The National Pharmacy Association (NPA) is calling on NHS England and its member to pause and reflect before further implementing the pharmacy contraception service. NPA board met on Tuesday (25 April) to discuss the Pharmacy Contraception Service and the board decided that - with the community pharmacy sector at breaking point - it cannot support the immediate roll out of this service. The association believes that with no new funding for the service, and all existing funds in effect already allocated to other pharmacy activity, any payments to the sector for delivery of the oral contraception service will ultimately be clawed back by NHS England. Tweeting after the meeting yesterday, the NPA said: "We can't tell pharmacy owners what they can and can't do. But we can tell them the facts; fact number one is that with no new funding currently available everyone will be a loser from the implementation of this service on the current terms." NPA Vice-Chair Jay Badenhorst added: "We cannot be expected to take on more and more services without the increase in funding necessary to deliver them effectively. Meanwhile, taking on additional work when current workload already exceeds capacity risks impacting negatively on the overall quality of care people experience in pharmacies. We still believe this could, in future, be a great new pharmacy service, but not without the extra funding necessary to deliver it safely and effectively. We want to offer women this extra support, but if it's worth doing it's worth doing properly."

The new board of the National Pharmacy Association (NPA) has adopted 10 principles for transforming the contractual framework in England. The board met for the first time in April, believes that the current Community Pharmacy Contractual Framework is failing NPA members, the wider sector, the NHS, Government and patients. The association's new vice chair, Jay Badenhorst, said: "We can't wait until the current framework limps to its finish line in 2024 before giving serious thought to the new race we must all run in the future." "Before negotiations for a new contract begin in earnest, we want to make our position clear to all of those who will be involved in its development. Years more of the same would be totally unacceptable. Tinkering at the edges of the current arrangements as the basis for a new deal could not achieve the transformation that is needed." Former chair of the NPA, Andrew Lane, listed some of the principles in a speech to industry leaders in January, but this is the first time the approach has been agreed in its entirety, following months of testing with NPA members.

The National Pharmacy Association (NPA) has commissioned Professor David Taylor of University College London to investigate the implications of all-time high inflation rates on community pharmacies in the UK. Professor Taylor's will assess rates of inflation affecting community pharmacy across the UK, using public data sources whilst examining inflated costs in the context of the current five year contractual framework in England (2019-2024). He would review the EY (Ernst & Young) report into pharmacy funding, to identify whether current inflationary pressures could change any of the findings and consider the policy implications and impacts of inflationary pressures, including pharmacy's ability to prepare for a more clinically focused future and maintain current core services. NPA chief executive Mark Lyonette said: "Inflationary pressures are eating into the limited funds provided by the NHS for pharmacy services. We believe the real level of inflation for pharmacy businesses could be higher than the CPI inflation rate, which itself is at a 40 year high. Staff and locum costs in the sector as well as medicines costs have risen dramatically.

BioNTech and Pfizer are to start tests on humans of next-generation vaccines that protect against a wide variety of coronaviruses in the second half of the year. Their experimental work on shots that go beyond the current approach include T-cell-enhancing shots, designed to primarily protect against severe disease if the virus becomes more dangerous, and pan-coronavirus shots that protect against the broader family of viruses and its mutations. In presentation slides posted on BioNTech's website for its investor day, the German biotech firm said its aim was to "provide durable variant protection". The two companies are currently discussing with regulators enhanced versions of their established shot to better protect against the Omicron variant and its sublineages. The virus' persistent mutation into new variants that more easily evade vaccine protection, as well as waning human immune memory, have added urgency to the search by companies, governments and health bodies for more reliable tools of protection. As part of a push to further boost its infectious disease business, BioNTech said it was independently working on precision antibiotics that kill superbugs that have grown resistant to currently available anti-infectives.

The British Generic Manufacturers Association (BGMA) has published a positioning paper which sets out the objectives that need to be delivered through the next Voluntary Pricing and Access Scheme (VPAS) on Thursday (15 June). The paper details how a financially sustainable VPAS can support widened medicines access to patients. VPAS is an agreement between the Department of Health and Social Care (DHSC), NHS England and The Association of the British Pharmaceutical Industry (ABPI). The scheme aims to limit increases in spending on branded medicines to no more than 2% per year via a rebate system which is charged on companies' sales revenues. Two years ago, the rate was 5.1% but for 2023 it has soared to 26.5%. Last year, the association had raised concerns over the rise in the VPAS rate for 2023 to 26.5 per cent. "The rocketing rate is in large part due to the growth in spend in on-patent medicines since 2019. Looking at the four completed years of the current VPAS scheme, data shows that the average annual growth rate for on-patent medicine sales value from 2019-22 was 18% compared to just 2% for off-patent products," said the association.

