 years ago, and has since suffered from significant shoulder subluxation. She came in to therapy after trialing botox. When she came to us, she was very discouraged that botox had not helped, and also discouraged because past OT had not helped her at all, and was now dealing with significant pain from subluxation. After research and discovering this article, I discussed with my FE and we agreed that this California Tri-Pull taping method would be appropriate to try with our patient. In short- the patient found that the tape really helped her, and felt like it was the first thing that had provided her a little bit of relief in a long time. -The taping method itself actually is not too complicated, and did not take long to practice and learn before completing on a patient. In short- this article discusses the methodology for taping, as well as research that backs the effectiveness of the taping method. Overall, the study finds some mixed results in effectiveness in terms of reducing subluxation and reducing pain, but overall can be a promising adjunct to the management of the hemiplegic subluxed shoulder

    This article discusses the various research opportunities available in area of adult stroke victims. Through the last 12 weeks, I have encountered a significant number of patients with CVA at the nursing home. I found the article extremely relevant to current occupational therapy practices due to the nature of treatments regarding CVA's. I think that it is critical for more research to be done, to help OT's better understand the nature of strokes and the best practices to treat them. AOTA developed a table to help researchers, students, and clinicians that summarizes various research opportunities on stroke patients. I believe this can be a helpful tool for OT's looking for innovative ideas and research to help apply to field work.
