STATworkUP - Medical App Journal - 1 views

  •
"STATworkUP™ has an immense amount of correlated data between symptoms, tests, diagnoses and treatments. While the app cannot be relied upon to thoroughly determine a differential diagnosis or provide an evidence-based summary of therapy, it can serve as an additional arrow in the clinicians quiver especially when wrestling with an uncommon diagnostic dilemma."

Clinical Gestalt - Reflective Interactive Medical Education | LinkedIn - 1 views

"Sharable free cases created by any author from any institution at any time. Simple non-linear navigation while authoring or learning. Traditional case components such as history (hx), physical examination (pe), etc. are clickable tabs. Flexible searches are created by authors. More keywords for less advanced learners, fewer keywords for more advanced learners Learner-built medical record created on the fly. Review is easy. Immediate feedback is provided by comparing the learner-assigned value with the author's value. Delayed feedback is provided during a final review and the summary. Sortable diagnoses forming a differential are created by the author and learner and compared. Non-linear navigation includes a "Tx" tab so problems can be treated while other data is gathered."

Virtual Interactive Case System (VICS): Perioperative Interactive Education (PIE), Toro... - 0 views

  •
"Welcome to the Virtual Interactive Case (VIC) system for creating simulations of encounters with patients in clinics. VIC cases are clinical reasoning exercises with feedback. Their role is to provide a bridge between theory and seeing patients in clinic (or ER), providing students with what Ericsson has called "deliberate practice" as a way of gaining clinical expertise. The strength of VIC is that it is optimized for rapidly creating a large number of cases, by using a patient template, and creating variations of cases with different differential diagnoses for the same presenting complaint."

Context matters: emergent variability in an effectiveness trial of online teaching modu... - 0 views

  •
"It was observed that each institutional context formed a complex educational ecology. The intervention became interwoven with different educational ecologies so that it could no longer be considered a stable variable across the study. We suggest that researchers should conduct implementation-profiling studies in advance of any intervention-based research to account for the constructing nature of educational ecologies on their interventions and in doing so to more clearly differentiate between efficacy and effectiveness studies."

Clinical reasoning - A guide to improving teaching and practice - 0 views

  •
By considering clinical reasoning as a skill to be learnt rather than a concept to be understood, a framework for teaching this skill can be developed. The learner initially observes a consultation by the teaching clinician, followed by the teacher explaining the reasoning processes used including hypothesising, hypothesis testing, re-analysis and differential diagnosis. The student then comments on the reasoning of the teacher in a subsequent consultation, followed by feedback from the teacher on the student's reasoning in a third consultation.

Toward the Construct Definition of Positive Deviance - 0 views

  •
In this article, the authors develop a definition of positive deviance, a foundational construct in positive organizational scholarship. They offer a normative definition of positive deviance: intentional behaviors that depart from the norms of a referent group in honorable ways. The authors contrast this normative perspective on deviance with statistical, supraconformity, and reactive perspectives on deviance. They also develop research propositions that differentiate positive deviance from related prosocial types of behaviors, including organizational citizenship, whistle-blowing, corporate social responsibility, and creativity/innovation. Finally, the authors offer some initial ideas on how to operationalize positive deviance.

WORKUP AND DIAGNOSIS OF A SOLITARY THYROID NODULE from Townsend: Sabiston Textbook of S... - 0 views

  • Rapid growth and signs of possible invasion, such as pain or hoarseness, are most suggestive, but not conclusive of malignancy
  • Clinical groups with the highest risk for malignancy in a thyroid nodule are children, males, adults younger than 30 or older than 60 years, and those exposed to radiation therapy, especially during childhood
  • history of specific endocrine disorders

  • Serum Tg has been reported as useful in predicting a well-differentiated carcinoma
  • Radioisotope Scanning
  • radionuclide scans allow assessment of thyroid function.
  • Technetium pertechnetate (99mTc) is taken up rapidly by the normal activity of follicular cells. It is trapped by follicular cells, but not organified. 99mTc has a short half-life and low radiation dose. Its rapid absorption allows quick evaluation of increased uptake (so-called hot) or hypofunctioning (so-called cold) areas of the thyroid.
  • 123I and 131I iodine scintigraphy is also used to evaluate the functional status of the gland
  • Advantages of scanning with 123I include a low dose of radiation (30 mrad) and short half-life.
  • 123I is a good choice for evaluating suspected lingual thyroids or substernal goiters.
  • 131I has a longer half-life (8 days) and emits higher levels of β-radiation. 131I is optimal for imaging thyroid carcinoma.
  • screening modality of choice for the evaluation of distant metastasis.
