Community pharmacies were responsible for dispensing over 99 per cent of items in 2022/23, revealed General Pharmaceutical Services for Northern Ireland, Annual Statistics 2022/23 published on Thursday (29 June). Department of Health and Social Care (DHSC) has implemented a process to roll over certain concessionary prices to the following month. From May 2023, the roll over process will apply to any products where agreement is reached on the final prices granted by DHSC for any price concession requests submitted late in the month (on or after the 23rd of the month) by Community Pharmacy England. Community Pharmacy England said: "As part of the Year 4 & 5 funding settlement for community pharmacy, it was agreed that an urgent review of the price concession setting system would take place." It has been working with DHSC to determine improvements to the price concession system. Community Pharmacy England said: "Pharmacy owners (members) will be able to see the lines that have rolled over on our price concessions page here. Please note any rolled over prices can be adjusted upwards if we receive reports from our members to indicate suppliers' selling prices have increased. The Joint Committee on Vaccination and Immunisation (JCVI) on Thursday (May 19) said that an anticipated autumn Covid booster campaign would be aimed at people aged over 65, care home residents, frontline health and social care workers and all adults in a clinical risk group. The UK is offering a spring booster to the over-75s, care home residents and immunosuppressed people, and ministers have spoken openly of plans for a further booster campaign in the autumn. In interim advice, the JCVI stopped short of recommending another shot for all adults, though said the advice would be reviewed and updated. "The JCVI's current view is that in autumn 2022, a Covid-19 vaccine should be offered to: residents in a care home for older adults and staff; frontline health and social care workers; all those 65 years of age and over; and adults aged 16 to 64 years who are in a clinical risk group," the UK Health Security Agency said in a statement. Prime minister Boris Johnson, who was fiercely criticised for his handling of the early stages of the pandemic, lifted Covid restrictions in England in February, crediting Britain's quick initial vaccine rollout and the rollout of boosters with breaking the link between cases and deaths.

The Royal College of General Practitioners (RCGP) has warned of a potential 'mass exodus' of qualified GPs due to mounting work pressures. A recent RCGP survey found that over 40 per cent of GPs are unlikely to remain in general practice over the next five years, with a quarter stating it is "very unlikely." The situation is particularly concerning in England, with the highest rate of GPs considering leaving the profession reported to be in the East of England and the South East (47 per cent) and the lowest in the North West (36 per cent). Stress was cited as the main factor for leaving the profession, with more than half of respondents indicating that they find the job too stressful. Additionally, 13 per cent of respondents intend to leave the UK and work overseas. Almost 4 out of 10 GPs said they regularly worked more than 40 hours a week despite fewer than 1 in 10 being contracted to do so. The pressures faced by GPs are so severe that 22 per cent report feeling overwhelmed and unable to cope once or twice a week. The College noted that GPs and their teams have delivered an average of 30 million appointments per month in the past year - over 4 million more each month than in 2019 - despite the number of fully qualified, full-time equivalent GPs decreasing by 601 over the same period.

A snap survey conducted by the National Pharmacy Association (NPA) revealed that over half of community pharmacy teams have felt unsafe in the past week due to ongoing riots across the country. The survey, which included responses from approximately 350 community pharmacies, also found that one in three pharmacies had considered closing because of safety concerns, while 13 per cent of pharmacy teams reported experiencing racist comments during this period. According to the NPA, over 40 per cent of pharmacists are of South Asian, Black, or other ethnic minority backgrounds, with the majority of independent community pharmacy owners believed to be of South Asian heritage. The association reports that businesses had been vandalised during the riots, leading some pharmacies to reduce their opening hours or close entirely, following advice from the police and local authorities. Despite a recent decline in disorder over the last few days, police have been advised to remain on high alert over the weekend.

The Pharmaceutical Services Negotiating Committee (PSNC) has reminded Community pharmacy contractors to ensure that the correct flu vaccines are being used for each patient cohort. The pharmacies have also been asked to ensure that they recorded accurately in the patient's clinical record for the NHS Flu Vaccination Service. The move was followed by PSNC after the cases where the standard egg-cultured quadrivalent influenza vaccine (QIVe) was administered in the 65 years and over patient cohort. PSNC said: "On investigation, it appears that a number of these errors are due to data input errors; however, it is important to be careful when selecting a flu vaccine for this patient cohort as there is no provision for the use of the QIVe vaccine in patients aged 65 years and over in the NHS Flu Vaccination Service." The Joint Committee on Vaccination and Immunisation has advised it is not an effective intervention for patients aged 65 years and over.

