As we transition in England through yet another NHS organisational change, I ask myself what does this mean for community pharmacy? I would like to think that this change will bring about opportunity and a chance for community pharmacy to showcase and continue the excellent work that was carried out during the height of the pandemic and is still ongoing today. I hope that it allows community pharmacy to be regarded as part of the NHS rather than sitting on the side lines. This change has to lead to better funding for community pharmacy, without sufficient funding we will see more pharmacies close. We are hearing a lot about winter pressures but this year it feels like all year round pressure. What I have seen, whilst the NHS is under such pressure, is North East London (NEL) CCG transitioning to an integrated care board (ICB) almost seamlessly. I have seen people transitioning into new roles, whilst working hard to ensure that all plates are still spinning, which at the moment is no mean feat. I spent a day out recently visiting pharmacies with the chief medical officer of NHS NEL, Dr Paul Gilluley. The visits were positive, we felt listened to and understood. The feedback was great, it was recognised that community pharmacy is often the informal front door to the NHS and that we have so much to offer in terms of ill health prevention. Community pharmacy can offer a total solution as long as we have the tools to do so, which can save so much time and money. An example is the GP CPCS service, which has launched well across NEL.

In its response to a Freedom of Information Request (FOI), the NHS Business Services Authority (NHSBSA) has confirmed a net decrease of 101 pharmacies in England - with 371 closures and 270 new contracts. Meanwhile, the General Pharmaceutical Council data projected a drop of 43 registered pharmacies in England, Wales, and Scotland for the year ending March 31, 2023, compared to the preceding period. The data, released by business advisor Christie & Co in its annual pharmacy market review on Sept. 5, revealed a total of 14,328 pharmacies across all four UK country regions as of March 31, 2023, indicating a 0.3 per cent decrease from the previous year. Corporate operators experienced a net reduction of 249 pharmacies, while the total number of independent operators remained consistent with 2022. According to the study, the corporate sector saw the most substantial shift, with a 13.7 per cent reduction in businesses operating 300 or more pharmacies. Following closely, groups managing 11 to 15 pharmacies experienced a 5.6 per cent decrease. Christie & Co attributed this shift in group size to operators acquiring additional pharmacies through corporate disposal opportunities throughout the year.

This year has been another turbulent year for the UK pharmacy sector. The sector continues to undergo significant transformations, driven by evolving healthcare needs, regulatory shifts, and economic pressures. For pharmacy owners, staying abreast of these trends is crucial to navigating the challenges and seizing opportunities. IQVIA, a leading global provider of clinical research services, commercial insights and healthcare intelligence, delves into the key issues, challenges, and developments shaping the sector, drawing insights from the latest industry data and reports, supported with data released by NHS England (NHSE). The community pharmacy landscape remains unsettled, with disruptive market dynamics at play. The decline in the number of pharmacies, particularly within the Managed Chains channel, underscores the volatility of the market. The number of active pharmacy contractors in England has dropped significantly, with more closures than openings each month, exacerbated by the exit of Lloyds Pharmacy from the UK market which completed at the end of 2023. In the year ending June 2024, there was a net closure of 440 sites within NHSE which equates to a 4% decline in available pharmacies1. The increase in closures from June 2023 was driven by the closures of Lloyds in Sainsburys and the ongoing divestment of the Boots estate.

The Pharmacists' Defence Association (PDA) is bolstering its leadership team with the appointment of Jay Badenhorst as the Director of Pharmacy, a newly created position aimed at fortifying its commitment to advancing pharmacy practice. This move comes amidst a period of significant change and challenge for frontline pharmacists, prompting the independent trade union to enhance its influence in the evolving healthcare landscape. Mr. Badenhorst, who previously served Whitworth Chemists as the Managing Director and also has leveraged his extensive knowledge of the pharmacy practice and organisational leadership as the Vice Chair of the National Pharmacy Association, reasons his "commitment to being at the forefront of positive change within the pharmacy profession." Mr. Badenhorst expressed his enthusiasm stating: "I am delighted to be joining an organisation that has so much vision, ambition and drive at a time when pharmacists have so many opportunities to develop their roles for the benefit of patients." According to Mark Koziol, Chair of PDA, the new position is a reflection of the union's commitment to pharmacy and described the new Director of Pharmacy as "a senior level, proven and award-winning pharmacy leader."

