This weekly lesson plan is through Excel and is pretty nest because it's easy to edit and use for any class schedule. 

This unit lesson is from the west Virginia Department of Education. The templates are standards-focused and require teachers to plan instructional design. The two links are excellent as to the first link brings you to a description of what should be in each box and explains what it means. The second link is a blank template that comes as a word document so you may type and print it out.
