The purpose of this FOA is to invite applications to promote health equity and improve the health of individuals and populations at risk for suboptimal health outcomes through the use of primary care and community interventions that address chronic conditions, including prevention and management of multiple chronic conditions. n 2016, ACC merged with the Society of Cardiovascular Patient Care (SCPC), a nonprofit organization dedicated to improving the Care and outcomes of patients worldwide facing the same complications. To preserve the legacy of SCPC, an endowment was created to support future cardiovascular quality or performance improvement initiatives, particularly those that underscore the importance of accreditation. To advance that mission, ACCF is accepting proposals that support cardiovascular quality improvement of cardiovascular Care and patient outcomes. Awards of up to $100,000 will support projects that contribute to the evidence base for accreditation; develop or evaluate cardiovascular quality initiatives that strengthen the accreditation process; provide opportunities for education that support quality or performance improvement projects; improve EMS systems of Care in collaboration with community hospitals providing accreditation services; or provide public health education to improve community measures/outcomes related to early recognition of heart attack symptoms and appropriate action steps.

Given the nascent stage of development, the small diverse patient populations, and the rapid expansion of the field, conducting research and establishing best practices in pediatric critical care is challenging. National conferences that convene experts from across the country hold the potential to establish the frameworks necessary to overcome these obstacles. It is anticipated that such conferences will foster collaboration among different clinical disciplines, geographically and culturally diverse institutions and medical centers, professional organizations and NIH Institutes. Specifically, it is hoped that these conferences will drive the development and acceptance of uniform definitions, facilitate the determination of common data elements, identify key knowledge gaps to advance research, and/or foster discussions on how research findings can be used to best inform guidelines for patient care .

This notice announces the opportunity to apply for State and Regional Primary Care Association (PCA) Cooperative Agreements funding to provide training and technical assistance (T/TA) to existing and potential Health Center Program award recipients and look-alikes1 (hereafter referred to as health centers). The Health Resources and Services Administration (HRSA) seeks to fund up to 52 organizations to provide T/TA to support health centers to provide comprehensive, high quality primary health Care and improve the health of individuals and communities, through the following goals: * Increase access to comprehensive primary Care; * Accelerate value-based Care delivery; * Foster a workforce to address current and emerging needs; * Enhance emergency preparedness and response; and * Advance clinical quality and performance.

This initiative will fund the dissemination and implementation of PCOR clinical and organizational findings into primary care practices to improve healthcare quality with a focus on cardiovascular care. AHRQ is particularly interested in applications from States with the highest cardiovascular disease (CVD) burden. The initiative will accomplish this goal by catalyzing the development of a sustainable, State-based external primary care quality improvement (QI) support infrastructure to expand the States current and future capacity to disseminate and implement PCOR evidence into primary care practice.

This Funding Opportunity Announcement (FOA) seeks to develop and test the next generation of interventions delivered through mobile health (mHealth) technology to improve diagnosis, linkage to care, retention in care, and viral suppression among youth living with human immunodeficiency virus (YLWH). This FOA supports planning for a clinical trial and can include a small pilot clinical trial that a) incorporates emerging and cutting edge technologies to enhance outcomes along the HIV care continuum, b) supports real-time clinical decision making, and c) facilitates effective long-term management of HIV. Critical to this FOA, proposed research should identify specific patient outcomes along the HIV care continuum that are expected to improve from technological approaches.

This funding opportunity announcement (FOA) seeks to stimulate research that tests optimal end-of-life and palliative care (EOLPC) interventions/models of care that are based on individual- and family-centered outcomes. The testing of EOLPC interventions and models of care are urgently needed that address racial, ethnic and/or cultural diversity in children and adults for those with serious, advanced illness. Trials are needed to test efficacy and effectiveness of these interventions and/or models of care.

This Funding Opportunity Announcement (FOA) invites applications to conduct innovative and feasible studies to test strategies to accelerate the adoption of guideline-based recommendations into clinical practice among populations with health disparities. Applications that propose strategies with a focus on providers who care for clinical populations with excess burden of cardiovascular, lung, blood, and sleep diseases and disorders, in concert with the health care delivery systems in which they practice, are strongly encouraged. Applications that test systems, infrastructures, and strategies to implement guideline-based care for NHLBI disorders in clinical care settings are also of high programmatic interest.

The Centers for Medicare & Medicaid Services (CMS) is announcing the availability of funds under this grant program to assist researchers in conducting health services research for 2013. The purpose of the Hispanic grant program is to implement Hispanic health services research activities to meet the needs of diverse CMS beneficiary populations. The grant program is designed to: 1) encourage health services and health disparities researchers to pursue research issues which impact Hispanic Medicare, Medicaid, and Children Health Insurance Program (CHIP) health services issues, 2) conduct outreach activities to apprise Hispanic researchers of funding availability to conduct research-related issues affecting Hispanic communities to expand the pool of applicants applying for such grants, 3) assist CMS in implementing its mission focusing on health care quality and improvement for its beneficiaries, 4) support extramural research in health care capa city development activities for the Hispanic communities, 5) promote research that will be aimed at developing a better understanding of health care services issues pertaining to Hispanics, and 6) foster an network for communication and collaboration regarding Hispanic health care issues.

