The Welsh government has reached an agreement with the NHS and Community Pharmacy Wales (CPW)-the statutory representative body for all community pharmacies-on a six per cent funding increase as part of the 2024/25 Community Pharmacy Contractual Framework (CPCF). The £9.9 million increase will bring the total CPCF funding to £175 million per year, representing a 24 per cent rise in funding since 2017. In a statement published on 20 December 2024, Jeremy Miles, cabinet secretary for health and social care, said that the additional investment will be used to address "both immediate pressures facing community pharmacies and to maintain momentum on our longer-term ambitions for reform." Miles also said that the government would continue to invest in integrating pharmacies in primary care clusters, developing the skills and scope of practice of the wider community pharmacy workforce including pharmacy technicians, and the Choose Pharmacy IT system. In contrast, community pharmacies in England are still awaiting progress on CPCF negotiations for 2024/25. Last month, Community Pharmacy England (CPE) warned of the sector's worsening financial situation, urging ministers to provide immediate funding relief.

Community Pharmacy England (CPE) is gearing up to conduct a vital Pharmacy Advice Audit, calling on pharmacy owners and their teams to help measure the increasing reliance on pharmacies within communities. With years of financial strains impacting the sector, despite the much-needed £645 million recovery plan funding, the sector finds itself on the edge. The launch of Pharmacy First sparked a notable surge in people seeking advice from community pharmacies, both within and beyond the funded service, as revealed by recent sector polling. To accurately assess this trend and advocate for sustainable funding, CPE is initiating an audit to gather data crucial for showcasing the extent of unfunded care provided by community pharmacies. Scheduled to commence this week, the Pharmacy Advice Audit will entail pharmacy teams documenting informal healthcare advice dispensed over a single day.

A harsh funding regime that hasn't adapted to changing macroeconomic realities means the viability of large parts of the pharmacy sector is at risk unless the model changes. Very simply, across the sector, revenue has remained largely fixed while costs have increased significantly, making it impossible for many pharmacies to sustain their business models. The sector has seen the closure of more than 1,500 community pharmacies since 2015, with 700 of these closures occurring since 2021. Larger pharmacy operators such as Lloyds, Boots, and Rowlands have seen the biggest decline in numbers, with smaller businesses operating between one and five pharmacies now accounting for almost 50% of the sector. Despite rising inflation and business costs, the NHS pharmacy funding model has remained fixed. Over the period of the current Community Pharmacy Contractual Framework (CPCF) - 2019-2024 - pharmacies have experienced a 30% real terms cut in core funding leading to an annual shortfall of over £750 million, equivalent to £67,000 per pharmacy in England. The current CPCF is due to end in 2024, and there is still no arrangement in place for future funding, leading to delayed and inefficient spending decisions and hampering the ability of the sector to plan and attract much-needed investment.

Community pharmacists across the UK are looking for ways to reduce operational costs and find new sources of revenue. This is a pressing issue for the sector: reversing a decade of cuts to pharmacy funding will be a mammoth task, while pharmacists have taken on new primary care responsibilities under Pharmacy First. This means that fresh sources of revenue and lower costs will overwhelmingly come from new technology and the efficiencies they can bring. As Britain's 11,500 community pharmacies assume a larger role on the frontlines of primary care, pharmacists should rapidly implement cost-effective tech solutions that provide pharmacies with a one-stop-shop to drive down costs and increase revenue. Opportunity in PGD reform New technology will be key for community pharmacists to benefit fully from reforms to PGDs. The scope of PGDs is now significantly expanding: as of June 26, pharmacy technicians are now on the list of registered healthcare professionals who can make use of PGDs. These reforms can benefit community pharmacies in several ways. For one, they will allow more pharmacy staff to administer 'frontline services' such as vaccinations - reducing costs and generating revenue. Further, the wider range of services that pharmacies can offer under Pharmacy First will make these procedures, as well as other kinds of preventative care, more accessible and convenient - increasing footfall. With international travel and tourism now growing apace, increased demand for travel vaccinations means that, according to research from Charac, the average UK community pharmacy can expect to generate £46,800 a year through travel health services.