The pharmacy market in England has seen a significant transformation in recent years, driven by a trend of large-scale disposals by corporate operators. Jonathan Board, Director - Pharmacy, Christie & Co, has told Pharmacy Business that several corporate operators are looking to sell off their marginal or non-performing assets while independent operators are eager to acquire and enhance businesses further. Due to their scale, corporate operators are compelled to adopt a 'one size fits all' approach. However, Jonathan believes this puts them at a disadvantage compared to many independent operators who are better positioned to meet local patient and community needs, delivering a more personalised approach to patient engagement, and a variety of other services "As such, where corporate disposals have taken place, there has been a steady stream of independent operators eager to take over and develop the businesses further," he said.

Prescribing in pharmacies is an emerging opportunity, and I'm passionate about supporting proper governance arrangements for businesses. But does the governance and administrative burden reduce if your business is not doing the prescribing? Recently, I've come across several independent pharmacy businesses across the UK who have been approached by prescribing clinics to dispense medicines for their patients. While these partnerships can present lucrative opportunities, they also come with significant responsibilities and potential risks. Pharmacy owners and superintendent pharmacists must be fully aware of the guidance set by the General Pharmaceutical Council (GPhC) and ensure that all requisite checks are performed to maintain compliance and safeguard patient safety. The growing trend of prescribing clinic collaborations For one reason or another, private prescribing clinics are sprouting up all over the place, offering specialised services such as weight management, hormone therapy, or dermatological treatments. These are quickly becoming a prominent feature in our healthcare landscape. Many of these clinics lack in-house dispensing capabilities, leading them to seek partnerships with pharmacies to satisfy a growing number of consumers. In the current economic climate, these collaborations can offer a new revenue stream and an expanded patient base. However, with these opportunities come responsibilities that must not be overlooked.

Well Pharmacy has announced a new partnership with RangeMe, a product discovery, sourcing and purchasing platform, as it looks to increase the diversity of products available to its customers. The independent pharmacy chain said the partnership will provide new, emerging and diverse brands increased access to its buying teams. There are already over 200,000 suppliers registered on the RangeMe platform. "We pride ourselves on being at the heart of the communities we serve and are delighted to be working with RangeMe to increase the diversity of the products we are able to offer to our customers across the UK," Reena Takhar, head of OTC & NHS buying at Well Pharmacy said. "Given the ongoing cost-of-living crisis, we're keen to do all we can to give businesses of all sizes the opportunity to connect with our buyersto allow us to offer new and interesting great value products to our customers." RangeMe helps retailers and their buying teams scale product sourcing efforts with streamlined submissions, simplified discovery tools, and the industry standard digital sell sheet.

Community pharmacies will be able to deliver approximately 15 million blood pressure screenings by 2026, revealed the Company Chemists' Association (CCA)'s new research. This will also prevent 15,000 people from suffering heart attacks or strokes. CCA has called the Government to take up their offer and commission pharmacies as the first port of call for cardiovascular care. "Thousands of lives can be saved with ambitious commissioning and the investment to match," said CCA. There is an opportunity to use the expertise of community pharmacy to go further and provide the care these newly identified patients' need. To do this the sector needs funding and workforce. The association said: "Community pharmacy is under great financial pressure, suffering a real term funding cut of over 30% over 8 years. With funding, the sector can transform to deliver this critical need for patients. Whilst there are plans to train Independent Prescribers (IPs), there are no clear roles for them to use these skills. There is a need to accelerate training plans to allow pharmacists to deliver the service described here and play their part in CVD management."