The Royal Pharmaceutical Society (RPS) has published Pharmacy: Delivering a Healthier Wales that outlines a plan focusing on what can be achieved by end of 2025. The society has urged people to share their views by responding to the consultation on four key themes which were already set out in Pharmacy: Delivering a Healthier Wales -enhancing patient experience; developing the pharmacy workforce; seamless pharmaceutical care; and harnessing innovation and technology. "We want to hear from as many of you as possible. It is important that the aims for pharmacy over the next three years are right. So please take a look at the draft 2025 goals, the proposed activity and measures and complete our consultation response form by 11 September," said RPS. "We have been proud to continue to manage this important ambition for Wales, working on behalf of the Welsh Pharmaceutical Committee and with colleagues from all over Wales. Pharmacies are valuable healthcare assets, meeting increasing public demand. However, a substantial portion of their capacity is consumed by unfunded work. According to Community Pharmacy England's (CPE) 2024 Pharmacy Advice Audit, community pharmacies conduct over 1.3 million unfunded consultations each week, equating to 69 million annually. The average pharmacy completes around 22 of these consultations daily, each taking approximately 5.6 minutes of staff time -adding up to over two hours daily. This represents nearly 50 per cent more consultations than in 2020 and a 78 per cent increase in time spent on unfunded healthcare advice since the 2020 audit. The audit, which surveyed over a third (3,916) of community pharmacies in England earlier this year, also highlights that pharmacies handle nearly 150,000 informal referrals from GPs and NHS 111 each week. All these referrals could have been routed through the Pharmacy First service, which would have potentially secured over £115 million in funding for the sector, the CPE noted.

The Department of Health and Social Care (DHSC) has confirmed on Thursday (15 June) that everyone aged 60 and over will continue to receive free prescriptions. In response to a consultation launched by the DHSC in 2021 looking at whether the age free prescriptions become available should be aligned with the State Pension age, which is currently 66, the government decided to freeze the age at 60. Minister for Health Neil O'Brien said: "This government recognises the pressures with the cost of living caused by Russia's invasion of Ukraine. That's why we are providing cost of living help worth £3,300 on average per household over last year and this, among the most generous help anywhere in Europe. "It's also why we have decided to maintain the prescription exemption age where it is. Nine out of 10 prescriptions are already free at the point of use and on top of that we also provide financial help to others on low incomes who don't automatically get prescriptions for free. "Prescription charge exemptions will also remain in place for those aged under 16 or aged 17 and 18 but in full time education, while those in receipt of certain benefits can also receive free prescriptions," said DHSC. For those who do not qualify for an exemption or the NHS Low Income Scheme, prepayment certificates (PPC) are available to help with frequent prescriptions should they be required. The annual PPC can be paid for through 10 instalments. This means people can have as many prescription items as needed for just over £2 per week.

The National Institute for Health and Care Excellence (NICE) has recommended first new treatment - 'romosozumab' for osteoporosis for over a decade. Thousands of people in England and Wales with severe osteoporosis who are at high risk of fracture are set to benefit from a new treatment - romosozumab - after NICE published the final draft guidance. The Pharmacists' Defence Association (PDA) has raised concerns over "a prominent pharmacist's call to create a sector 'blacklist' for locums regarding disputes over rates." A few contractors took it to the Twitter to share screenshots of the messages by locums demanding more pay, which led to the call for 'blacklisting' those locums. "The motivation regarding the current talk of creating a blacklist seems intrinsically tied to hourly rates. Although there are occasional and isolated anecdotal reports on social media of alleged incidents of locums seeking higher rates than already agreed, these are far outstripped by reports of pharmacy businesses unwilling to negotiate and who do not want to pay the necessary rate to engage a locum and instead have set capped or fixed rates," said the PDA. It added that all parties should honour agreements they enter into, including a pharmacy's commitment to the NHS that it will open at set times to provide pharmaceutical services to patients and the public.