Community Pharmacy England (CPE) is inviting community pharmacy owners to share their insights and views about increasing Pharmacy First thresholds and the effects of GP collective action on their pharmacies. This feedback will be discussed at the upcoming Community Pharmacy England Committee meeting in September. During the meeting, the Committee members will also discuss urgent matters including the 2024/25 negotiations, collaboration with the new government, and their ongoing strategies. A key point of discussion will also be the effects of GP work-to-rule action on community pharmacies. CPE has raised concerns that many pharmacies might face challenges meeting the scheduled increases in the monthly Pharmacy First payment threshold, noting that pharmacy owners are already considering their options if they start failing to meet the threshold. Calling it "a severe blow to an already financially strained sector", McQuillan emphasised that the additional burden has the potential to threaten the viability of many pharmacies. "Community pharmacies are the frontline of our healthcare system, by penalising these pharmacies for an error not of their making, the Department of Health is jeopardising the foundation of local healthcare delivery," McQuillan criticised. Prime Minister Rishi Sunak, during today's (15th May ) Prime Minister's Questions (PMQs), reiterated his commitment to supporting community pharmacies amidst growing concerns over pharmacy closures. Addressing questions regarding the Pharmacy First initiative, Sunak assured the House of Commons of his dedication to bolstering resources for these vital healthcare providers. In response to a query raised by Member of Parliament, St Ives MP Derek Thomas, citing statistics from the National Pharmacy Association (NPA) about "committing to directing funds towards Community Pharmacy to alleviate financial pressure and prevent closures, Sunak remarked that he "cares deeply about the future of community pharmacies." Sunak acknowledged the indispensable role played by community pharmacists in alleviating pressure on urgent care services, emphasising the importance of initiatives like Pharmacy First in government's commitment to supporting community pharmacies. "There are over 10 and a half Community pharmacies across the country working incredibly hard to serve their patients," Sunak said praising the community pharmacists and highlighted the concerning trend of pharmacy closures, citing a significant increase compared to the previous year.

A new report from Community Pharmacy England (CPE) has raised alarms about the severe financial challenges plaguing pharmacies across the sector, threatening their ability to provide even the core services for their patients. The findings are based on responses from over 6,100 pharmacy owners and 2,000 pharmacy staff who participated in the recent Pharmacy Pressures Survey 2024. The report reveals that spiralling costs and increased workloads, coupled with a 30 per cent funding cut in real terms since 2015, are putting countless pharmacies at risk of closure. "Nearly 1 in 6 may close within the next year," the report revealed. A vast majority (94 per cent) of pharmacy owners reported that they have seen significant increases in costs, with almost two-thirds (64 per cent) saying they are operating at a loss. Concerningly, these financial pressures are now affecting patient care, as 18 per cent of pharmacy staff reported severe impacts. The survey revealed that most pharmacies are experiencing longer prescription dispensing times (86 per cent), delays in responding to patient inquiries (80 per cent), and less time to spend with patients (79 per cent).

The National Pharmacy Association (NPA) will deliver the national #saveourpharmacies petition to Prime Minister Keir Starmer and Health Secretary Wes Streeting on September 19. Over 300,000 people have signed the cross-sector petition in support of hard-pressed community pharmacies across the UK, which have been ravaged by funding cuts over the past decade. The NPA is inviting pharmacy bodies in England to join them on the second national pharmacy day of action to demonstrate unity in calling for a new deal to support the pharmacy sector. Earlier this year, the NPA distributed petition sheets to pharmacies across the country to maximise the number of signatories to increase pressure on the government to act in support of the pharmacy network. In preparation for the September 19 day of action, the NPA is sending out thousands of campaign packs including medicine bag stickers, posters, and postcards, to pharmacies across the UK. Through these materials, the NPA is urging pharmacies and their patients to highlight the issues of funding cuts and closures.