The National Pharmacy Association has published a medium-term prospectus for the development of pharmacy services, calling it a challenge to old ways of thinking and an opportunity to redefine the sector's role in the NHS. It follows months of dialogue with NPA members about what a clinical future could look like for the sector. The document - Making Changes Meeting Needs - will be shared with the King's Fund and Nuffield Trust who are currently working on a new vision for the future on behalf of Community Pharmacy England. NPA chair, Nick Kaye, said: "Building out from the existing portfolio of services, there are some major opportunities within this decade, encompassing prevention, medicines optimisation, long term medical conditions and urgent care.

Titan PMR, a pioneer of cutting pharmacy technology has set up additional new team of 12 staff to cater for a deluge of independent pharmacists investing in new businesses. The company is utilising the opportunity that is underway as large chains look to sell off or close their community pharmacies, offering an opportunity for pharmacists looking to provide efficient dispensing and additional services. In January this year Lloyds Pharmacy, which was bought by a private equity firm in 2022, announced plans to close all its 237 branches inside Sainsbury's stores over the course of 2023. Currently, more than 5,500 of around 14,000 registered pharmacy premises across the UK are owned by corporates and supermarkets. The new team at the Bristol-based company, hired to support this sector, sees its total workforce growing to more than 50 staff.

An executive director of Day Lewis and also board director and member of Association of Independent Multiple pharmacies (AIMp) Jay Patel highlighted the key issues and on ground challenges faced by the community pharmacies in a meeting hosted by Prime Minister Rishi Sunak and health secretary Steve Barclay in Downing Street on Saturday (January 7) to tackle the NHS pressure. In his interview with BBC, he said: "He (prime minister) was very engaging and trying to understand the key issues and practical challenges on the ground." Patel described the meeting atmosphere as 'collaborative and proactive', and said, "people were genuinely asked their options and there were listeners too." Acknowledging publication of the Royal Pharmaceutical Society (RPS) Scotland's new professional vision for the future of pharmacy in Scotland - Pharmacy 2030, a Parliamentary Motion has been lodged in the Scottish Parliament. The vision, produced in collaboration with the National Pharmacy Technician Group Scotland, expects all pharmacists in a patient-facing role to become independent prescribers by 2030. The motion recognised the aim of the publication that by 2030 pharmacy teams will work together, using their expertise to make the best use of medicines and, will take a person-centred approach, providing care holistically rather than by clinical condition. It highlighted the important role that RPS would play by working with partners across health and social care to turn this vision into a reality.

Simon Harris bought an average-sized store from a national chain and turned it into a profitable business in just over a year - a great example of how to start up a pharmacy and become successful. His formula was quite simple - recognise the available opportunities, take some calculated risks and reap the rewards. Simon went into pharmacy because he felt it was a profession that would allow him to "help people" and have a direct impact on their lives. "I also thought it was a profession which would allow me to have my own business one day." Since qualifying as a pharmacist from Bath University in the late 90s, Simon did his pre-reg at a large Boots store in Broadmead, where we had some "great experience" as a pharmacist manager. He decided to take a year out and went around the world with his "now wife" and ended up working as a pharmacist for a pharmacy group Down Under which had a contract with the sports industry, i.e. Australian Open and the Australian Football League. "It was quite an experience because at the time I knew nothing about Aussie football but I ended up being their pharmacist after a short conversion course."

A leading pharmacist and member of the Royal Pharmaceutical Society (RPS) Scottish Pharmacy Board has been chosen to share her experiences of working in a community pharmacy in Scotland. Catriona Sinclair FRPharmS will speak at a roundtable entitled 'International Experiences in Professional Pharmaceutical Health Services' in Madrid on Thursday (May 5). Catriona, who owns and runs a pharmacy in the Highlands, is expected to give her take on community pharmacies' significant contribution towards achieving improved health outcomes and reducing health inequalities for local populations in Scotland. As an active independent prescriber, she is at the cutting edge of the profession in Scotland. Speaking ahead of her visit to Madrid, Catriona said: "I'm honoured to have the opportunity to collaborate with pharmacists from across Europe, and to share my experiences of working as a community pharmacist in Scotland.