The Lesbian Health Fund (LHF), a program of GLMA: Health Professionals Advancing LGBT Equality, was established in 1992 to define, study, and educate lesbians and their health care providers about lesbian health issues. LHF's mission is to improve the health of lesbians, other sexual minority women (SMW) and their families through research. Research areas include: Understanding social, family, and interpersonal influences as sources of stress or support Eliminating inequalities in health care, including barriers to care, and improving quality of care and utilization rates Development and testing of interventions to address mental and physical health needs of lesbians and other SMW, including but not limited to depression, identity related issues, eating disorders, substance abuse, obesity, cancer risks, cardiovascular disease and sexually transmitted infections Sexual and reproductive health, including family & parenting issues The deadlines for receipt of applications are May 15 and October 15.

i. Purpose: The purpose of these activities is to support the goals of the HHS Action Plan for the Prevention, Care, and Treatment of Viral Hepatitis, 2014-2016 (available at http://aids.gov/pdf/viral-hepatitis-action-plan.pdf) by ensuring Hepatitis B-infected pregnant women are identified so that their infants can receive timely post-exposure prophylaxis, improvements in post-vaccination serologic testing to improve efficiencies, and data collection to assess infant outcomes ii. Outcomes: Increased identification of Hepatitis B-infected pregnant women; increased rates of post-vaccination serologic testing among infants born to Hepatitis B-infected pregnant women; and assessment of factors associated with infant outcomes iii. Strategies and Activities: Collaborations: To maximize opportunities for Hepatitis B prevention through vaccination, referral for Care, and treatment of persons found to have chronic Hepatitis B infection, this FOA encourages Perinatal Hepatitis B Prevention Program collaborations and service integration as a program imperative of the National Center for HIV/AIDS, Viral Hepatitis, STD, and TB Prevention. Such collaborations can increase efficiency, reduce redundancy, eliminate missed opportunities, and improve outcomes through the use of shared data and services. a. With CDC-funded programs: Applicants should create and build upon internal health department collaborations to improve identification of Hepatitis B-infected pregnant women; screen their household and sexual contacts for Hepatitis B and complete vaccination of susceptible persons; refer persons with chronic Hepatitis B infection for Care and treatment; and report infants, household, and sexual contacts with chronic Hepatitis B infection to the National Notifiable Diseases Surveillance System. b. With organizations external to CDC: Opportunities for collaboration with non-CDC organizations will be encouraged; non-CDC organizations may include commercial laboratories and health system

This FOA aims to increase testing of populations most affected by chronic hepatitis B and/or hepatitis C; and training of health care providers. Part A is Networking & Capacity Building and Part B is Professional Education & Training. Part A strategies & activities should focus on leading and growing an existing national coalition that is geographically diverse, is in at least different 10 states; and whose members are actively involved in viral hepatitis. The aim is to share strategies, materials, and lessons learned among coalition members; provide technical assistance to conduct culturally competent educational outreach; build the capacity to test the target populations for chronic viral hepatitis; and engage members in implementing CDC’s national viral hepatitis educational campaign(s).Outcomes for Part A are: increased number and capacity of coalition members to deliver culturally competent educational outreach to target population; increased numbers of individuals tested for chronic viral hepatitis; and increased exposure to CDC’s national education campaign. Part B activities are: developing free, accurate, up-to-date, CE-accredited comprehensive web-based, on-demand hepatitis materials, resources and trainings for health care professionals. The outcomes for part B are increasing the knowledge and skills of health care professionals, including the ability to screen, diagnose, medically manage, and provide care to people with chronic viral hepatitis.

The Rita and Alex Hillman Foundation supports nurse-led healthcare innovation and research, with the goal of improving patient care in hospitals, clinics, homes, and communities. Nurses operate at the junction of public policy, medical science, systems-based healthcare delivery, and one-on-one patient care. Drawing on its knowledge of this unique and important role, the foundation believes it has valuable insights with respect to relevant modes of research and alternative models that can lead to more equitable, patient-centered health care. To that end, the foundation is accepting applications for its Hillman Emergent Innovation Serious Illness and End of Life Emergent Innovation program. Through the program, up to five grants of $50,000 will be awarded in support of pre-evidence or untested innovations that challenge traditional approaches to serious illness and end-of-life care and lead to bold, nursing-driven interventions targeting the unmet needs of vulnerable populations, including the economically disadvantaged, racial and ethnic minorities, LGBTQ people, the homeless, rural populations, and others.