Medical sector has the third highest number of businesses currently for sale, with hospitality and retail technology firms most likely to sell up next year, according to the UK Business Investment Report. There are 4,300 registered medical businesses in the UK, and approximately 355 (one in every 12.11) are currently up for sale. Topping the list was the hospitality sector with one in every 9.88 businesses up for sale, ahead of retail technology businesses (1 in 10.13). At the bottom of the table were owners of agriculture businesses with just one in every 943 businesses up for sale. Transportation and software/IT business owners followed in a similar manner with one in every 802 businesses being up for sale.

Community Pharmacy Wales and the Welsh government have entered an agreement on Thursday (December 16), enabling all pharmacies to deliver provision of contraception, treatment for common minor ailments, and access to repeat medicines in an emergency. Expansion of clinical community pharmacy service will be supported with an increased funding of £20 million a year by April 2024 from the current level of £11.4m. The two bodies have been negotiating the terms of agreement, which also includes plans to roll out a Wales-wide pharmacy prescribing service, allowing trained pharmacists to treat an extended range of conditions that currently require GP visits. The agreement will allow patients to access NHS services closer to home which in turn would free up GP and other NHS services for patients with more complex needs. Eluned Morgan MS, minister for Health and Social Services said: "I welcome the positive approach to negotiations taken by Community Pharmacy Wales, embracing our ambition for a reimagined community pharmacy service, which is an integral part of a strong primary care landscape."

The regulatory amendments proposed will enable pharmacists to dispense medicines in their original packaging for private prescriptions starting this Autumn, according to the Department of Health and Social Care. In the recently published draft OPD regulations, the DHSC has indicated that the alterations, permitting pharmacists to vary the dispensed quantity by up to 10 per cent to avoid splitting medicine packs, will be implemented for private prescriptions 'immediately upon the enforcement of the Human Medicines Amendment Regulations in the autumn,' as highlighted in a briefing by Community Pharmacy England. CPE stated that the regulations related to NHS prescriptions will come into effect when the pharmaceutical terms of service regulations expressly apply the OPD amendments. Moreover, new directive mandating the dispensing of sodium valproate products solely in their original packaging (except when an assessment of risk necessitates an alternative approach) will align with the rollout of the private prescription regulations during the autumn. CPE further noted that these regulations are currently in draft, indicating that they are not currently in effect and may undergo revisions prior to their implementation.

The General Pharmaceutical Council is in the process of appointing five new council members within the next two years, coinciding with the conclusion of the current members' terms. The regulator aims to recruit five individuals from diverse professional and personal backgrounds for its governing council, the GPhC has said. "We are seeking three members to join us in April 2024 and two in April 2025," said GPhC in its job advertisement brief. "For the 2024 intake, two of the new council members must be lay members of the public, while one must be a Saxton . Moreover, at least one out of the three must primarily reside or work wholly or mostly in Wales. As for the new members commencing in 2025, one must be a lay member, and the other must be a registrant member." The members will be initially appointed for a three-year term, with the option to extend their position for a maximum of eight years, GPhC added. The council, which serves as the regulator's governing body, currently comprises 14 members. Half of the council consists of lay members, who bolster public confidence and infuse novel viewpoints into the GPhC, as highlighted by the regulator. "This council will play a pivotal role in shaping the regulatory body's future strategic goals," the regulator added. "These members must contribute positively to patients and society, ensuring the benefits of secure and efficient pharmacy care."