Pharmacy sales specialist, Hutchings Consultants has revealed its market predictions for the next 12 months in a new report. The broker is anticipating that both independent group operators and corporate owners will continue to reassess their existing portfolios in 2024, making strategic disposals of varying sizes. "This is likely to result in transaction volumes remaining high for the foreseeable future," stated the report authored by Paul Steet, Associate Director, Hutchings Consultants. Community Pharmacy Contractual Framework negotiations will be the most notable opportunity for the sector to capitalise upon this year, the broker said, adding that it's "a belated opportunity for the sector to secure a vital uplift to the contract's fixed global sum."

The Health Committee's new report has reminded the government of a much-needed pharmacy workforce plan to support recruitment, training, and retention of staff. The report makes a number of recommendations to optimise workloads across primary care, reduce pressure on general practice and hospitals, and support the integrated care systems. It also recognises the potential of pharmacist independent prescribers to support patient care, backed by appropriate supervision, training, and opportunities for professional development. RPS England gave evidence to MPs on the Committee at a public hearing in May, highlighting key issues to support the pharmacy workforce. RPS director for England, Ravi Sharma, said: "Today's report underlines the urgent need for the Government to set out a comprehensive workforce plan for health and care. "It rightly recognises that boosting recruitment and retention, supporting staff wellbeing, fostering inclusion and diversity, and investing in education and training will be crucial to the future of the NHS.

Alitam Group, the 100-plus pharmacy store consolidation, has appointed Bharti Patel, the former group executive director of Avicenna to its executive board. Patel, who was part of the leadership team at Avicenna until late 2021, brings with her a wealth of experience covering pharmaceuticals, multiple retail and independent pharmacy, homecare and NHS primary and secondary care. In her early career, she worked at Lloyds Pharmacy, where she rose from a prescription medicines buyer to director of procurement over a period of eight years. She is known in the industry for her strong leadership, commercial acumen and ability to create opportunities for innovation and strategic partnerships. Alitam founder and CEO Feisal Nahaboo said in a statement that Patel's appointment will further strengthen the group's ability to deliver its transformative multi-billion-pound 'Pharmacy of the Future' concept.

Global consumer healthcare company Haleon has launched a new centre of excellence for a global leader in consumer health which will bring together world leading academics in human behaviour and frontline healthcare professionals including community pharmacists. Unveiled at the International Pharmaceutical Federation (FIP) Congress in 'sunny' Seville, Spain on September 20, the Centre will operate as a community of healthcare professionals and specialists in behavioural science, health psychology and the social sciences to solve some of the most pressing everyday health challenges. Named the Centre for Human Sciences (CHS) the initiative will be the first major programme for healthcare professionals since the Haleon's launch on July 18 as an independent, global leader in consumer health. The Centre's mission is to support practising health professionals - pharmacists, pharmacy assistants and dental professionals - in serving their patients and communities. Combining science with deep human understanding, CHS will bring expertise in physiology together with human sciences to deliver real world-solutions and tangible interventions, resulting in measurable improvements in health outcomes through sustained behavioural change. The Centre is facilitated by Robert Horne, professor of Behavioural Medicine at UCL School of Pharmacy, who started his career as a practising pharmacist but later chose to become a behavioural scientist when he saw an opportunity to address issues around psycho-social factors that acted as a hindrance in pharmacy practice. Centre for Pharmacy PostGraduate Education (CPPE) has announced the launch of a new learning programme, Preparing to train as an independent prescriber. The programme aims to build the confidence and competence of pharmacists who are planning to apply and train as independent prescribers and is open to pharmacists from all sectors of practice, including community pharmacists who are planning to commence their independent prescriber training with a Higher Education Institute (HEI). The course is made up of four main components, including a self-directed e-course and self-assessment framework, a mandatory online workshop to support you to review your self-assessment of competencies, an optional online workshop to support your readiness for academic writing and reflective practice, and optional peer support sessions, providing the opportunity for you to develop your professional network and establish communities of practice.