Sector leaders have welcomed the NPA ballot results, which demonstrate that community pharmacies are prepared to take collective action if a funding increase is not achieved. Nearly all pharmacy owners, who participated in the NPA ballot, said they were willing to limit their services to protest funding cuts. Commenting on the poll results, Malcolm Harrison, CEO of the Company Chemists' Association (CCA) said: "The results of the NPA ballot serves to underline the huge pressure that pharmacy contractors face, following a decade of underfunding. He emphasised the need to start the Community Pharmacy Contractual Framework (CPCF) renegotiations for 2024/25 without delay. "The government and NHS must uplift all elements of the contractual framework, to stop further reductions in pharmacy opening hours and permanent pharmacy closures, and to ensure that patients can continue to receive the medicines and clinical care and advice they need," he added. Harry McQuillan, Chairman of Numark, stated that while the NPA ballot outcome has once again brought the financial crisis facing the sector into focus, this is "an everyday reality" for many independent pharmacies and that it has reached "breaking point." He warned that rising operational costs including the increase in national insurance and minimum wage will push many more to the brink.

"We need to fix the front door to our NHS by investing in community pharmacy," asserts Daisy Cooper as more and more pharmacies "worry about the future of their business." Reflecting on the challenges faced by pharmacists during the pandemic, she emphasised their pivotal role in launching vaccination schemes across communities and the larger role pharmacies play in complementing traditional NHS facilities. Cooper recalls her collaboration with local pharmacies in St. Albans, where she worked hand in glove with them to address challenges faced by pharmacists to obtain information from NHS England regarding protocols and procedures. She told Pharmacy Business reporter that she had to work out something "hand in glove to help them get those vaccinated schemes up and running, as pharmacies were desperate to take part in the vaccination scheme" in her constituency. Describing their unique selling point (USP), Cooper emphasised the convenience and immediacy of pharmacies located in high street areas and warned that pharmacy closures "should be a real wake up call for the government."

The new Labour government's first budget has not been well received by community pharmacies, who are 'deeply worried' about the increase in National Insurance and the national living wage. They have also voiced their dissatisfaction with the Autumn Budget 2024 for lacking measures to address pharmacy closures. Commenting on the Chancellor's Budget, Paul Rees, CEO of the National Pharmacy Association (NPA) said: "Millions of people who depend on local pharmacies will be holding their breath today, hoping that the £22.6bn increase in health spending announced by the Chancellor will include money to stem the devastating closure of local health services in the past decade. "There's absolutely no mention in the Budget of action to halt the closure of our vital NHS pharmacy network, which has been shrinking at the rate of seven a week as pharmacies are forced to close through underfunding." According to the NPA, around 700 pharmacies have shut since just the start of 2022 due to the impact of a 40 per cent cut to funding coupled with rising demand.

Pharmacies are valuable healthcare assets, meeting increasing public demand. However, a substantial portion of their capacity is consumed by unfunded work. According to Community Pharmacy England's (CPE) 2024 Pharmacy Advice Audit, community pharmacies conduct over 1.3 million unfunded consultations each week, equating to 69 million annually. The average pharmacy completes around 22 of these consultations daily, each taking approximately 5.6 minutes of staff time -adding up to over two hours daily. This represents nearly 50 per cent more consultations than in 2020 and a 78 per cent increase in time spent on unfunded healthcare advice since the 2020 audit. The audit, which surveyed over a third (3,916) of community pharmacies in England earlier this year, also highlights that pharmacies handle nearly 150,000 informal referrals from GPs and NHS 111 each week. All these referrals could have been routed through the Pharmacy First service, which would have potentially secured over £115 million in funding for the sector, the CPE noted.

Community pharmacies are bracing themselves to withdraw services from patients after an industry ballot supported plans for major action unless the government provided a significant increase in funding for the sector. The National Pharmacy Association (NPA) on Tuesday (12) announced the results of an unprecedented collective action ballot in which nearly all pharmacy owners, who participated in the ballot, voted to limit their services, insisting that a lack of additional funding is jeopardising patients health and safety. Over 63 per cent of NPA members in England, Wales and Northern Ireland took part in the ballot- representing 3,049 independent community pharmacies in England alone, or 3,399 with Wales and Northern Ireland included. Nearly 98 per cent of pharmacy owners in England voted to reduce their opening hours to the minimum required by their contract. This could result in fewer pharmacies remaining open during evenings and weekends. More than 93 per cent voted to stop offering "unfunded" free home deliveries of medicines, and over 96 per cent voted to withdraw from locally commissioned services, including certain addiction support, emergency contraception, and stop smoking services.

