PSNC:No mandatory clinical audit for pharmacy contractors - 0 views

    The Pharmaceutical Services Negotiating Committee (PSNC) has confirmed that the community pharmacy contractors are 'not required' to undertake a contractor-chosen or an NHS England determined clinical audit in 2023/24. "A clinical audit on anticoagulants is included in the Pharmacy Quality Scheme (PQS) 2023/24. If contractors choose to not participate in PQS, and therefore do not complete the anticoagulant clinical audit, there is still no requirement to complete two clinical audits in 2023/24," said PSNC. "The Department of Health and Social Care and NHS England have also committed to consider, during 2023/24, the removal of the requirement for a contractor-chosen clinical audit permanently."

    The National Health Service (NHS) has said that the roll-out of blood-thinning drugs has helped save thousands of lives. In January 2022, the NHS launched a drive to rapidly expand the use of life-saving direct oral anticoagulants (DOACs) in people who are at increased risk of strokes. Since then, more than 24 million prescriptions have been given to such patients and around 460,000 more people have started taking the drugs. This prevented an estimated 17,000 strokes and 4,000 deaths in the last 18 months, as revealed by the new NHS data. Amanda Pritchard, the NHS chief executive, hailed the "lifesaving NHS rollout" at the King's Fund annual conference. It is part of a major NHS drive on "prevention" - to catch more killer conditions earlier and save more lives.

While Apixaban (an anticoagulant) was the drug with the highest costs of £430 million. Apixaban 5mg tablets had the largest increase in cost between 2021/22 and 2022/23 in England, with an increase of £280 million. NHSBSA's annual statistics report 'Prescription Cost Analysis-England 2022/23' published also has revealed an eight per cent rise in the prescription items that were dispensed in the community in England. The report highlighted that in 2022/23, 1.18 billion prescription items were dispensed at a cost of £10.4 billion in the community in England, witnessing an increase of eight per cent from £9.69 billion in 2021/22.

    Recently launched generic version of Apixaban by Teva UK is said to bring savings to the NHS drug bill while making sure patients get the medicine they need. The generic apixaban is available for the prevention of stroke and systemic embolism in adult patients with non-valvular atrial fibrillation (NVAF), with one or more risk factors, such as prior stroke or transient ischaemic attack (TIA) and treatment of deep vein thrombosis (DVT) and pulmonary embolism (PE), and prevention of recurrent DVT and PE in adults. Previously there was only a 'branded' product available, but Castleford-based Teva UK succeeded in invalidating the apixaban patent and SPC (supplementary protection certificate) in the UK High Court and so is now able to launch its own 'generic' version - which will bring savings for the NHS while making sure patients get the medicine they need. "We've always said that we stand up for the patient", said Kim Innes, General Manager of Teva UK and Ireland. "The launch of generic apixaban emphasises Teva's commitment to doing the right thing by putting patients at the heart of everything we do by giving them and the NHS access to affordable treatments." Apixaban is an anticoagulant which directly inhibits factor X (factor Xa), inhibiting thrombin formation and the development of thrombi (blood clots). For at-risk patients, such as those with, or at risk for DVT, or NVAF, the risk of stroke related to blood clots forming in the body and traveling to the brain is a serious concern. Each year, DVT affects around 1 person in every 1,000 in the UK and if left untreated, about 1 in 10 people with a DVT will develop a PE.

    Atorvastatin was the most dispensed drug in England in 2021/22 with 53.4 million items, revealed the annual Prescription Cost Analysis (PCA) published by NHS Business Services Authority. According to the official statistics, Apixaban (an anticoagulant) was the drug with the highest cost of £401 million. The statistics revealed that the cost of prescription items dispensed in the community in England was £9.69 billion, a 0.87 per cent increase of £83.7 million from £9.61 billion in 2020/21. "The number of prescription items dispensed in the community in England was 1.14 billion, a 2.58 per cent increase of 28.7 million items from 1.11 billion in 2020/21." FreeStyle Libre 2 Sensor kit (a glucose monitoring system) was the presentation with the largest absolute increase in cost between 2020/21 and 2021/22 of £69.8 million, from £2.84 million to £72.6 million.

The audit is aimed to demonstrate how pharmacies support their communities every day and assist PSNC in gathering evidence for use in the funding discussions with the government and the NHS. It said that last year's Advice Audit provided compelling evidence for use in negotiations with the government and helped in persuading MPs to support the case for Covid-19 costs. PSNC said: "We would therefore be very grateful to anyone able to take part in this year's audit."
