The General Pharmaceutical Council (GPhC) has unveiled its Carbon Net Zero Action Plan for sustainable pharmacy regulation, aiming to achieve carbon net zero by 2040. The action plan focuses on three main elements: setting an example, incorporating sustainability into regulatory standards, and fostering collaboration for sustainable pharmacy care. Roz Gittins, Chief Pharmacy Officer at the GPhC, emphasised the urgent need for pharmacy sector to embrace sustainable healthcare practices. "Climate change is a global emergency and has been declared the biggest threat to public health," she said, adding that it not only poses health risks but also amplifies existing health inequalities, with immediate consequences for patients, the public and health services. She highlighted that the GPhC is actively engaged in addressing climate change, both as an employer and regulator. "Our aim is to positively influence the pharmacy sector to embrace climate action and sustainable healthcare principles in line with our vision to have safe and effective pharmacy care at the heart of healthier communities," Gittins said.

Sustainability also brings financial benefits to businesses. This has been proven by Prabjaudt Singh Channa, Director of Priory Pharmacy. Prabjaudt is a proud winner of the Sustainable Pharmacy of the Year 2023, awarded by Pharmacy Business in association with Teva UK. Prabjaudt thanked the award organisers for acknowledging the importance of sustainability within the sector and for recognising his modest endeavours to reduce environmental impact and safeguard future generations. "I just felt really happy and proud of this recognition because originally when I set myself down on this path, I didn't really think anyone would be that bothered about the small changes that we were trying to make and the impact," says Prabjaudt. In addition to his family's background in the field, his passion for chemistry also drove him to pursue a career in pharmacy. He finds being a pharmacy contractor enjoyable because it allows him to be flexible, pursue various interests and shape his professional path.

For patients with stage 4 or 5 chronic kidney disease and hepatitis C virus (HCV) infection, 12 weeks of treatment with glecaprevir and pibrentasvir results in a high rate of sustained virologic response, according to a study published online Oct. 11 in the New England Journal of Medicine. Edward Gane, M.D., from Auckland City Hospital in New Zealand, and colleagues conducted a multicenter trial to examine the efficacy and safety of combination treatment with the NS3/4A protease inhibitor glecaprevir and the NS5A inhibitor pibrentasvir for 12 weeks in adults with HCV infection and compensated liver disease with severe renal impairment, dependence on dialysis, or both. Participants had stage 4 or 5 chronic kidney disease. One hundred four patients were enrolled in the trial. The researchers found that the sustained virologic response rate was 98 percent. During treatment, none of the patients had virologic failure, and none had a virologic relapse after the end of treatment. Pruritus, fatigue, and nausea were reported in at least 10 percent of the patients. Twenty-four percent of the patients reported serious adverse events. Because of adverse events, four patients discontinued the trial treatment prematurely; three of these had sustained virologic response.

Released on Monday morning, the election manifesto shared by the Liberal Democrats pledges to "strengthen patients' rights" by tackling public health inequalities and providing early access to community services. The Liberal Democrats Party's manifesto promises to address the issues at both the "front and back doors" of the National Health Service (NHS), which the party claims has been plunged into crisis by the Conservatives. Pledging to invest in public health and early access to community services, the party aims to "work towards a fairer and more sustainable long-term funding model for pharmacies and build on the Pharmacy First approach to give patients more accessible routine services and ease the pressure on GPs." The pledge also includes making prescriptions for people with chronic mental health conditions free on the NHS, as part of a wider commitment to review the entire schedule of exemptions for prescription charges.

To maximise the benefits of the Pharmacy First service for patients, the NHS, and community pharmacies, Community Pharmacy England (CPE) has advised the community pharmacists to to establish a robust relationship between community pharmacies and local general practices. The news comes after several reports highlighted issues regarding GP referrals for the seven minor ailment conditions under the Pharmacy First services. According to the reports, many pharmacists claimed that "many GPs in England are not sending patients to them to be treated - and that some are refusing to participate at all in the "groundbreaking" Pharmacy First scheme". In response to foster better relationship with the GPs, CPE has said that strong local collaborations will enable general practice teams to confidently refer patients to the Pharmacy First and other community pharmacy services where appropriate. Moreover, the organisation emphasised implementation of long-term changes in practice will take time. The current focus on integrating GP referrals to Pharmacy First will likely require sustained effort over several months or longer.

'Community pharmacy is far too important to be allowed to wither away' desc-Thousands of pharmacies took part in the second day of the 'Save Our Pharmacies' protest organised by the National Pharmacy Association (NPA) on Thursday. Similar to the first day of protest in June, pharmacies carried out several symbolic actions, such as blacking out windows, to warn of the threat of future closures. Meanwhile, NPA chief executive Paul Rees led a delegation of pharmacy leaders to deliver a 350,000-strong petition to No 10 Downing Street, calling for "immediate, fair and sustained funding to safeguard NHS pharmacy services and ensure local pharmacies have a future." The delegation included NPA Chair Nick Kaye; Janet Morrison, CEO of Community Pharmacy England (CPE); Malcolm Harrison, CEO of Company Chemists' Association (CCA), and Gerard Greene, CEO of Community Pharmacy Northern Ireland (CPNI). Steve Race MP, who is the new chair of the All-Party Parliamentary Group on Pharmacy, and Sadik Al-Hassan, one of only two pharmacists in the House of Commons, also joined them.

Pharmacy minister Stephen Kinnock has pledged to enhance the role of community pharmacies, particularly in deprived areas, by making better use of the skills of pharmacy teams. In a video message at the SIGMA UK Community Pharmacy Conference 2024 on Sunday, Kinnock also announced plans to expand the Pharmacy First initiative and introduce an independent prescribing service, making prescribing an integral part of the service delivered by community pharmacies. He said: "Since taking on the role of minister of state for care in government, my message has been very clear: we must get our fantastic primary care sector back on its feet to be able to deliver for our patients. "This government is committed to shifting care from hospital to community, from analogue to digital, and from sickness to prevention, and pharmacies will play a vital role in that." Kinnock acknowledged the pressures faced by the sector and expressed concerns that so many community pharmacies have exited the market over the past decade.

A harsh funding regime that hasn't adapted to changing macroeconomic realities means the viability of large parts of the pharmacy sector is at risk unless the model changes. Very simply, across the sector, revenue has remained largely fixed while costs have increased significantly, making it impossible for many pharmacies to sustain their business models. The sector has seen the closure of more than 1,500 community pharmacies since 2015, with 700 of these closures occurring since 2021. Larger pharmacy operators such as Lloyds, Boots, and Rowlands have seen the biggest decline in numbers, with smaller businesses operating between one and five pharmacies now accounting for almost 50% of the sector. Despite rising inflation and business costs, the NHS pharmacy funding model has remained fixed. Over the period of the current Community Pharmacy Contractual Framework (CPCF) - 2019-2024 - pharmacies have experienced a 30% real terms cut in core funding leading to an annual shortfall of over £750 million, equivalent to £67,000 per pharmacy in England. The current CPCF is due to end in 2024, and there is still no arrangement in place for future funding, leading to delayed and inefficient spending decisions and hampering the ability of the sector to plan and attract much-needed investment.

A new survey has highlighted growing concerns about pharmacists' increasing workload and their often-overlooked role within the healthcare systems. In the survey commissioned by consumer health company Kenvue, over 90 per cent of pharmacists reported a workload increase over the past year, with only 37 per cent considering their current workload sustainable. Their primary concerns include the impact of workload on wellbeing, expanding clinical roles and services, and challenges with staff recruitment and retention. Pharmacists reported spending most of their time advising patients on common ailments such as coughs, colds, pain, and allergies. However, they spend significantly less time on preventive services, which could further reduce pressure on the wider healthcare system. For instance, only seven pharmacists focus primarily on smoking cessation, and just four per cent on oral health. The survey also highlighted pharmacists' sense of being undervalued, with only 22 per cent feeling valued by policymakers, despite their significant contributions to primary care provision.

As NHS England prepares to relaunch its Pharmacy First public-facing campaign on Monday, 11 November, Community Pharmacy England (CPE) has highlighted the need for a sustained, large-scale effort to maximize the service's impact. Minor updates have been made to the original campaign materials from earlier this year, including additional information on the age criteria for the clinical pathways. Community pharmacies are informed that campaign materials are available on the Department of Health and Social Care's Campaign Resource Centre website. These include a campaign toolkit, social media assets, and posters. These materials can be used ahead of the official launch date. CPE chief executive Janet Morrison said: "It's good that we are finally seeing the resumption of the NHS Pharmacy First marketing campaign, alongside new resources to help with promotion, as we head into winter.

Nick Kaye, chair of the National Pharmacy Association (NPA), gave evidence to the UK's official COVID-19 Inquiry on Monday (4 November), highlighting the importance of building a "sustainable and resilient community pharmacy network" to enhance preparedness for future pandemics. Kaye also recommended utilising the existing community pharmacy infrastructure for public health initiatives, such as vaccination programmes, and stressed the need to recognise community pharmacists and their teams as integral members of primary care. "Seven pharmacies a week are closing across the four nations, and that is tragic for any future response. So, a resilient community pharmacy network that exists is going to be key," he told the Inquiry. He noted that 90 per cent of most NPA members' income comes from providing NHS services, underscoring the critical role pharmacies play in healthcare delivery. However, Kaye highlighted that pharmacies were often overlooked and underappreciated during the pandemic.

With the UK General Election set for 4 July, pharmacy bodies are actively engaging with all political parties to secure support for the community pharmacy sector. They are pressing parties to address urgent issues such as pharmacy closures and medicine shortages. "The upcoming general election is a critical opportunity for us to strengthen supporters of community pharmacy in readiness for the new Parliament," said Janet Morrison, chief executive of Community Pharmacy England (CPE). She stressed the need for unity within the sector in advocating for community pharmacy and raising awareness of the issues with election candidates from all parties. As the general election approaches, CPE has called upon all political parties to support pharmacies by "introducing a long-term sustainable funding model and reviewing the medicines supply chain, including much-needed short-term relief measures."

The launch of Pharmacy First sparked a notable surge in people seeking advice from community pharmacies, both within and beyond the funded service, as revealed by recent sector polling. To accurately assess this trend and advocate for sustainable funding, CPE is initiating an audit to gather data crucial for showcasing the extent of unfunded care provided by community pharmacies. Scheduled to commence this week, the Pharmacy Advice Audit will entail pharmacy teams documenting informal healthcare advice dispensed over a single day.

As the general election campaign heats up, major political parties - including Labour, the Conservatives, the Liberal Democrats - have released their manifestos, detailing their plans and visions for the future of community pharmacy. While the Conservative Party promises to expand Pharmacy First initiative, Labour pledges support for small businesses, with plans to overhaul the business rates system. The Liberal Democrats' election manifesto commits to developing a fairer and more sustainable long-term funding model for pharmacies. Announcing their plans recently, Prime Minister Rishi Sunak stated that they are investing in community care services to secure the future of the NHS and make healthcare more accessible. The Conservatives plan to fund these initiatives by cutting NHS managerial positions back to pre-pandemic levels and halving the government's management consultancy expenditure.

Since its launch on January 31st, the Pharmacy First service by the Company Chemists Association (CCA) has achieved a significant milestone, completing over 200,000 consultations to date. This milestone underscores the service's vital role in providing same-day NHS care through community pharmacies across England. Recent analysis by the CCA has revealed a pressing need to bolster GP referrals, with disparities observed between different Integrated Care System (ICS) areas, resulting in what's described as a 'postcode lottery' for patients. The UK incurs substantial healthcare costs for managing dental caries (tooth decay), with per-person expenses estimated at £18,000, a new study has revealed. This figure is notably higher than those reported in countries such as Brazil, France, Germany, Indonesia, and Italy. Published in BMC Public Health and commissioned by the European Federation of Periodontology (EFP), the study also found that the economic impact of tooth decay is disproportionately higher in the most deprived groups. Researchers, including Professors Moritz Kebschull and Iain Chapple from the University of Birmingham's School of Dentistry, suggested that targeted preventative measures could help reduce this financial burden dramatically. "Our study highlights the critical need for early and sustained preventive measures to address the economic and health disparities associated with dental caries," said Professor Chapple. "By focusing on the most vulnerable populations, we can achieve substantial cost savings and improve overall oral health outcomes."