The UK has witnessed a nearly 50 percent increase in pharmacy closures this year compared to the same period in 2023, which was already considered the worst year in recent memory. A new analysis by the National Pharmacy Association (NPA) revealed that 177 pharmacies closed their doors between January and April 2024, compared to 116 in the same period last year. This is the equivalent of 10 local pharmacies shutting down every week. Analysis of NHS figures by the NPA showed that 403 pharmacies closed their doors last year, nearly five times higher than the number that closed in 2022. Since 2017, over 1,400 pharmacies have shut down across the UK. Rising costs and declining real terms funding have led to the fastest rate of closures in decades, said the NPA, highlighting that "real terms funding has decreased by 40 per cent over the last decade."

NHS England announced at the Pharmacy Show this morning that Ministers have decided to amend the Pharmacy First thresholds for the remainder of the year. This decision follows concerns raised by Community Pharmacy England (CPE) regarding difficulties faced by pharmacy owners in meeting the required number of clinical pathway consultations for monthly Pharmacy First payments In response to CPE's representations to the Department of Health and Social Care (DHSC) and NHS England, the threshold was earlier reduced from 20 to 15 in August, and from 30 to 20 in October. At the Pharmacy Show on Monday, Ali Sparke from NHS England announced the revised thresholds, setting them at 20 for November and December 2024. Starting in January and February 2025, the number of clinical pathway consultations needed for the monthly Pharmacy First payment will rise to 25, with a further increase to 30 in March.

The National Pharmacy Association (NPA) has warned of potential pharmacy closures in the coming months if long-standing funding cuts are not addressed and pharmacies are not supported with the National Insurance rise. With Downing Street suggesting that GP surgeries may receive extra support to offset the increase in employer National Insurance contributions (NICs) later this year, NPA chief executive Paul Rees has called on the government to extend similar support to pharmacies. A No 10 spokeswoman clarified that contracted workers, including GPs, were not eligible for an exemption from the NICs hike, which she said was consistent with the approach of previous governments. "There is a general process whereby departments, the Department of Health, for example, confirm their funding for general practices," the spokeswoman said, noting this process would occur later in the year. In response, Paul said that it would be "an insult" if GPs were provided support for the National Insurance rise but not hard-working pharmacies, who have faced nearly a decade of cuts in funding and are shutting at record rates.

The National Pharmacy Association (NPA) has disclosed alarming figures on pharmacy closure in England, attributing them to the impact of funding cuts. According to NPA analysis of NHS Business Services Authority (NHSBSA) data, 222 pharmacies permanently closed their doors in 2024, marking the second-highest annual closure rate on record. This equates to an average of four pharmacies closing each week. Nearly 90 per cent of council areas have seen at least one pharmacy shut permanently in just the last two years. NPA chair Nick Kaye voiced concern that pharmacies have been "pushed to breaking point" by 40 per cent real terms cuts to their budgets and the ones still open are left "hanging on by their fingertips" and could be forced to close without "an urgent increase in their funding." "It is patients that have lost out the most, with more people in isolated areas having to travel further for vital medication as well as access to medical advice convenient to them," he said.

Knights Pharmacy has significantly expanded its footprint in South Wales through its recent acquisition of pharmacies in Abercarn and Cwmcarn from the Evans Group. This move marks another strategic expansion for the Redditch-based pharmacy group, bolstered by funding from HSBC UK. The Abercarn site, which handles approximately 12,000 medication items annually, and the Cwmcarn site, dispensing around 5,000 items, will now operate under the Knights Pharmacy banner. This acquisition adds to Knights Pharmacy's growing portfolio, which now spans 49 community sites across England and Wales, a substantial increase from 18 sites just three years ago.

Community Pharmacy England (CPE) has taken an interesting step to increase public awareness and understanding of the Pharmacy First service. The organisation has launched a new video animation that provides clear and concise information about the service, highlighting the seven conditions covered, the eligible age ranges within the clinical pathways consultations, and explaining how patients can access these services. Pharmacy owners and their teams are encouraged to use the new animation with existing resources to communicate the benefits and scope of the service to their  patients. The animation can be linked to pharmacy social media content and downloaded for display on digital screens in pharmacies. General practices are also encouraged to use the animation on digital screens in their practice waiting areas.