CDC announces supplemental funding for organizations that previously were awarded funding under the Notice of Funding Opportunity (NOFO) CDC-RFA-OE-17-1701. The purpose of his supplemental NOFO is to further strengthen the nation's capacity to carry out public health activities in the area of primary immunodeficiency diseases (PID) to widen the scope of the current grant, increase and improve physician education and public health awareness for/of PID. The intent is to increase the scope of the campaign to disseminate educational information on a national level to health care providers, educators, third-party payers, impacted families, and others who may help expedite clinical recognition and improve health outcomes for Americans with primary immunodeficiency diseases. The intended outcomes of this supplemental grant include increase in scope, in direct scale to conduct the following activities: - Development of materials and implementation of displays - Providers engage in education opportunities - Skills and knowledge of health care providers about primary immunodeficiency diseases increases - Improve integration of primary immunodeficiency diseases prevention into clinical care - Expedited clinical recognition of primary immunodeficiency diseases - Increase community and provider knowledge of primary immunodeficiency diseases - Increase the number of people appropriately diagnosed with primary immunodeficiency diseases - Increase access to care for people with primary immunodeficiency diseases. Agency for Toxic Substances and Disease Registry (ATSDR) in collaboration with the US Environmental Protection Agency (EPA) has maintained a long-standing relationship in the development and advancement of the Pediatric Environmental Health Specialty Unit (PEHSU) Program, through ongoing cooperative agreements initiated by ATSDR. The PEHSU serves health professionals, community organizations, governmental officials, federal staff, child-care providers, parents, and others having interest in environmental conditions that influence reproductive and pediatric health. Primary focuses of the PEHSU are to: (1) Build the capacity of primary care clinicians to recognize environmental exposure risks, provide risk reduction counseling, and deliver patient care to those at risk of or harm from acute or chronic exposures to hazardous substances in the environment. (2) Integrate environmental health content, placing emphasis on hazardous substances in the environment and related health effects, into pre-service clinical (i.e., medical, nursing, and allied health) course work; and primary care residency programs (i.e., clinicians in pediatrics, family medicine, and obstetrics and gynecology). (3) Increase patient and population awareness of environmental exposure risks and ways to reduce those risks. (4) Provide education and consultative services to community members, clinicians, state and local health departments, appropriate federal programs, and others involved in protecting children and couples of reproductive age from environmental threats. (5) Provide educational and consultative assistance in communities where ATSDR and/or Environmental Protection Agency (EPA) are addressing environmental contamination.

This NOFO aims to improve access to, coordination of, and continuity of health care for individuals with thalassemia or sickle cell disease which will lead to a decreased occurrence of transfusion-related complications, improved quality and increased length of life. These health care improvements will result from a multi-faceted approach that includes (1) identification of patients, families, communities, and providers who will benefit from increased knowledge about therapeutic transfusions and their potential complications, (2) development and dissemination of materials that will increase understanding of the diagnosis and management of these blood disorders, and (3) collection of biological specimens from individuals with thalassemia or sickle cell disease. This NOFO builds upon findings from the work completed as part of CDC-RFA-DD14-1406. In particular, (1) the difficulty in identifying any thalassemia patient in the U.S. and their health care provider prior to medical complications manifesting, other than those born in states where the condition is a part of the mandatory newborn screening panel, (2) the lack of standardized practices across, and sometimes within, clinical care settings and blood banks for transfusions of patients with thalassemia or sickle cell disease, (3) the need for dissemination of evidence- or consensus-based guidelines about best practices for transfusions, and (4) the room for improvement in increasing blood donation from communities most affected by thalassemia or sickle cell disease.

This notice solicits applications for fiscal year (FY) 2018 for a new 3-year initiative entitled Evidence-Informed Approaches to Improving Health Outcomes for People Living with HIV (PLWH). The purpose of this program is to support a single organization that will systematically identify, catalog, disseminate, and support the replication of evidence-informed approaches and interventions to engage PLWH who are not receiving HIV health care (i.e., individuals who are not currently in care or have never been in care) or are at risk of not continuing to receive HIV health care.

The American Cancer Society is accepting proposals for research projects with the potential to generate new knowledge of the effects of the United States healthcare system structure and the role of insurance in both access to and the outcomes of cancer screening, early detection, and treatment services. ACS is keenly interested in supporting rapid learning research on the effects of health policy changes on patients, providers, and health systems. This includes but is not limited to facilitators and barriers to care; unintended consequences; differential experiences and outcomes of patients seeking or receiving care; best practice models for quality care; and economic impact. Awards will not exceed $200,000 per year (direct costs) for up to four years. To be eligible, applicants must be affiliated with a nonprofit institution in the United States, one of its territories, or the Commonwealth of Puerto Rico; and be a U.S. citizen, non-citizen national, or permanent resident of the United States. Independent investigators at all stages of their career are encouraged to apply.