Pharmacy professionals to be included as key stakeholders in the implementation, delivery and evaluation of a wide range of genomic services, said the Royal Pharmaceutical Society (RPS). RPS's statement has been developed in collaboration with pharmacy organisations who have co-badged the report, such as the British Oncology Pharmacy Association, the UK Clinical Pharmacy Association, Association of Pharmacy Technicians and the College of Mental Health Pharmacy. It looks at current and future roles for pharmacy professionals in genomic medicine across many aspects of practice such as person-centred care and collaboration, professional practice, education, leadership, management and research. Pharmacists and pharmacy technicians in the UK have already established roles in the application of genomic medicine in some areas of practice, such as antimicrobial stewardship and infectious diseases, and the management of certain genetic conditions, such as cystic fibrosis. The society believes, the current role of pharmacy professionals in genomics can be expanded upon in the future to both lead and support many relevant aspects of genomic implementation. These are described across all healthcare sectors, within the Genome UK strategy produced by the UK Government, and within the implementation plans published in England, Scotland and Wales. The British Generic Manufacturers Association (BGMA) has sought a judicial review of the Department of Health and Social Care's (DHSC) decision to negotiate a new Voluntary Scheme for branded medicines with the Association of the British Pharmaceutical Industry (ABPI). Mark Samuels, Chief Executive of BGMA said: "The Government has decided not to involve the trade body representing these medicine suppliers in its negotiations on the voluntary scheme for branded medicine pricing (VPAS). "We are deeply concerned by this decision. It has left us no choice but to take legal action." "While not all generic drugs fall within VPAS, four out of ten products in the current scheme are branded generics or biosimilars. As the representative trade body for both generic and biosimilar UK manufacturers, we must play a full part in the VPAS negotiations for the next period of the scheme from 2024 to 2028." "The VPAS tax has risen five-fold in under two years, an unprecedented tax increase. Yet our sector currently has no input into the negotiations on future schemes or rates; this is untenable as any decisions made on VPAS could significantly define the future of our sector in the UK and its ability to supply the NHS. The association had raised its full participation in the negotiations with the Government last November.

The Company Chemists' Association (CCA) has published a 'pharmacy paradox paper' which outlines how community pharmacy can match the ambition amidst the workforce crisis. The sector is experiencing a workforce crisis, exacerbated by NHS recruitment of community pharmacists into primary care networks. Pharmacies are also facing a significant increase in demand on services. CCA said, "A stretched and reduced pharmacy workforce is having to deliver more and more. This is unsustainable." "Unless the tension between ambition and capacity is addressed, there is a risk that community pharmacies will not only not meet their potential, but the existing offer could be compromised." Therefore, the association has set out solutions to resolve the current paradox between ambition and reality.

The Pharmaceutical Services Negotiating Committee (PSNC) will launch a new influencing campaign to build wider political support for community pharmacies this week. The campaign aims to ensure that MPs and Peers across Parliament understand the risks to pharmacy services that the current pressures on the sector are causing. In a Parliamentary event held today (Tuesday 15 November) MPs and Peers heard from PSNC, LPC and other pharmacy bodies about the current situation. PSNC briefed them on the urgent need for action to address the pressures on the sector. The committee will be working with LPCs and contractors to organise a series of pharmacy visits for key Parliamentarians including Minsters, Committee and APPG Members. This is happening alongside work to develop a new vision and strategy for the community pharmacy sector, all of which is part of PSNC's strategy to lay the ground for future CPCF negotiations.

The Pharmaceutical Services Negotiating Committee (PSNC)'s members are seeking immediate rescue packages for the sector to help with energy bills and to ease capacity constraints. In a meeting held on 14th and 15th September, the committee members expressed their anger and frustration on the reluctance of NHS England and government to fund pharmacy sustainably. The meeting was held to consider practical steps to ease the pressures on the community pharmacy sector, and to oversee the progress of negotiations on the Community Pharmacy Contractual Framework (CPCF) and other work. Committee members, as pharmacy contractors, shared their experiences of the current pressures on all contractors, including the inability to deliver some services and to maintain core service levels; the capacity and workforce crises facing the sector; the critical need for funding support this winter; and the urgent need for Government to adapt the Price Concessions system to meet the needs of contractors. "The Price Concessions system is no longer working for contractors in the current volatile medicine supply environment and PSNC is clear this is not acceptable," said the committee.

The Pharmaceutical Services Negotiating Committee (PSNC) has announced the margin delivery rates will increase from October as the agreed £100m write off is implemented. "The baseline margin allowance will remain £800m per year in 2022/23 and 2023/24," said PSNC. "Margin levels in the last two full years (2020/21 and 2021/22), as measured by the Margin Survey of independent pharmacies, were in excess of the baseline allowance, meaning that an excess was accrued." As new services have been introduced and take up of these has accelerated, the amount of unallocated funding left in the CPCF is now less than it previously was. As such, the value of the Transitional Payments will decrease from its current level. Some of the unallocated funding remaining in 2023/24 will be repurposed into a Flat Payment for all contractors (see details below). Over the course of the second half of 2022/23, the value of the Transitional Payments will be phased down. This will start in October 2022 with a reduction in payment levels to approximately 85% of the current level, as shown in the following table:
